The syllable method: proportion of impaired syllables as an indicator of spasmodic dysphonia severity.

We have previously proposed the 'mora method' to evaluate the degree of impairment in spasmodic dysphonia (SD) in Japanese-speaking patients. With this method, impairment is judged as the proportion of impaired morae in a 25-mora sentence in a longer passage read aloud. As the mora is the phonologically isochronic unit in Japanese, the proportion of impaired morae in speech can be used to represent the temporal proportion of impaired Japanese speech. This proportional measure of impairment reflects the perceived severity of the impairment, and is a more reliable measure than a 4-point perceptu

[A 68-year-old man with speech disturbance as the initial symptom followed by bradykinesia and dementia. Clinical conference].

We report a 68-year-old man with progressive speech disturbance and dementia. He was well until 1995, when he noted an onset of difficulty in speech. He was able to name simple objects and understand language, however, he showed great difficulty in spontaneous speech. In 1998, he visited our service. He was alert and oriented, but he showed moderate degree of dementia. He did not appear to have aphasia but he showed marked dysarthria and slurred speech. He showed limb-kinetic apraxia in his right hand. He showed moderate restriction in his vertical gaze, masked face, and dysphagia. He walked n

The representation of articulation in the primary sensorimotor cortex.

Bilabial, alveolar and velar sounds are produced at different locations around the oral cavity. fMRI was used to localize cortical representation of articulation in seven healthy subjects during repetitive lip and vertical tongue movements as well as repetitive articulation of /pa/ (bilabial), /ta/ (alveolar), /ka/ (velar consonants) and a combination of all three in /pataka/. Centers of gravity (COG) analysis of fMRI activation in the primary motor (M1) and sensory cortex (S1) revealed /pa/- adjacent to lip representation and /ta/- articulation to tongue representation. The articulation of /p

[Central transmission time in children with the delayed language development].

The prolonged central transmission time (CTT) in ABR examinations in children with the delayed speech was described in 1992. Now after 4 years the examinations were repeated. The children with actually good speech revealed normalization of CTT, but in children with dyslalia and dyslexia this interpeak latencies were once again prolonged.

Standardized format for depicting hearing preservation results in the management of acoustic neuroma.

The Committee on Hearing and Equilibrium of the American Academy of Otolaryngology-Head and Neck Surgery recently published guidelines for reporting hearing preservation in the treatment of acoustic neuromas. These suggestions included pretreatment and posttreatment pure-tone hearing thresholds, word recognition scores, and hearing classification. We present a standardized reporting format that addresses the Committee's recommendations and displays individual patient audiologic data as a simple, concise plot of posttreatment hearing results. To illustrate the use of the recommended format, pre

Comparison of different voice samples for perceptual analysis.

BACKGROUND: Antiphospholipid syndrome (APS) is an acquired autoimmune disorder characterized by recurrent vascular events in large, medium, or small vessels. These events contribute to cardiovascular disease primarily through thrombosis and atherosclerosis (AS). Carotid atherosclerosis (CAS) represents a particularly high-risk manifestation of subclinical AS in patients with APS. However, the shared molecular signatures linking APS and CAS remain unclear. METHODS: Bulk transcriptome datasets from Gene Expression Omnibus (GEO) were analyzed to identify differentially expressed genes (DEGs) in

Reversible palsy of the hypoglossal nerve complicating infectious mononucleosis in a young child.

We report a 7-year-old boy with serologic evidence of active Epstein-Barr virus (EBV) infection who developed transient unilateral hypoglossal nerve palsy, with complete recovery within 21 days. This is, to our knowledge, the youngest reported patient with isolated hypoglossal nerve palsy in the context of EBV infection. Acute EBV infection should be considered early in the evaluation of children with twelfth nerve palsy in order to avoid extensive ancillary testing.

Acoustic correlates of dysphonia: type and severity.

The purpose of this study was to explore the acoustic discrimination and graded severity of three clinical voice types. Listeners classified 102 samples of dysphonic vowels /a/ and /i/ on the basis of voice types: breathy, hoarse, and strained. The vowels were analyzed acoustically with two measures of perturbation and 2 measures of spectral noise. Discriminant analysis showed that apriori, acoustic classifications of voice type were made with 92% accuracy using four acoustic parameters: (a) cepstral peak prominence (CPP), (b) jitter standard deviation (SD-J), (c) fundamental frequency (F0), a

Auditory performance and acoustic reflexes in young adults reporting listening difficulties.

We aimed to determine whether reported difficulties in speech understanding are associated with abnormal acoustic reflex thresholds (ARTs). The acoustic reflex has been shown to have a role in the understanding of speech at high intensities by ensuring that the strong low-frequency components of sound do not excessively mask the higher-frequency components, which are important for speech understanding. There is also wide variance in individual ARTs. Hence, the possibility arises that subjects reporting listening difficulties in noise have abnormal acoustic reflex function. In this investigatio

A model of practice for the management of velopharyngeal dysfunction.

This paper reviews normal speech production, the speech characteristics associated with velopharyngeal dysfunction, the investigations required and treatment options. A model of service delivery is proposed which has been in place over the past 4 years. This model highlights multidisciplinary management by a specialist team. An audit of the service has been undertaken. Of the 225 patients audited, some interesting findings are presented on aetiology, diagnosis and trends in management. Systematic and comprehensive collection of data on large numbers of patients permits meaningful audit of proc

Speech timing in apraxia of speech versus conduction aphasia.

This study examined temporal parameters of speech in subjects with apraxia of speech, conduction aphasia, and normal speech. They were asked to repeat target words in a carrier phrase 10 times. Acoustic analyses involved measurement of stop gap duration, voice onset time, vowel nucleus duration, and consonant-vowel (CV) duration. Speakers with apraxia of speech had longer and more variable stop gap, vowel, and CV durations than did subjects with aphasia or normal speech. Speakers with conduction aphasia had longer vowel durations and CV durations than subjects with normal speech. Also, subject

Auditory responsiveness of premature infants utilizing visual reinforcement audiometry (VRA).

The purpose of this investigation was to study the relationship between visual reinforcement audiometry (VRA) performance and age (i.e., corrected and mental) with 60 premature infants. VRA performance was classified as unacceptable, marginal, or acceptable based on conditionability and number of responses obtained before habituation to the task. The results indicated that mental age and corrected age were significantly related to VRA performance. It was found that a corrected age of 8 mo and/or a mental age of 6 mo is typically required for acceptable performance using VRA. A lack of responsi

Psychometric characteristics in patients with frontal lobe degeneration of non-Alzheimer type.

Twenty patients, selected from a longitudinal study of patients with presenile and senile dementia, underwent psychometric assessment including semi-structured observations of behaviour in the test situation and examination for dysphasia. The patients were chosen on the basis of post-mortem investigation and showed frontal lobe degeneration (FLD) of non-Alzheimer type in 16 cases and Pick's disease in four cases. The patients are described with reference to cognition, language function, behaviour and emotional reactions in the test situation. The test results were analyzed and compared with ps

Conductive hearing loss and speech development.

The central issue about conductive hearing loss and speech development is the impact of mild to moderate hearing loss that may be intermittent. Although opinions vary considerably about what this does to the ability of the child to learn, most studies support a position of concern. This concern should focus on identification and treatment of such children, with special teaching techniques in addition to medical and surgical treatment.

Training parents to use the natural language paradigm to increase their autistic children's speech.

Parents of four nonverbal and four echolalic autistic children were trained to increase their children's speech by using the Natural Language Paradigm (NLP), a loosely structured procedure conducted in a play environment with a variety of toys. Parents were initially trained to use the NLP in a clinic setting, with subsequent parent-child speech sessions occurring at home. The results indicated that following training, parents increased the frequency with which they required their children to speak (i.e., modeled words and phrases, prompted answers to questions). Correspondingly, all children

Hearing loss in the elderly.

Family physicians play an important role in detecting auditory impairment in the aged and in referring these patients for audiologic evaluation. Although most elderly patients with impaired hearing do not benefit from medical or surgical treatment, aural rehabilitation is often successful. Acceptance of a hearing device is critical to rehabilitation.

Intrinsic laryngeal muscle activity in a spastic dysphonia patient.

EMG activity from four intrinsic laryngeal muscles (thyroarytenoid, posterior cricoarytenoid, interarytenoid, and cricothyroid) was obtained from one female spastic dysphonia patient while she performed a variety of speech and nonspeech tasks. These tasks were performed before and during a period of temporary unilateral laryngeal muscle paralysis. In the nonparalyzed condition, adductory muscle activity showed intermittent sudden increases that coincided with momentary voice arrests. These muscle patterns and accompanying voice interruptions were not present either when speech was produced in

Recurrent laryngeal nerve pathology in spasmodic dysphonia.

Since it was first described in 1871, spasmodic (spastic) dysphonia has been considered a disease of psychogenic origin. Unsupported theories of possible organic etiology have appeared sporadically in the literature. In 1976 sectioning of the recurrent laryngeal nerve for patients with this disease was reported with resultant improvement in voice production. This was attempted because the spasmodic dysphonic has, in effect, already compensated vocal cords bilaterally. It was reasoned, therefore, that if one of these was paralyzed the patient would immediately be converted to a state approximat
