A 46-year-old white female underwent a left hemipelvectomy for chondrosarcoma. She presented with total incontinence and a bulging vaginal mass. Exam confirmed complete transurethral bladder eversion that was addressed with transvaginal multilayer bladder neck closure and suprapubic tube placement. Eventually she underwent abdominal hysterectomy, mesh sacral colpopexy, and catheterizable stoma creation. Patient is continent of urine 3 months postoperatively. We present the first reported case of bladder eversion after hemipelvectomy and propose possible pathophysiologic mechanisms.

Cesarean hysterectomy for placenta previa percreta with bladder invasion often induces not only massive hemorrhage but also severe bladder/ureter injuries. BACKGROUND: Cystinuria is the most common inherited cause of urinary tract stones in children. It can lead to obstructive uropathy, which is a major cause of renal failure. Genetic studies have identified two genes, SLC3A1 and SLC7A9, to be directly involved in cystine stone formation. Slc3a1 knockout male mice develop cystine stones in the bladder and, to a lesser extent, in the kidney. Slc3a1 knockout female mice also develop cystinuria, but they do not form stones. PURPOSE: We investigated the clinical usefulness of adjusting reference volume per void values for a recently reported strong tendency for volume per void to increase with increasing 24-hour volume in asymptomatic individuals. Our approach was to 1) test whether the adjustment increases the separation between volume per void measurements in patients with detrusor overactivity and an asymptomatic reference population, and 2) compare by regression analysis volume per void vs 24-hour volume relationships in patients with detrusor overactivity and asymptomatic volunteers.

The intrauterine contraceptive device has been in use for many years, and migration of the device from the uterus to the pelvic cavity has been reported by many investigators. Although perforation of the uterus by an intrauterine contraceptive device is not uncommon, intravesical migration and secondary stone formation are rare complications. We report on a 27-year-old woman in whom an intrauterine contraceptive device (Copper-T) migrated from the uterus to the bladder and resulted in formation of a stone.

PURPOSE: Previous studies have shown that the initial reaction of the rabbit bladder to partial bladder outlet obstruction is increased blood flow at day 1 and a return to baseline blood flow at 1 week. Mucosal and muscle blood flow followed this pattern but mucosal blood flow was always 4 to 5-fold greater. In this study the prevalence of bladder dysfunction in 18 children with gross bilateral reflux was investigated via cystometric recordings. In all except 1 infant maximal voiding detrusor pressure was 100 cm. or greater water (range 86 to 244). With exact diagnosis and correct surgical technique, the Lich-Grégoir anti-reflux plasty is safe and complications are few--reflux recurrences are rare. The only serious complication is neurogenic bladder voiding disturbance, which has been observed in some cases after one-stage bilateral procedure and excessive mobilization of the bladder neck. Postoperative ureteral obstructions of the urine flow, generally due to oedemas, are thus temporary. They can be regarded as pseudocomplications.

Young-Dees' method of bladder neck reconstruction is described. A material of 17 cases with complete or almost complete incontinence on account on a deficient sphincter who were submitted to this operative method is presented. It is concluded that the method can be recommended as the primary operation for incontinence, particularly in patients with exstrophy of the bladder or epispadias as satisfactory continence could be obtained in 60%. No deterioration of renal function was observed and the patients functioned well both socially and sexually.
