Is obligatory bed rest after lumbar puncture obsolete?

After lumbar puncture (LP) an epidural CSF leakage caused by delayed closure of a dural defect leads to a decrease in CSF pressure. The resultant venous dilatation as well as downward shift of the brain with traction on pain-sensitive blood vessels and nerves frequently evokes post-lumbar puncture headache (PLPH), when the patient assumes the upright position. In previous studies differing opinions have been expressed about the prophylactic value of the posture taken by the patient after LP. The present study was designed to evaluate the benefit of the decrease of hydrostatic CSF pressure on t…

Lateral cervical advancement flaps for the correction of webbed-neck deformity.

In the webbed-neck deformity, a horizontal excess of cervical skin creates bilateral and often asymmetrical skin webs from the mastoid to the acromion. Hair extends laterally to the free edge on the posterior web surface, creating a wide nuchal hairline. A technique of correction is presented. Through an incision along or within the hairline, the glabrous anterior web surface is undermined with the platysma muscle into the anterior cervical triangle until posterosuperior traction will obliterate the web. The posterior hair-bearing web surface is also elevated, and an excess of scalp is excised…

Halo-thoracic brace immobilization in 188 patients with acute cervical spine injuries.

The authors report 188 patients with acute cervical spine injury with fracture who underwent Halo-thoracic brace immobilization. The majority of the fractures were considered unstable. Early neurological assessment revealed 24 patients without neurological deficit. There were 164 patients with associated cervical cord injury; 84 patients with incomplete, and 80 patients with complete tetraplegia. Management consisted of skull traction and application of the Halo-thoracic brace about 1.3 weeks after admission. The average radiological union time was 11.5 weeks following a mean of 10.2 weeks of …

Clinical and research aspects of subtotal open-sky vitrectomy. XXXVII Edward Jackson Memorial Lecture.

Subtotal open-sky vitrectomy can be used to improve visual function in some eyes for which closed vitrectomy is useless. Open-sky vitrectomy … ed in 61 eyes (21%). The greatest number of successes (16 of 30 cases [53%]) was achieved in severe tractional retinal detachments. Such results are encouraging because these eyes were considered inoperable by other techniques. Open-sky vitreous surgery is performed through a corneal incision, but a scleral incision over the pars plana may become the metho …

The cervical myelopathy associated with rheumatoid arthritis: analysis of patients, with 2 postmortem cases.

The clinical findings in a group of 32 rheumatoid patients with symptomatic myelopathy are described. The myelopathy appeared in late middle … on. Medical or conservative therapy was not effective, and the preferred approach was use of a halo traction device followed by posterior fusion, with or without laminectomy. Pathologically, in 2 autopsy cases, the maximal change had occurred in the central gray matter and adjacent posterior and lateral columns. We postulate that direct pressure caused int …

Ehlers-Danlos Syndrome: Arthroscopic Management for Extreme Soft-Tissue Hip Instability.

PURPOSE: To present outcomes in a series of patients with Ehlers-Danlos syndrome (EDS)-hypermobility type who underwent hip arthroscopy for associated hip pain and extreme capsular laxity. METHODS: A retrospective chart review identified 16 hips with confirmed EDS--hypermobility type that underwent hip arthroscopy for continued pain and capsular laxity. All patients had complaints of "giving way" and pain, an easily distractible hip with manual traction under fluoroscopy, and a patulous capsule at the time of surgery. No patient had osseous evidence of acetabular hip dysplasia or prior confir…

Apical and intermediate anchors without fusion improve Cobb angle and thoracic kyphosis in early-onset scoliosis.

BACKGROUND: The main goal of treatment in early-onset scoliosis is to obtain and maintain curve correction while simultaneously preserving spinal, trunk, and lung growth. This study introduces a new surgical strategy, called the modified growing rod technique, which allows spinal growth and lung development while controlling the main deformity with apical and intermediate anchors without fusion. The use of intraoperative traction at the initial procedure enables spontaneous correction of the deformity and decreases the need for forceful correction maneuvers on the immature spine and prevents p…

Three-dimensional volume changes of the palatal vault and pharyngeal airway following facemask therapy and maxillary expansion: a prospective comparative study.

BACKGROUND: This study evaluated three-dimensional volumetric changes in the palatal vault and pharyngeal airway following facemask therapy (FM) with or without slow maxillary expansion (SME) in late mixed and early permanent dentition. Additionally, it compared dimensional changes in the palatal vault, including its length, height, and width. MATERIALS AND METHODS: The sample included 26 patients (16 females, 10 males), aged 10 to 14 years (mean age: 11.88 ± 1.18 years) with Class III malocclusion who had completed facemask therapy, either with or without slow maxillary expansion. Age-dependent effects of Delaire facemask therapy for class III malocclusion : Impact on maxillary sutures and palatal morphology.

BACKGROUND AND AIM: Treatment effects of a combined rapid maxillary expansion (RME) and Delaire facemask (DFM) therapy have so far only been scientifically investigated through cephalometric analyses. The combination of cephalometric, dental cast and cone-beam computed tomography (CBCT) scan analysis was not yet used for investigating morphologic changes of the tooth-bearing palate. The aim of the present study was to determine whether patient age at treatment begin has an influence upon palatal length changes after RME/DFM therapy, and to what extent transverse palatal sutures contribute to t

A five-year follow-up of a root fracture in a ten-year-old boy.

The management of horizontal root fracture still presents a challenge to clinicians. The case of a 10-year-old boy with a horizontal root fracture in the left maxillary central incisor (tooth number 21) is described here. The fracture was present at the junction of the apical and the middle thirds. We report both the emergency care and the 5-year follow-up. At the same time, an orthodontic treatment was planned for a retained maxillary cuspid (tooth number 23), first composed of a high-pull headgear and then by a fixed multi-bracket appliance. Today, the tooth number 21 remains vital and sympt
