Contrasting perspectives on pain following hand injury.

The purposes of this article are 1) to compare contrasting ways of documenting pain, 2) to identify the usefulness of information each method provides, and 3) to examine implications for hand therapy. Participants are tracked for 12 months in an ongoing study of Adaptation to Hand Injury that incorporates quantitative measures of both intensity and functional impact of pain, as well as semistructured qualitative Adaptation Interviews that elicit and document patients' responses to pain and injury experience. Each method of documenting pain tells us something important. Numeric measures of pain

Analgesic Effect of Sinew Acupuncture for Patients with Soft-Tissue Injuries: A Pilot Trial.

OBJECTIVE: To investigate the immediate analgesic effect of sinew acupuncture for patients with soft-tissue injuries (STIs). METHODS: Two hundreds eligible adult patients suffering from STIs were recruited and received sinew acupuncture with flexible treatment schedules. The number of treatment sessions was pragmatically decided by each patient on the basis of their pain relief. The outcome measurement was the change of pain rating in the Visual Analogue Scale (VAS) during the first 5 sessions. The adverse effect was also observed. RESULTS: Of the 200 patients recruited, 7 were excluded due

Axillary nerve palsy after retrograde humeral nailing: clinical confirmation of an anatomical fear.

INTRODUCTION: Locked antegrade or retrograde nailing of humeral shaft and proximal humerus fractures is a well established treatment option. Anatomic-morphological studies revealed a potential high risk of axillary nerve injury within proximal interlocking screw insertion. However, clinical experiences do not seem to confirm this, as there is a lack of interlocking screw insertion associated axillary lesions in literature. CASE REPORT: We report about a 69-year-old man with a humeral shaft fracture (AO-type 12-A3) stabilized by a retrograde implanted interlocking nail. Proximal interlocking s

Functioning free muscle transplantation for brachial plexus injury.

Sixty-four cases of functioning free muscle transplantation to reconstruct brachial plexus injuries were reviewed for the years 1986 to 1991. Results were assessed using the Medical Research Council grading system, and success was defined as a muscle strength rating of 4 (ability to lift at least 1 kg). Functioning free muscle transplantation for shoulder abduction was ineffective because of its complex requirements. Functioning free muscle transplantation for biceps replacement using 2 or 3 intercostal nerves for innervation is a recommended procedure (success rate, to 80%). For function belo

Death by greyhound.

We present the case of a man who was last seen leaving to feed his greyhound dogs. He was found dead lying in a yard just outside the gate to one of the dog pens. Holes were present in the fence on both sides of the gate. The holes were of sufficient size to allow the passage of the head and neck of a greyhound to the shoulders. To open the gate one had to bend down to unfasten a latch 45 cm above ground level. Leaning down to unfasten the latch brought one into close proximity to the holes on either side of the gate. The scene and clothing appeared undisturbed. Autopsy (K.A.L.) revealed the c

Early mobilisation following mini-open rotator cuff repair: a randomised control trial.

This study compared the clinical outcomes following mini-open rotator cuff repair (MORCR) between early mobilisation and usual care, involving initial immobilisation. In total, 189 patients with radiologically-confirmed full-thickness rotator cuff tears underwent MORCR and were randomised to either early mobilisation (n = 97) or standard rehabilitation (n = 92) groups. Patients were assessed at six weeks and three, six, 12 and 24 months post-operatively. Six-week range of movement comparisons demonstrated significantly increased abduction (p = 0.002) and scapular plane elevation (p = 0.006) in

Locking Plate Fixation versus Nonoperative Management in Patients Aged ≥65 Years with Proximal Humerus Fractures: Clinical and Functional Outcomes from a Retrospective Cohort Study.

OBJECTIVES: This study aimed to compare the clinical and functional outcomes of geriatric patients (≥65 years) with proximal humerus fractures treated with locking plate fixation or nonoperative management and to identify factors associated with the development of complications. We hypothesized that fracture pattern, rather than treatment modality alone, would be more strongly associated with functional outcomes and complication risk in this population. METHODS: Patients aged 65 years and older who were treated for proximal humerus fractures were included in this retrospective cohort study. P

Axillary nerve palsy incidence after total shoulder arthroplasty with and without neuromuscular blockade.

BACKGROUND: Axillary nerve palsy is a rare but significant complication of shoulder arthroplasty. The axillary nerve lies just beneath the inferior glenoid, increasing susceptibility to traction and compression injures during surgery. Neuromuscular blocking agents are commonly used to facilitate exposure and dissection but may reduce the surgeon's ability to perceive excessive traction or tension on the nerve. The aims of this study are twofold, to (1) determine whether intraoperative paralytic use is associated with post-operative axillary nerve palsy and (2) assess its relationship with oper

Surgical management of complex humeral shaft fractures (AO/OTA 12-C): A multicenter comparison of open plating, MIPO, and intramedullary nailing.

BACKGROUND: The optimal fixation strategy for complex humeral shaft fractures, especially those with severe comminution, remains controversial. This study compared clinical and radiological outcomes after open reduction and internal fixation (ORIF), minimally invasive plate osteosynthesis (MIPO), or intramedullary (IM) nailing for AO/OTA type 12-C humeral shaft fractures. PATIENTS AND METHODS: This multicenter retrospective comparative study included 75 adults with AO/OTA type 12-C humeral shaft fractures treated surgically between 2015 and 2023. Patients were grouped by the fixation method,
