Subclinical Achilles tendon structural and pathology changes in asymptomatic amateur marathon runners: a correlational and multivariate MRI study.

OBJECTIVE: To investigate the morphological characteristics and pathological changes of the Achilles tendon in asymptomatic amateur marathon runners using high-resolution MRI and to identify the key demographic and running-related factors that influence these structural and pathology changes. METHODS: Forty-eight asymptomatic amateur marathon runners (39 males, median age 38.0 years) underwent ankle 3.0T MRI. Quantitative parameters of the dominant leg's Achilles tendon-including thickness, volume, cross-sectional area (CSA), and length-were measured. MRI findings suggestive of Achilles tendi

A New Accessible Six-Strand Flexor Tendon Repair Technique: A Four-Group Comparative Study on a Porcine Model.

Background: Six-strand flexor tendon repairs offer superior biomechanical strength, yet commonly used techniques often require looped sutures or proprietary materials that limit global accessibility. The Elsaftawy repair technique was developed as a six-strand construct using universally available braided polyester, aiming to provide high strength without reliance on specialty sutures. Methods: Sixty porcine digital flexor tendons were randomized into four groups: Elsaftawy 6-strand, Jin Bo Tang 6-strand, FiberLoop®, and modified Kessler 4-strand. Core repairs only were performed by a single e…

Isolated avulsion of the short head of the biceps tendon from the coracoid insertion: A case report and narrative review of the literature.

Isolated injuries to the short head of the biceps (SHB) are uncommon, and the existing literature mainly includes case reports documenting tears at the muscle belly or myotendinous junction. This case report presents the first reported SHB tendon injury at the coracoid insertion in a 34-year-old healthy male patient after trauma while playing with his daughter. The injury occurred with the arm forced into hyperextension, abduction, and external rotation, with the elbow fully extended. Diagnosis was confirmed by MRI, showing an isolated insertional tear of the SHB tendon with intact coracobrach

Genetic determinants of tendon and ligament injuries in athletes: a narrative review from single variants to gene-gene interactions.

Tendon and ligament injuries are common in athletes and can result in significant functional impairment. Genetic factors are increasingly recognized as key contributors to individual susceptibility. A structured narrative literature search was conducted in PubMed, Scopus, Web of Science, and Google Scholar to identify studies investigating genetic polymorphisms associated with tendon and ligament injuries in athletic populations. Eligible studies published in English and involving elite, professional, recreational, or physically active individuals were narratively synthesized. This review synt

Peroneus longus versus hamstring tendon autografts in arthroscopic anterior cruciate ligament reconstruction: A randomised controlled study.

BACKGROUND: Hamstring tendon autograft is widely used for anterior cruciate ligament (ACL) reconstruction, but concerns persist regarding graft size, harvest duration, and donor-site morbidity. Peroneus longus tendon (PLT) autograft has emerged as a potential alternative. This study compared PLT and hamstring tendon (HT) autografts in arthroscopic ACL reconstruction. METHODS: A randomised controlled study was conducted at a tertiary orthopaedic centre. Adults with ACL rupture were allocated to HT (n = 81) or PLT (n = 81) autograft. Primary outcomes included graft harvest time and graft dimens

Undivided tibialis anterior tendon with simultaneous insertion into the medial cuneiform and the base of the first metatarsal: a previously unclassified variant.

INTRODUCTION: The tibialis anterior tendon (TAT) typically divides into two slips inserting onto the medial cuneiform and first metatarsal. This report describes an unusual undivided TAT inserting simultaneously onto both bones. DESCRIPTION: During dissection of a 74-year-old Korean female cadaver, an undivided TAT was observed crossing the first tarsometatarsal joint and attaching to both the medial cuneiform and first metatarsal. The total attachment width was 18.9 mm, with 8.68 mm (45.9%) on the medial cuneiform and 10.22 mm (54.1%) on the first metatarsal. The contralateral side showed th

The rectus femoris tendon autograft in knee ligament reconstruction: Anatomy, biomechanics, clinical applications and results-A narrative review.

The quadriceps tendon has gained significant momentum as a graft source for knee ligament reconstruction, with the isolated harvest of its superficial layer-the rectus femoris tendon-rapidly emerging as a popular technical refinement. This shift has been driven largely by practical considerations: the cost and limited availability of proprietary instrumentation promoted for partial quadriceps tendon harvesting, combined with the appeal of a familiar, hamstring-like workflow using standard tendon strippers. As a result, numerous surgical technique descriptions have appeared in recent years. How

Impact of Proximal Conjoint Tendon Injury on Return to Play in the BF-ST Complex: A Prospective MRI-Based Study.

Background/Objectives: Proximal hamstring injuries involving the biceps femoris-semitendinosus (BF-ST) conjoint tendon (CT) often exhibit delayed healing, yet the prognostic significance of CT involvement and intratendinous injury morphology has not been fully clarified. This study aimed to determine whether full-layer CT injury, particularly bilateral involvement in Zone C, prolongs return-to-play (RTP) in competitive rugby athletes. Methods: This prospective study evaluated 41 university rugby players with acute BF-ST complex injuries using clinical examination, ultrasonography, and MRI. Inj

Achilles Tendon Rupture Following Extracorporeal Shockwave Therapy for Chronic Tendinopathy Despite Symptomatic Resolution: A Case Report.

Extracorporeal shockwave therapy (ESWT) is widely used in the management of chronic Achilles tendinopathy (AT) and is associated with improved pain and functional outcomes. However, symptomatic recovery may not necessarily reflect restoration of tendon structural integrity or mechanical resilience. We report the case of a 61-year-old active male with a ten-year history of chronic mid-portion AT refractory to conservative management who underwent six sessions of radial ESWT, resulting in complete pain resolution and return to unrestricted sporting activity. Six months after completing treatment

A novel tension-slide double intramedullary cortical button technique for distal biceps tendon reconstruction: a case series.

BACKGROUND: Complete distal biceps ruptures are primarily treated with direct repair; however, patients with chronic ruptures, poor tissue quality, or tendon deficiency may need to be treated with reconstruction. Prior studies have shown dual intramedullary cortical button fixation has the highest load to failure and unicortical fixation decreases the risk for injuring the posterior interosseous nerve intraoperatively. The tension-slide technique aids in tensioning the distal biceps tendon repair and significantly decreases bone-to-tendon gap formation. The aim of this study is to present the

Investigating the crosslinking of an aligned, all-natural chitosan-gelatine-cellulose based polymeric scaffold for tendon tissue engineering.

Crosslinking is a key step in the production of stable all-natural polymeric scaffolds for tissue engineering, as it slows the degradation and increases the mechanical properties of the material. In this study, we investigate the crosslinking parameters of a natural, anisotropic scaffold produced from gelatine, chitosan and cellulose through a modified freeze drying protocol, with the goal of maintaining the porous architecture of the scaffold while improving its degradation and mechanical strength. Genipin and EDC were selected as the two crosslinking alternatives, while crosslinker concentra

Central microglia-adenosine-related signaling impairs bone-tendon interface repair via sympathetic ADRB2 activation.

Bone-tendon interface (BTI) injuries pose a major clinical challenge because surgical repair often fails to restore the native enthesis and its structural and mechanical integrity. Although local repair mechanisms have been extensively studied, whether central neuroimmune circuits contribute to BTI healing remains unclear. Here, using a murine rotator cuff injury model, we provide evidence that BTI injury engages a sensory-central-sympathetic regulatory axis that contributes to impaired repair. BTI injury activated sensory afferent signaling and was associated with microglia-mediated neuroinfl
