The identification of the gastric bacterium Helicobacter pylori was a significant advancement in the treatment of peptic ulcer disease. H. pylori infects the gastric mucosa and its eradication is associated with the prevention of ulcer recurrence. The significance of this finding is that bacterial infections can be treated and cured, offering hope to individuals with peptic ulcers. During 1993, four consecutive adult male patients underwent emergency surgery for free perforation of nonspecific sigmoid colon ulcers. All four patients presented with peritonitis and in each patient the Hartmann's procedure was performed. In each patient there was a single perforation but two of the patients presented with multiple erosions and ulcerations, and the other two- with solitary ulcers. BACKGROUND: The course of gastric ulcer disease and its relations to certain indicators of inflammation and Helicobacter pylori in the gastric mucosa were examined in a 10-year follow-up.
METHODS: Thirty-three patients with gastric ulcer were examined endoscopically with biopsies in 1981-82 and invited for re-examination in 1991-92. Twenty-one of them were able to comply with the invitation. A gastroscopy with biopsies was performed on each of these, and the results were compared with those of age- and sex-matched control patients with non-ulcer H. pylori-positive gastritis.

Pharmacological suppression of gastric acid secretion has traditionally been the most rational approach to healing ulcers successfully. However, ulcers initially healed using antisecretory therapy have a tendency to relapse after treatment is withdrawn. This tendency is altered definitively by eradication of Helicobacter pylori. During 19 years (from 1974 to 1992) 5205 operations for complicated ulcer disease of duodenum (4201 observation), stomach (915) and of double localization (89) were conducted. In 585 of 5205 patients complications occurred: of operative wound healing-in 17.4%, in the region of intervention--in 56%, dysfunction of other organs and systems--in 26.6%, 44 (0.84%) of them died. The mortality after conduction of stomach resection according to Bilroth-I was 0.8%, according to Bilroth-II--5.5%, after vagotomy--1%.

The results of surgical treatment of 800 patients with ulcer gastroduodenal bleeding, in 122 of whom diabetes mellitus was diagnosed, were analyzed. Organpreserving operations combined with vagotomy were conducted to the whole of the patients, the elaborated in clinic individualized approach to the diabetes mellitus prophylaxis was used. The glucose-correcting therapy methods were depicted with the respect to diabetes mellitus form, hemorrhage severity and indications for the operation.

Duodenogastric (D-G) reflux is commonly associated with peptic ulcer disease and chronic gastritis. To determine D-G reflux in association with each disease entity, we studied 43 chronic gastritis patients and 21 peptic ulcer patients with cholescintigraphy. Scoring (0 to 3+) was judged by the amount of Tc-99m diethylacetanitanilido-IDA (EHIDA) in the stomach. A 12-year-old boy, operated on for remitting abdominal pain and hematochezia, had an ileal duplication, with Helicobacter "gastritis" under a chronic peptic ulcer in the ectopic fundic gastric mucosa. We suggest the Helicobacter infection might have played a role in the pathogenesis of the inflammation and the peptic ulceration in this rare location.

The authors investigate the relevance of ulcer clinical variants (recent ulcer, of 10 years history and 2-3 annual exacerbations, chronic ulcer with aggravations and incomplete remissions) to production of endogenic PGE by gastric mucosa and to blood PGE level. It is shown that with ulcer progression PGE synthesis activization is replaced by its inhibition. A reverse relationship appeared between gastric juice and blood PGE and duration of ulcer. Etiopathogenetic heterogeneity is suggested in the group with complicated ulcer.

Ninety patients with endoscopically proven duodenal ulcer were followed for 1 year. All patients were initially treated with an H2-receptor antagonist. Those with more than two relapses within 1 year received triple therapy consisting of colloidal bismuth subcitrate, amoxicillin, and metronidazole. Results showed that 27 patients were symptom-free after one full course of H2 antagonist, 47 had one or two clinical recurrences, and 16 patients had more than two recurrences. Several studies have shown that cigarette smoking affects duodenal ulcer (DU) recurrence.
