Identification and characterization of intestinal lactobacilli strains capable of degrading immunotoxic peptides present in gluten.

AIM: Identify and characterize bacteria from the proximal gastrointestinal tract of pigs capable of degrading immunogenic gluten peptides. METHODS AND RESULTS: Bacteria were cultured from the small intestine of pigs fed a 20% gluten diet and from an enrichment media with the 18-mer peptide LQLQPFPQPQLPYPQPQL. Isolates were screened for the production of specialized proteolytic enzymes and the ability to degrade and remove metastable peptides from α-gliadin (16-mer and 33-mer) and ω-gliadin (17-mer), with established roles in the aetiology of coeliac disease. Degradation was determined by ELIS

Clostridium difficile colitis: review of the therapeutic approach.

Clostridium difficile infection (CDI) is the leading cause of antibiotic-associated and nosocomial infectious diarrhea. Presenting as clostridium difficile colitis, it is a significant cause of morbidity and mortality. Metronidazole is regarded as the agent of choice for CDl therapy and also for the first recurrence in most patients with mild to moderate CDI. Vancomycin is recommended as an initial therapy for patients with severe CDI. With recent Food and Drug Administration-approval fidaxomicin is available for clinical use and is as effective as vancomycin with lower relapse rates. Rifaximi

A review of drug therapy for functional dyspepsia.

The management of functional dyspepsia (FD) is a challenge for gastroenterologists in clinical practice. The eradication of Helicobacter pylori (H. pylori) and the utility of antacids, prokinetics and antidepressants are recommended as treatment choices for FD by consensus. Unlike in Europe and the USA, H. pylori eradication in Asia can lead to a higher proportion of FD patients with symptom relief and a higher symptom response rate to prokinetics. Moreover, response rates to anti-secretory drugs such as proton pump inhibitors deviate considerably from those in the Western world. Digestive enz

The role of the microbiome in rheumatic diseases.

There is a growing understanding of the mechanisms by which the influence of the microbiota projects beyond sites of primary mucosal occupation to other human body systems. Bacteria present in the intestinal tract exert a profound effect on the host immune system, both locally and at distant sites. The oral cavity has its own characteristic microbiota, which concentrates in periodontal tissues and is in close association with a permeable epithelium. In this review we examine evidence which supports a role for the microbiome in the aetiology of rheumatic disease. We also discuss how changes in

Constipation in childhood.

Constipation in children is an often long-lasting pediatric functional gastrointestinal disorder with a worldwide prevalence varying between 0.7% and 29.6%, and estimated health-care costs of US$3.9 billion per year in the USA alone. The pathophysiology of childhood constipation is multifactorial and remains incompletely understood; however, withholding of stools, starting after an experience of a hard, painful, or frightening bowel movement is the most common cause found in children. A thorough medical history and physical examination, including a rectal examination in combination with a bowe

The treatment of mice with Lactobacillus casei induces protection against Babesia microti infection.

In this study, we report that administration of Lactobacillus casei confers protection to mice against the intracellular protozoan Babesia microti. Mice treated with L. casei orally or intraperitoneally were inoculated 7 days later with an infectious dose of B. microti. Mice treated with lactobacilli showed significant reduction in the percentage of parasitized erythrocytes (PPE) compared to untreated mice. When mice were inoculated intraperitoneally with L. casei 3 or 0 days before challenge with B. microti, the PPE was significantly lower compared to untreated mice and there were no differen

Lactic acid bacteria and the human gastrointestinal tract.

OBJECTIVE: This review summarises the effects of lactic acid bacteria on lactose malabsorption, bacterial/viral or antibiotic associated diarrhoea, and describes the impact of lactic acid bacteria on cancer and the fermentative products in the colon. RESULTS: Eight studies (including 78 patients) demonstrated that lactase deficient subjects absorbed lactose in yogurt better than lactose in milk, while two studies (25 patients) did not support this. Two studies (22 patients) showed that unfermented acidophilus milk was absorbed better than milk, while six studies (68 patients) found no signifi

Faecalibacterium prausnitzii: Mechanistic Insights and Therapeutic Potential in Host-Microbe Crosstalk.

Faecalibacterium prausnitzii (Fp), a potential candidate for a next-generation, is widely distributed in the feces and intestinal tract of mammals. Accumulating evidence indicates that Fp and its metabolites exert anti-inflammatory, immunomodulatory, and maintain intestinal barrier integrity. In this review, we summarize the biological characteristics of Fp and its nutritional cross feeding interactions with other microbial species. We further synthesize current evidence on the local roles of Fp in intestinal diseases and systemic effects on distant organs via the gut-organ axis. Furthermore,

Not quite a cure yet: unlocking the unfulfilled promise of live biotherapeutics for disease treatment.

Few live microbial therapeutics have demonstrated conclusive or reproducible clinical efficacy. Cochrane meta-analyses of probiotic interventions across multiple clinical trials, analyzed using a rank-biserial correlation test, revealed that intestinal inflammation is negatively correlated with clinical responsiveness. This is exemplified by Escherichia coli Nissle 1917, which shows efficacy in maintaining remission comparable to frontline therapies, yet fails to demonstrate clear benefit in the treatment of active ulcerative colitis, an inflammatory bowel disease defined by chronic intestinal

Ruminococcus bromii alleviates constipation by pullulanase-driven resistant starch degradation and microbiota modulation.

Constipation is a prevalent gastrointestinal disorder associated with alterations in the gut microbiota. However, the potential microbial targets and their underlying mechanisms remain to be explored. Our analysis found the genus Ruminococcus was depleted in constipated patients. We then tested four species, R. bromii, R. torques, R. obeum, and R. gnavus, in constipated mice, finding all alleviated symptoms. R. bromii was most effective, with its pullulanase enzyme being key to degrading resistant starch. This degradation boosted short-chain fatty acid production and fostered beneficial bacter

The Microbiota-Gut-Brain Axis in Depression: Mechanisms, Microbiota-Targeted Interventions, and Translational Challenges.

Depression, a global mental health pandemic, persists with unmet therapeutic needs due to the limitations of conventional antidepressants. Emerging evidence suggests that the microbiota-gut-brain axis (MGB axis) is a crucial regulator of depressive pathophysiology, facilitating bidirectional communication between the gut microbiota and the central nervous system (CNS) through neural, immune, endocrine, and metabolic pathways. This review explores the complex mechanisms underlying MGB dysfunction in depression, including vagus nerve-mediated signaling, cytokine-driven neuroinflammation, and hyp

Advances in fungal vaccine development: Progress in the face of emerging challenges.

Historically overshadowed by bacterial and viral infections, fungal diseases are resurging globally, fueled by rising antifungal resistance and the increasing vulnerability of immunocompromised populations in healthcare settings. The widespread misuse and overuse of antifungals have accelerated the emergence of resistant fungal pathogens, complicating clinical management. Despite significant progress in vaccine development, with candidates such as NDV-3A and PEV7 advancing to late-stage trials, none have received FDA approval. Consequently, novel, more effective, and tolerable antifungal thera

A Lactobacillus plantarum strain isolated from kefir protects against intestinal infection with Yersinia enterocolitica O9 and modulates immunity in mice.

Lactobacillus plantarum C4, previously isolated from kefir and characterized as a potential probiotic strain, was tested for its protective and immunomodulatory capacity in a murine model of yersiniosis. The inoculation of BALB/c mice with a low pathogenicity serotype O9 strain of Yersinia enterocolitica results in a prolonged intestinal infection with colonization of Peyer's patches. Pretreatment with C4 was without effect on fecal excretion of yersiniae, but shortened the colonization of Peyer's patches. This protective effect was associated with pro-inflammatory status in the intestinal muc

[Gut translocation septicemia in infant].

Intestinal microbiotype necessary for life is a source of complications in childhood. Bacterial translocation is responsible of endogenous septicaemia and invasive complications. We report five cases of severe invasive infections associated with diarrhoea, digestive bleeding or sepsis. Biological parameters for inflammation are highly positive, and blood cultures reveal bacterial identification: salmonella enteridis, enterobacter cloacae, campylobacter jejuni, escherichia coli or clostridium difficile. We describe the predisposing factors and susceptibility status to develop translocation: inv

Inhibition of chlamydial infection in the genital tract of female mice by topical application of a peptide deformylase inhibitor.

Chlamydia trachomatis is an obligate intracellular bacterium responsible for a number of health problems, including sexually transmitted infection in humans. We recently discovered that C. trachomatis infection in cell culture is highly susceptible to inhibitors of peptide deformylase, an enzyme that removes the N-formyl group from newly synthesized polypeptides. In this study, one of the deformylase inhibitors, GM6001, was tested for potential antichlamydial activity using a murine genital C. muridarum infection model. Topical application of GM6001 significantly reduced C. muridarum loading i

Gastrointestinal consequences of cesarean section birth: A systematic review of short- and long-term effects in infancy and beyond.

BACKGROUND: Cesarean section (C-section) delivery rates are rising globally, raising concern about their potential long-term health consequences on offspring. Emerging evidence suggests that C-section, by bypassing the physiological and microbial exposures of vaginal birth, may adversely affect neonatal gastrointestinal (GI) development and immune regulation. AIM: To evaluate the short- and long-term gastrointestinal outcomes in infants born via C-section, compared to those born vaginally. METHODS: A systematic literature search was conducted across PubMed, Scopus, and Web of Science for stu
