Endovascular treatment for bow hunter's syndrome: case report.

INTRODUCTION: Bow hunter's syndrome is a unique clinical entity caused by mechanical occlusion of the vertebral artery on head rotation. Although it is usually treated by direct surgical intervention, we report successful treatment using endovascular stent placement for contralateral vertebral artery stenosis. CASE DESCRIPTION: A 56-year-old man presented with repeated vertigo and loss of consciousness caused by turning his head to the left. Right vertebral angiogram showed no abnormalities with the head in the neutral position. However, with the head rotated 60 degrees to the left, the right…

Case of meningeal carcinomatosis with gastric cancer which manifested meningeal signs as the initial symptom; the palliative benefit of radiotherapy.

A 53-year-old male presenting with anorexia, intermittent diplopia, general fatigue, headache and vertigo was admitted to our hospital. He was diagnosed as having gastric cancer by endoscopy of his upper gastrointestinal tract. Brain computed tomography (CT) showed no abnormalities, but magnetic resonance imaging (MRI) showed slight enhancement in the cerebellar sulcus. Cytological examination of cerebrospinal fluid revealed malignant cells. He became blind one week after hospitalization. We diagnosed his condition as meningeal carcinomatosis (MC) and started radiotherapy. His vision improved …

Cognitions associated with anxiety in Ménière's disease.

OBJECTIVES: The purpose of this longitudinal study was to identify cognitions associated with anxiety and maintenance of anxiety in people w … y, fear-avoidance of physical activity, belief that dizziness would develop into a severe attack of vertigo, and several illness perception subscales (emotional representations, consequences, psychological causes, and perceived treatment effectiveness). Anxiety on follow-up was predicted by higher baseline levels of autonomic/somatic symptoms and intolera …

Temporomandibular joint and correlated fissures: anatomical and clinical consideration.

An anatomical study of the fissures related to the temporomandibular joint (TMJ) was performed in 40 dry human skulls. The length of the squ … n of the TMJ may lead to a variety of ear symptoms, i.e., otalgia, tinnitus, hearing loss, possibly vertigo and, less often, to tongue symptoms (collectively characterized as temporomandibular syndrome). These symptoms often relate to the important anatomic structures (anterior malleolar ligament, anterior tympanic artery and chorda tympani nerve) coursin …

Posturographic changes associated with music listening.

OBJECTIVES: The aim of this study was to ascertain whether listening to music might cause changes in human stability and be useful in fall prevention and rehabilitation. The aim was also to find what percentage of subjects without neurologic signs or symptoms associated with falling had less than ideal stability. DESIGN: Computer dynamic posturography (CDP) provided stability scores in 266 subjects without a history of falls or vertigo. Subjects were randomized into several different music listening groups and one control group. The music listening groups were given a daily specific music lis…

Induction of apoptosis and cell-cycle arrest in human colon cancer cells by meclizine.

Meclizine (MEC), a histamine H1 antagonist, is used for the treatment of motion sickness and vertigo. In this study, we demonstrate that MEC dose-dependently induced apoptosis in human colon cancer cell lines (COLO 205 and HT 29 cells). Results of a DNA ladder assay revealed that DNA ladders appeared with MEC treatment in COLO 205 cells at dosage of >50 microM. In addition, the total cell number decreased dose-dependently after treatment with MEC in COLO 205 and HT 29 cells. Using flow cytometry, the percentage of COLO 205 cells arrested at G0/G1 phase increased dose-dependently. Analysis of c…

Mixing model systems: using zebrafish and mouse inner ear mutants and other organ systems to unravel the mystery of otoconial development.

Human vestibular dysfunction is an increasing clinical problem. Degeneration or displacement of otoconia is a significant etiology of age-related balance disorders and Benign Positional Vertigo (BPV). In addition, commonly used antibiotics, such as aminoglycoside antibiotics, can lead to disruption of otoconial structure and function. Despite such clinical significance, relatively little information has been compiled about the development and maintenance of otoconia in humans. Recent studies in model organisms and other mammalian organ systems have revealed some of the proteins and processes r…

Perspective on the vestibular cortex throughout history.

The human vestibular organ transmits sensory information to various components of the central nervous system related to head movement and, obviously, among these components, to its terminal region(s) in the vestibular parts of the cerebral cortex. Study of vestibular structures dates back to historical epochs when primitive considerations on cerebral global function were made without knowledge of a cerebral cortical region related to vestibular function. At the time of Menière in the 19th century, patients with vertigo were defined as having cerebral congestion. Cerebral mapping and computatio…

Destructive, granulating lesion in the temporal bone after elevated plasma homocysteine.

OBJECTIVE: This is a presentation of a seemingly new otolaryngologic disease. SETTING: This study was conducted at a tertiary referral center. CASE REPORT: A 38-year-old healthy man developed left-sided sudden deafness with vertigo and temporary left facial palsy. A granulating and destructive lesion in the left temporal bone was discovered; repeated histologic examination only showed simple granulation tissue. After 6 months, a part of the bony cochlea was extruded. With approximately 8 months' delay and after the patient had had postoperative lung embolism, plasma homocysteine was found to…

Use of the Parodi Anti-Emboli System and transient subclavian steal for cerebral protection during emergent vertebral artery recanalization.

PURPOSE: To report the use of the Parodi Anti-Emboli System (PAES) for cerebral protection during emergent vertebral artery recanalization. CASE REPORT: A 56-year-old chimney sweep was referred with recurrent episodes of vertigo and gait ataxia. Left vertebral artery (LVA) flow was barely detectable on duplex Doppler, and brain computed tomography revealed a small infarct in the posterior inferior cerebellar artery territory. Angiography showed subtotal ostial stenosis of the LVA with poor distal flow and possible thrombus. Due to a high risk of distal embolization with percutaneous treatment…
