A 50-year-old man presented with a 6-month history of localized alopecia on the left frontal area of the scalp. Biopsy revealed desmoplastic melanoma with associated neurotropism. Wide excision was performed and showed desmoplastic melanoma extending to excision margins. Hair follicles were focally reduced in number and were infiltrated by melanoma cells. He has subsequently required multiple wide excisions, exenteration of the left eye and radiotherapy. He has had multiple recurrences and has developed involvement of the first division of the trigeminal nerve intracranially. To the authors' k…

Steatocystoma multiplex (SM) is characterized by multiple, widespread cutaneous cystic lesions. These lesions can appear virtually anywhere on the body but are more common in areas where the pilosebaceous apparatus is well developed, such as the trunk (especially the presternal area), neck, axilla, inguinal region, scalp, and proximal extremities. We present a case of a woman with predominantly acrally located SM. To the best of our knowledge, a predominantly acral distribution has never been reported. We hypothesize that this patient's phenotype may be due to her genetic predisposition, possi…

Acrodermatitis enteropathica is characterized by eczematous and scaly plaques on the face, scalp, acral, and anogenital regions. In addition to typical lesions, unusual prominent vesiculobullous lesions are also described. We report a full-term, 9-month-old boy who has acrodermatitis enteropathica and Pseudomonas sepsis. In this patient there were clinical findings of sepsis and eczematous vesiculobullous lesions on the periorificial and acral areas. Serum zinc level was extremely low. Pseudomonas aeruginosa was identified in cultures of blood and fluid which was aspirated from the bullous les…

BACKGROUND: Magnetoencephalography (MEG) is a method of determining the brain activity noninvasively be detecting the magnetic fields associated with neuronal electrical activities.
METHODS: By using 37-channel DC-superconducting quantum interference devices, MEG activity was recorded in a patient with neurocysticerosis, who had a long-term history of epilepsy.
RESULTS: MEG clearly demonstrated accumulation of current dipoles originating from high-frequency waves around the cysticercal cyst, while scalp electroencephalogram failed to reveal paroxysmal discharge. Intraoperative electrocortico…

Pattern-reversal and diffuse flash visual evoked potentials (VEPs) were obtained from 4 normal adults. A spatiotemporal dipole model was used to determine the location of the hypothetical equivalent dipoles consistent with the scalp distribution of the VEPs. Equivalent dipoles representing ERG and VEP activity were placed within 3-D magnetic resonance images of the brain. Most of the localization error appeared to be due to inadequate sampling of the potential field in frontal and occipital areas by the 10-20 system of electrode placement. Locating electrophysiologic dipoles within magnetic re…

The present study describes the successful treatment of a 9-year-old girl with trichotillomania. Intervention involved praising attractive hair and ignoring hair loss, education in hair hygiene, response prevention for night-time hair pulling, and a variable schedule of monetary rewards for criterion hair growth. Progress was assessed by counting hairs in random scalp areas and measuring bald areas. Hair growth increased from three hairs per area sampled at baseline to such dense hair growth that individual hairs could not be counted and no bald spots were noted. Results maintained at 2, 6 and…

A 24-year-old woman had massive purulent and verrucous vegetations with fissuring in the groin and axillae. She also had scaling, crusted, bullous, and purulent lesions on the scalp, face, axillae, umbilicus, groin, and hands. Biopsy specimens of skin for histopathologic examinations and direct immunofluorescent study showed typical bullous pemphigoid, and serial serum samples tested by indirect immunofluorescent study confirmed the diagnosis. A basement membrane antibody was presented on four occasions at titers of 320, 1,280, 169, and 320. The differential diagnosis included dermatitis veget…

Cephalohematoma is a self-limiting lesion in 1%-2% of neonates. Cephalohematoma bleeds between the bone and periosteum and is limited to cranial sutures. Complications of the cephalohematoma include hyperbilirubinemia and secondary infection. We describe a neonate with cephalohematoma cultures positive for S. epidermidis as well as progressive deepening of the skull. There was no evidence of systemic infection, such as sepsis, meningitis, or osteomyelitis. In the ultrasound and brain MRI, the abscess was restricted to the scalp without involvement of the brain parenchyma and osteomyelitis of t…

Focal atonic seizures are recognized rarely as ictal phenomena, they can correspond to both generalized epilepsy and focal epilepsy. The areas of the brain involved in the management of this type of seizure are: the negative motor area and the primary motor and primary somatosensory cortices, although the neurophysiology that generates them is still unclear. We present the case of a patient with focal atonic seizures in the left upper limb, refractory to drug treatment. Neuroimaging was performed, a parietal cortical lesion was diagnosed. A scalp Video EEG and then a Stereo EEG was performed, …

Folliculotropic mycosis fungoides (FMF) is a particular subtype of mycosis fungoides (MF), characterized by an infiltration of neoplastic CD4+ T cells in the epidermis which can spread to all follicular structures, sebaceous glands, sweat glands and hair follicles. Clinically, FMF can exhibit various cutaneous symptoms. However, these symptoms often occur on the scalp, face and neck, which are rarely affected by conventional MF. We report cicatricial alopecia in a patient with FMF as alopecia lymphomatica. This peculiar symptom should be kept in mind as a critical differential diagnosis of sca…

Female androgenetic alopecia (FAGA) is a common cause of hair loss in women. Typically, it is characterized by a reduction of hair density on biparietal and vertex regions of the scalp with preservation of the anterior hair implantation line. However, it can also appear with recession of the anterior hairline known as FAGA with male pattern (FAGA-M). In these cases, it is important to consider a state of hyperandrogenism and look for other signs such as hirsutism, acne, and menstrual irregularities. A sudden onset in a postmenopausal woman must make us suspicious of an androgen-secreting tumor…

Brazilian keratin treatment (BKT) is a prevalent hair straightening method widely used by women in the world. The degree of concentration of formaldehyde safe for sensitive patients is still obscure. Report claims that these products contain intolerably high levels of formaldehyde. Yet, hair straightening products may lead to severe allergic contact dermatitis, by means of the responsible allergens such as formaldehyde and its releasers. This case report presents the case of a 37-year-old female patient developing extensive edema of the face and acute inflammatory changes in the scalp from che…

Trichilemmal carcinoma is a rare skin tumor that mainly occurs in the elderly (mean age, 71 years) and is localized in the repeatedly sun-exposed areas, most commonly on the face, scalp, neck, and dorsa of the hands. Its differential diagnosis is made with squamous cell skin cancer, clear-cell porocarcinoma, hidradenocarcinoma, and melanoma. The prognosis of trichilemmal carcinoma is most favorable than that of other skin tumors during radical removal. The paper describes a case of an 80-year-old man with long-standing trichilemmal carcinoma of the skin in the area of the shoulder joint, which…

Amicrobial pustulosis of the folds is a chronic relapsing neutrophilic dermatosis characterized by sterile pustules compromising skin folds, scalp, face and periorificial regions. It predominantly affects women. Demodicosis is an inflammatory disease associated with cutaneous overpopulation of the mite Demodex spp., the pathogenesis of which is not completely established, but is frequently related to local immunodeficiency. A case of a young woman with amicrobial pustulosis of the folds, and isolated worsening of facial lesions, is reported; investigation revealed overlapping demodicosis. Ther…

Transcranial electrical stimulation (tES) is a neuromodulatory technique in which low voltage constant or alternating currents are applied to the human brain via scalp electrodes. The basic idea of tES is that the application of weak currents can interact with neural processing, modify plasticity and entrain brain networks, and that this in turn can modify behaviour. The technique is now widely employed in basic and translational research, and increasingly is also used privately in sport, the military and recreation. The proposed capacity to augment recovery of brain function, by promoting lea…
