Can memories kindle nonconvulsive behavioral seizures in humans? Case report exemplifying the "limbic psychotic trigger reaction".

We present three hypotheses-(1) the limbic psychotic trigger reaction (LPTR) is a form of nonconvulsive behavioral seizures (NCBS), (2) kindling may occur in the LPTR, and (3) kindling may occur with memory stimuli-and report a case that may exemplify a LPTR kindled by memory and triggered by light and smell. The LPTR has a primate model, in which NCBS are kindled by intermittent exposure to actual subthreshold stimuli. In humans, we propose that such triggering stimuli can be revived by memory alone. Thus, individualized stimuli can trigger partial limbic seizures or seizure-like bizarre epis

Drop attacks and epileptic syndromes.

1. In addition to 4 types of drop attacks previously described by Ikeno et al., we could ascertain the falls of the myoclonic-tonic type in genuine drop attacks. The genuine drop attacks were seen in Lennox-Gastaut syndrome, epilepsy with myoclonic-astatic seizures and other symptomatic generalized epilepsies, and never in partial epilepsies. 2. None of the patients with partial epilepsy revealed genuine drop attacks as defined. Their drop attacks were slower (i.e. the falls were completed in more than 1 sec.). 3. In the 15 patients with Lennox-Gastaut syndrome, the drop attacks disappeared in

Effect of isoproterenol on serum potassium and magnesium.

Some ventricular arrhythmias can be related to a decrease in the level of potassium (K) and/or magnesium (Mg). Because adrenergic stimulation decreases serum K+ and Mg++, we decided to investigate the effects of a beta-receptor agonist, isoproterenol, on serum K+ and Mg++, and their consequences on the induction of tachycardia. Programmed atrial and ventricular stimulation was performed in 95 patients before and during infusion of 1.6 micrograms.ml-1 of isoproterenol. During isoproterenol infusion, 61 patients had no inducible tachycardias (group I) and 34 had inducible sustained tachycardias

[Functional autonomic blockade in the diagnosis of functional sinus node disorders].

The influence of the autonomic nervous system on sinus node automatism was assessed in 61 patients with suspect sinus node dysfunction. Cardiac frequency and corrected sinus node recovery time (CSNRT) were determined before and after functional autonomic blockade with intravenously administered propranolol (0,2 mg/kg) and atropine (0,04 mg/kg). A pathologic CSNRT was found in 59% of patients before and in 54% after autonomic blockade. In 44% pathologic CSNRT occurred during both conditions. In these patients intrinsic sinus node disease can be assumed. In 15% of patients an initially pathologi

Prevalence of indications of alcohol and drug use among patients treated for injurious falls by Emergency Medical Services in the USA.

OBJECTIVE: The association between alcohol consumption and increased injuries from falls is well established, but there is a lack of data on … hest, at 30.3%, in the age group 21-64 years, for falls occurring on streets and sidewalks, without syncope or heat illness as contributing factors. Reported substance use involvement was more frequent for men compared with women for each location type. Glasgow Coma Scale scores indicative of moderate or severe trauma were more prevalent among falls invol …

Bioenergetics of the combat sports brain: Between risk and resilience.

Combat sports provide a unique human model in which cerebral ischaemia-reperfusion stress, adaptive neuroprotection, and impact-induced neurodegenerative risk coexist. Across striking and grappling disciplines, exercise-induced metabolic and redox stress, repetitive head impacts and transient cerebral ischaemia-reperfusion during vascular neck restraints expose the brain to competing adaptive and injurious stimuli. Cerebral ischaemic preconditioning (cIPC) research demonstrates that brief, sublethal reductions in cerebral blood flow (CBF) activate a conserved hormetic programme involving modul

Usefulness of anesthetic block of the greater occipital nerve in the treatment of migraine in children and adolescents.

INTRODUCTION: Peripheral nerve blockade (PNB) is a common treatment in adults with headache. Although there is little evidence in pediatrics … das and PedsQL scales, the greater the effect, the better the response. Only 2 had adverse effects (syncope and local pain). CONCLUSIONS: NOM blockade is a safe, effective and useful procedure for the acute and preventive treatment of migraine in pediatric age. It relieves the burden associated with headache and is associated with an improvement in the q …

Ventricular conduction is a marker for arrhythmic risk in SCN5A-E1784K overlap sodium channel disease.

AIMS: SCN5A-E1784K (c.5350G>A) is the most common variant associated with the long QT (LQTS) and Brugada syndromes (BrS). It can manifest va … atal cardiac arrest, and documented sustained VT or VF. 'Cardiac events' were defined as arrhythmic syncope plus any lethal events. Clinical characteristics and ECG parameters corrected for age were investigated for association with lethal and/or cardiac events. Fourteen (6%) subjects experienced a lethal event and 45 (19%) a cardiac event. PR interval an …

Cardiac arrest secondary to mitral annular disjunction: a case report.

BACKGROUND: Mitral annular disjunction with bileaflet mitral valve prolapse is increasingly recognized as a substrate for malignant ventricular arrhythmia and sudden cardiac arrest. Prior myocardial fibrosis adjacent to papillary muscles and frequent ventricular ectopy increase arrhythmic risk. CASE SUMMARY: We describe the case of a woman in her 40s who suffered an out-of-hospital ventricular fibrillation cardiac arrest with immediate bystander CPR and ambulance arrival within 5 min. Return of spontaneous circulation occurred approximately 30 minutes after multiple shocks. She had a backgrou

Simultaneous transcatheter aortic valve replacement and thoracic endovascular aortic repair by the transcarotid approach for aortic stenosis with chronic type-A aortic dissection.

UNLABELLED: Frail patients presenting with ascending aortic dissection and aortic stenosis (AS) are usually treated with a minimally invasive approach. Although transcatheter aortic valve replacement (TAVR) is becoming the standard treatment for AS, reports of catheter-only treatment for patients with AS complicated by chronic type-A aortic dissection (TAAD) are scarce. We present a case of a patient who underwent simultaneous TAVR and thoracic endovascular aortic repair (TEVAR) using a transcarotid approach. An 86-year-old woman was hospitalized for syncope due to severe AS. Computed tomograp…

Toward a hyperventilation detection system in freediving: a proof of concept using force sensor technology.

BACKGROUND AND AIM: Hyperventilation before breath-hold diving (freediving) is widely accepted as a risk factor for hypoxic syncope or blackout (BO), but there is no practical way to address it before dives. This study explores the feasibility of using a force sensor to predict end-tidal carbon dioxide (P ETCO2) to assess hyperventilation in freedivers. METHODS AND RESULTS: Twenty-one freedivers volunteered to participate during two national competitions. The divers were instructed to breathe normally and perform three dry apneas of 1, 2, and 3-min duration at 2-min intervals in a sitting pos…

Isolated splenic tuberculosis presenting with spontaneous rupture in an immunocompetent adolescent: a case report from an endemic area.

INTRODUCTION AND IMPORTANCE: Tuberculosis (TB) remains a major global health concern, particularly in developing countries, where its extrapulmonary forms present diagnostic challenges. Splenic involvement is rare and can lead to life-threatening complications if not promptly recognized and managed. CASE PRESENTATION: We describe the case of an 18-year-old Ethiopian male with a history of childhood TB who presented with severe left upper quadrant pain, progressive weight loss, fever, night sweats, and recurrent syncopal episodes. Despite initial intravenous antibiotic therapy, symptoms persis
