A Genetic Marker Associated with De Quervain's Tenosynovitis.

De Quervain's tenosynovitis is a repetitive strain injury involving synovial inflammation of the tendons of the first extensor compartment of the wrist. It is relatively common in the general population, and is the most common radial-sided tendinopathy seen in athletes. Identifying a genetic marker associated with de Quervain's tenosynovitis could provide a useful tool to help identify those individuals with an increased risk for injury. A genome-wide association screen was performed using publically available data from the Research Program in Genes, Environment and Health (RPGEH) including 4,

Epidemiology of ankle and foot overuse injuries in sports: A systematic review.

Studies regarding ankle and foot overuse injuries are quite diverse in research methodology, data reporting, and outcomes. The aims of this systematic review were to analyze the methodology of published studies regarding ankle and foot overuse injuries in different sports disciplines and to summarize epidemiological data of ankle and foot overuse injuries. Four electronic databases, PubMed (MEDLINE), EMBASE, CINAHL, and SPORTDiscus(®) were systematically searched up to June 2011. A total of 89 articles on 23 sports disciplines were included in this review. Soccer, running, and gymnastics were

Use of latissimus dorsi perforator flap to facilitate simultaneous great toe-to-thumb transfer in hand salvage.

Severe crushing and degloving injury of the hand often involves multiple tissue structures requiring several stages of reconstructions. This is more challenging when tissue defect and loss of thumb co-exist, requiring vascularised tissue transfer and toe-to-thumb reconstruction in a hand that has severely compromised vascularity. The management variables to be considered include the use of pedicled or free flap, simultaneous or staged procedures, consideration for early rehabilitation and flap selection to facilitate simultaneous free tissue transfers without further jeopardy to the vascularit

A prospective randomized comparison of patellar tendon versus semitendinosus and gracilis tendon autografts for anterior cruciate ligament reconstruction.

Seventy patients with patellar tendon or hamstring tendon autografts for single-incision anterior cruciate ligament reconstruction were evaluated at least 2 years after surgery. All reconstructions were performed by the same surgeon, and metal interference screws were used for fixation of all grafts. No significant differences were noted between groups for Lysholm score, reduction in activity, KT-1000 arthrometer findings, quadriceps muscle size, return to sports, or ability to jump and do hard cuts and pivots. Significantly more patients in the patellar tendon group had patellofemoral pain at

[Differential anterior interosseous nerve syndrome diagnosis].

The differential diagnosis of the rupture of flexor pollicis longus tendon and profundus tendon to index finger to the interosseus anterior nerve syndrome can be difficult and can lead to misinterpretation of the clinical impression. Two cases are reported to demonstrate this problem. In the first case a spontaneous rupture of flexor pollicis longus was found, when first an interosseus anterior nerve syndrome was suspected. In a second case surgical exploration of flexor pollicis longus tendon and profundus tendon to index finger was done on the assumption of a rupture, that revealed intact te

Common foot and ankle injuries in dance.

Classical ballet is a popular but physically demanding activity. Minor injuries become increasingly common as the dancer encounters the greater workload of professional dancing. The doctor must have a basic knowledge of ballet technique and an understanding of the mental approach of dancers to accurately diagnose and effectively treat their injuries. The practice of dancing on the toes, the exaggerated turn out of the feet, and the extreme flexibility of the hips and spine all lead to unusual injuries. With informed conservative treatment, most ballet injuries will heal, but surgery is occasio

Foot and ankle injuries in theatrical dancers.

The theatrical dancer is a unique combination of athlete and artist. The physical demands of dance class, rehearsal, and performance can lead to injury, particularly to the foot and ankle. Ankle sprains are the most common acute injury. Chronic injuries predominate and relate primarily to the repeated impact loading of the foot and ankle on the dance floor. Contributing factors include anatomic variation, improper technique, and fatigue. Early and aggressive conservative management is usually successful and surgery is rarely indicated. Orthotics play a limited but potentially useful role in tr

What Is the Clinical Efficacy of Combined Semitendinosus Autograft Reconstruction and Scar Tissue Repair for Chronic Irreparable Patellar Tendon Rupture?

BACKGROUND: Chronic patellar tendon rupture is a rare but serious injury that can lead to significant functional impairment if not treated effectively. Traditional repair methods often result in rerupture because of tendon defects, adhesions, and poor tissue quality. Although tendon graft reconstruction is the first-line treatment, the role of combining the remaining scar tissue with tendon grafting in improving patient-reported outcomes has not been fully explored. QUESTIONS/PURPOSES: (1) What improvements in patient-reported outcomes and radiographic results were observed after reconstructi

The Lesser Tuberosity Osteotomy Exposure for Total Shoulder Arthroplasty.

BACKGROUND: The lesser tuberosity osteotomy (LTO) is a commonly employed technique for mobilizing the subscapularis tendon during anatomic total shoulder arthroplasty that is performed through a deltopectoral approach. During this procedure, the lesser tuberosity is osteotomized from the proximal aspect of the humerus while maintaining the strong tendon-to-bone attachment of the subscapularis tendon insertion. After the shoulder arthroplasty is performed, the lesser tuberosity osseous fragment is then resecured to the proximal aspect of the humerus with heavy nonabsorbable suture, which allows

Muscle fibers are injured at the time of acute and chronic rotator cuff repair.

BACKGROUND: Rotator cuff tears are a common source of shoulder pain and disability. Even after surgical repair, some patients continue to have reduced function and progression of fatty degeneration. Because patients with chronic cuff tears often experience muscle shortening, it is possible that repairing the tendon to its anatomic footprint induces a stretch-induced muscle injury that could contribute to failures of the repair and perhaps ongoing pain. QUESTIONS/PURPOSES: We hypothesized that, compared with acutely torn and repaired muscles, the stretch that is required to repair a chronicall

Overuse injuries of the upper extremity in baseball.

The shoulder and elbow are prone to many overuse injuries in baseball. Injury to the muscles or ligaments leads to pain and loss of effectiveness in competition. Although many of these disorders respond to conservative management, surgery is sometimes necessary to provide pain relief and restore function. Unfortunately, overuse injuries that require surgery still are career-threatening injuries. Correct diagnosis and treatment increase the probability of a throwing athlete returning to competition.

Simultaneous Bilateral Quadriceps Tendon Rupture After Low-Energy Trauma in a 73-Year-Old Man: A Case Report and Literature Review.

Simultaneous bilateral quadriceps tendon rupture is an exceptionally rare injury of the knee extensor mechanism and may present a diagnostic challenge, particularly after low-energy trauma. Prompt recognition is essential to restore knee extension and ambulatory function. We report the case of a 73-year-old man who presented to the Emergency Department after falling while descending stairs and landing directly onto both knees. He developed immediate bilateral knee pain and inability to ambulate. Clinical examination revealed palpable suprapatellar defects bilaterally, inability to perform stra

Decoding imaging in ankle impingement: a comprehensive review.

Ankle impingement syndromes comprise a heterogeneous group of osseous and soft tissue pathologies caused by abnormal contact between articular or peri-articular structures, resulting in chronic pain and restricted motion. They are anatomically classified into posterior compartment (posterior proper and posteromedial), anterior compartment (including anterior proper, anterolateral and anteromedial), and extra-articular lateral hindfoot impingement (ELHFI). Posterior impingement is most commonly related to os trigonum or a prominent Stieda process, with MRI demonstrating marrow edema and posteri

Study of bone-tendon interface healing in an animal model using a synthetic scaffold and PRP.

PURPOSE: Biological matrices have been used to reinforce large rotator cuff tear repairs. However, rapid resorption and initial immune reactions presented challenges in clinical practice. This study evaluates whether a resorbable synthetic matrix (scaffold), used alone or with platelet-rich plasma (PRP), impacts repair processes at microscopic, ultrasound, and biomechanical levels in a rabbit model of induced tendon-bone interface injury. METHODS: An experimental study was performed on 24 rabbits. Two experimental groups (n = 12 each) and a control group (n = 24) were defined. In the first gr

Novosorb Biodegradable Temporizing Matrix for Reconstruction of Complex Upper-Extremity Wounds.

PURPOSE: Dermal matrices can be used in management of upper-extremity wounds to create vascularized wound beds in the setting of exposed bone or tendon. Early outcomes of Novosorb biodegradable temporizing matrix (BTM) demonstrated success when used in the treatment of complex wounds. We hypothesize that BTM is effective for reconstruction of upper-extremity wounds. METHODS: A retrospective review was performed for patients who underwent reconstruction of upper-extremity wounds with BTM between January 2017 and May 2022. RESULTS: In total, 51 patients (39 males and 12 females) were included.

How Institution of the Sup-ER Protocol in a Clinic Changed Procedure Patterns in Upper Brachial Plexus (Erb's Type) Birth Injuries.

BACKGROUND: For children with upper brachial plexus birth injury (BPBI; C5, C6, ±C7 roots), most clinics first recommend nonsurgical treatment followed by primary and/or secondary surgical interventions in selected patients. Since 2008, we have used an infant shoulder repositioning protocol (supination-external rotation [Sup-ER]) designed to prevent shoulder internal rotation contracture and its potential effects on the shoulder joint. This study characterizes our clinic's current choice, number, and timing of primary and secondary procedural interventions (including Botox) and compares Sup-ER
