Life expectancy and death by diseases of the circulatory system in patients with bipolar disorder or schizophrenia in the Nordic countries.

OBJECTIVE: Excess mortality from diseases and medical conditions (natural death) in persons with psychiatric disorders has been extensively reported. Even in the Nordic countries with well-developed welfare systems, register based studies find evidence of an excess mortality. In recent years, cardiac mortality and death by diseases of the circulatory system has seen a decline in all the Nordic countries, but a recent paper indicates that women and men in Denmark, Finland, and Sweden, who had been hospitalised for a psychotic disorder, had a two to three-fold increased risk of dying from a card

Hospital anxiety and depression scale. A quantitative analysis in medical outpatients, psychiatric outpatients and normal subjects.

Depressive and anxiety symptomatology represent the most common psychiatric manifestations that complicate the management and prognosis of patients with somatic disorders. The Hospital Anxiety and Depression Scale (HADS) is a reliable, valid andpractical screening tool for identifying and quantifying anxiety and/or depression in non-psychiatric out patients. The aim of the present study was to compare the psychometric properties of the HADS among internal medicine outpatients, psychiatric outpatients and the generalpopulation. The present study involved 264 subjects: 95 internal medicine outpa

Gender differences in subtypes of late-onset depression and mania.

BACKGROUND: It is currently not known whether elderly men and women present with different subtypes of depression and mania/bipolar disorder. The aim of this study was to compare the prevalence of subtypes of a single depressive episode and mania/bipolar disorder according to the ICD-10 for elderly men and women in a nationwide sample of all out- and inpatients in psychiatric settings. METHODS: All patients older than 65 years who received a diagnosis of a single depressive episode and mania/bipolar disorder in the period from 1994 to 2002 at the end of their first outpatient treatment or at

Factors associated with high use of public mental health services by persons with borderline personality disorder.

OBJECTIVE: The research presented here was a pilot study to identify clinical factors associated with high use (as opposed to lower use) of inpatient psychiatric services by persons with borderline personality disorder. METHODS: The initial sample was a random sample of English- and Spanish-speaking persons aged 18 to 60 years who had received at least one outpatient mental health service in the previous 90-day period and were enrolled in one of the participating mental health centers in King County, Washington. A random sample of persons who met selection criteria was randomly drawn; persons

Predicting Conversion From Unipolar Depression to Bipolar Disorder and Schizophrenia: A 10-Year Retrospective Cohort Study on 12,182 Inpatients.

Background: The initial stages of bipolar disorder (BD) and schizophrenia (SCZ) often exhibit depressive symptoms and syndromes, leading to potential misdiagnosis and treatment for unipolar depression (UD). However, no consensus exists on individualized and time-varying intervenable conversion predictors for both BD and SCZ. Methods: This study examined the rate of true conversion from UD to BD and SCZ, considering factors such as sex, family history of mental illness, psychotic features, recurrent depression, and treatment patterns. The objective was to develop predictive models for short-, m

Cortisol and Oxytocin Could Predict Covert Aggression in Some Psychotic Patients.

Background: The covert or indirect type of aggression has a risk of converting in violent acts and, considering that, it is very important to identify it in order to apply effective preventive measures. In cases of psychotic patients, the risk of becoming violent is harder to predict, as even neuter stimuli may be perceived as threat and trigger aggression. Treating all the psychiatric patients as potential aggressive subjects is not the best preventive measure as only a few of them are aggressive and this measure may further enhance the stigma on mentally ill patients. There is a current need

AVATAR therapy for auditory verbal hallucinations in people with psychosis: a single-blind, randomised controlled trial.

BACKGROUND: A quarter of people with psychotic conditions experience persistent auditory verbal hallucinations, despite treatment. AVATAR therapy (invented by Julian Leff in 2008) is a new approach in which people who hear voices have a dialogue with a digital representation (avatar) of their presumed persecutor, voiced by the therapist so that the avatar responds by becoming less hostile and concedes power over the course of therapy. We aimed to investigate the effect of AVATAR therapy on auditory verbal hallucinations, compared with a supportive counselling control condition. METHODS: We di

Reducing Caregiver Burden in First-Episode Psychosis: Effects of a Psychoeducational Pathway Led by an Advanced Practice Nurse-A Multicentre Stepped-Wedge Randomized Trial.

Family caregivers play a pivotal role in the recovery of people experiencing first-episode psychosis (FEP). However, they may suffer from high levels of psychological distress and lack access to structured support. Although international recommendations highlight the need for psychoeducational interventions for caregivers, their implementation in France remains limited. This study aims to evaluate the impact of a structured, multicomponent support pathway, coordinated by advanced practice nurses (APNs), for family caregivers of people with FEP. This multicentre, stepped-wedge (cluster with seq

Automated Analysis of Smiling Mimicry Across the Psychosis-Risk Continuum.

AIM: Early identification and support for individuals with psychotic-like experiences (PLEs) are essential for timely intervention and improved outcomes. Emotional mimicry, which refers to the automatic imitation of another person's emotional expression, such as a smile, is a fundamental social process that supports smooth interpersonal interaction and is known to be impaired in schizophrenia. However, its role across the broader psychosis-risk continuum remains unclear. This study examined smiling mimicry (the emotional mimicry of smiling) in individuals with PLEs and explored its relationshi

Correlates of Engagement and Associations With Outcomes in a Cannabis Harm-Reduction Mobile App for Youth With First-Episode Psychosis: Exploratory Analysis of the CHAMPS Pilot Randomized Controlled Trial.

BACKGROUND: Continued cannabis use among young people with first-episode psychosis (FEP) has been linked to poorer clinical and functional outcomes (eg, increased symptom severity and higher relapse rates). Digital harm-reduction interventions may represent a promising, person-centered approach to reduce at-risk cannabis use behaviors in this population. However, evidence remains limited regarding which subgroups are more likely to engage with these interventions and whether specific levels of engagement are required to achieve more favorable cannabis-related outcomes. OBJECTIVE: This explora

Different profiles of multi-dimensional symptoms and psychological characteristics and their related suicide behavior among college students with psychotic-like experiences.

BACKGROUND: Participants with psychotic-like experiences (PLEs) have higher suicide behavior (e.g., suicide attempts, suicide deaths), but it remains unclear which features influence suicide behavior in this population. This study aims to identify different PLEs subgroups based on multiple clinical symptoms and psychological characteristics relevant to suicidality, and examine their associations with suicide behavior. METHODS: Among 6390 college students at baseline, 2913 screened positive for lifetime PLEs. Latent profile analysis was conducted to identify different PLEs profiles based on ba

Psychotic-like experiences and cognitive biases in adolescents: birthweight and sex moderating effects.

INTRODUCTION: Genetic and environmental factors are involved in the pathogenesis of psychosis. Obstetric complications such as low birth weight are among the more extensively studied environmental risk factors for schizophrenia and the psychosis spectrum. Cognitive biases, such as "jumping to conclusions" (JTC), are also linked to psychosis risk. The study aimed to examine the occurrence of cognitive biases and psychotic experiences in adolescents and explore whether birth weight is associated with these variables, moderated by sex. METHODS: A total of 240 adolescents (aged 14-15) from Sabade

First-episode psychosis among people who are incarcerated: What is hidden behind the "prison psychosis" concept?

"Prison psychosis" is a historical concept that is still used in clinical practice, but little explored in research. In this short paper, we propose to revisit this concept by studying first-episode psychosis (FEP) in the prison population. We analyzed a series of incarcerated people who were hospitalized for a FEP from November 2020 to May 2021 in a French facility. We dated the onset of the first psychotic symptoms (PSs) in relation to the date of incarceration and of hospitalization and we calculated the duration of untreated psychosis (DUP). While the characteristics of our sample were con

Engagement, Disengagement and Re-Engagement in Mental Health Services Among Young Patients With First-Episode Psychosis: A Scoping Review.

INTRODUCTION: Engagement in mental health services among young patients with first-episode psychosis (FEP) is crucial for preventing relapse. We conducted this scoping review to establish the proportion and determinants of engagement, disengagement and re-engagement in outpatient mental health care among young patients from 13 to 35 years with FEP. METHODS: We used the guidelines for scoping review by the Joanna Briggs Institute (JBI) and the Arksey and O'Malley framework. We searched for published and unpublished studies guided by the inclusion criteria: studies published in English from 199

The trajectory of sedative adverse events caused by antipsychotics: a meta-analysis of individual participant data from randomised, placebo-controlled, clinical trials in acute phase schizophrenia.

BACKGROUND: Sedative adverse events are common in patients with schizophrenia undergoing antipsychotic treatment, which affects treatment adherence and the patients' quality of life. Although tolerance to sedation is believed to develop, robust evidence documenting the timing of sedation onset and resolution remains elusive. To address this gap, we aimed to assess the dynamics of onset and resolution of sedation across various antipsychotics in patients with schizophrenia. METHODS: In this meta-analysis, we included placebo-controlled, randomised controlled trials (RCTs) of antipsychotic mono

Exploring retinal thickness variations in adolescents with first episode psychosis and schizophrenia: A comparative study with healthy siblings and controls.

BACKGROUND: The retina offers a way to indirectly evaluate inflammation and degeneration in the brain. Recently, scientists are focusing on the retina as a valuable tool to understand brain structure and function. This study aims to compare retinal layer thicknesses of adolescents by using Optical coherence tomography (OCT), between first episode psychosis and schizophrenia patients, healthy siblings of schizophrenia patients and healthy control groups. METHODS: The study included 18 first episode psychosis (FEP) and 22 schizophrenia patients, 29 healthy siblings of schizophrenia patients and

Do antipsychotic medications work: An exploration using competency to stand trial as the functional outcome.

This study explores the effectiveness of antipsychotic medications in restoring competency to stand trial in individuals with severe mental illness, particularly psychotic disorders. While antipsychotic medications are known for reducing symptoms of psychosis, this research focuses on their ability to improve functional outcomes necessary for competency to stand trial (CST). Among over 3,000 patients in California's forensic state hospital system, 86.5% were successfully restored to competency, with 98.8% discharged on antipsychotic medications. Patients on antipsychotic monotherapy demonstrat…

Ictal Cotard delusion as a manifestation of nonconvulsive status epilepticus: A case report and commentary.

Psychosis of epileptic origin can present a wide range of cognitive and affective symptoms and is often underrecognized. Usually occurring in the inter- and postictal phase, epileptic psychosis is mostly related to temporal lobe epilepsy. Here, we describe the clinical presentation and diagnostic workup including routine EEG recording and brain MRI of a 63-year-old woman expressing isolated nihilistic delusions comprising belief of being dead and denial of self-existence. EEG showed an ictal pattern fulfilling the Salzburg criteria of nonconvulsive status epilepticus and brain MRI revealed ext
