Distinguishing sleepiness and fatigue: focus on definition and measurement.

Sleepiness and fatigue are two interrelated, but distinct phenomena; observed in a number of psychiatric, medical and primary sleep disorders. Despite their different implications in terms of diagnosis and treatment, these two terms are often used interchangeably, or merged under the more general lay term of 'tired'. Sleepiness is multidimensional and has many causes (multidetermined) and distinguished from fatigue by a presumed impairment of the normal arousal mechanism. Despite its ubiquity, no clear consensus exits as yet as to what constitutes sleepiness. Definitions of sleepiness, to date

Impact of heart failure on quality of sleep.

Chronic heart failure is an important health problem associated with a high mortality and morbidity. Appropriate treatment reduces mortality and leads to improved exercise tolerance but many patients report poor quality of sleep. Sleep studies of patients with heart failure suggest that sleep disordered breathing is experienced in 50% of patients and is a powerful predictor of poor prognosis. Sleep disordered breathing broadly comprises obstructive sleep apnoea, when upper airway instability causes mechanical obstruction to breathing; and central sleep apnoea, characterised by an absence of ve

Treatment efficacy in behavioral pediatric sleep medicine.

Behavioral interventions have been identified as the treatment of choice for many forms of pediatric sleep disturbance. We adopt criteria established by the Task Force on Promotion and Dissemination of Psychological Procedures (1996) to evaluate the literature base describing behavioral interventions for pediatric sleep disturbance. Three well-established interventions, one promising intervention and one probably efficacious intervention, have emerged for bedtime refusal and frequent nighttime awakenings. Only one intervention for sleep terrors and sleepwalking has sufficient empirical support

Pulmonary haemodynamics in obstructive sleep apnoea.

In patients with obstructive sleep apnoea syndrome (OSAS), pulmonary haemodynamics can show both transient perturbations during sleep and permanent alterations. During sleep, repeated fluctuations in pulmonary artery pressure and pulmonary wedge pressure, coincident with apnoeas, can be observed. Calculation of transmural pressure values is preferable to intravascular pressures in OSAS, due to the marked swings in intrathoracic pressure associated with obstructive apnoeas. Pulmonary artery pressure may progressively increase during sleep, particularly in close sequences of highly desaturating

Sleep disorders in children.

Sleep disorders are common in childhood, and may affect multiple aspects of a child's life and the lives of other family members. A sleep disorder assessment should begin with detailed sleep history and a review of interrelated health issues. Factors contributing to disturbed sleep may be discovered or confirmed by a thorough physical examination. Thereafter, appropriate ancillary testing can provide support for a specific clinical diagnosis. The spectrum of childhood sleep disorders includes OSA, narcolepsy, RLS/PLMD, sleep onset association disorder, and parasomnias. Diagnosing sleep disorde

[Sleep talking].

Sleep talking is very common in the general population. Its prevalence remains stable from childhood through adulthood. Sleep talking is often associated with other parasomnias: sleep walking, sleep terrors or REM sleep behavior disorders. It may arise from either REM or non REM sleep, when associated with REM sleep it is more comprehensible and often associated with clear sentences and recall of sleep mentation. Sleep talking is a benign entity and does not require any treatment; however an exceptional organic cause or psychopathology should be suspected if the onset is late (after 25 years);

Relationship disturbances and parent-child therapy. Sleep problems.

This article has attempted to establish the importance of considering behavioral disturbances in infancy and early childhood as disturbances of the parent-child relationship. When the psychologic and mental mechanisms of the individual infant are too immature to sustain disturbed behavior across several settings and when the behavioral disturbance seems to be specific to a particular relationship, it is more appropriate to diagnose the pathology as being in the relationship. The article has offered a diagnostic framework of relationship pathology that spans the spectrum from normal variation (

Drug-induced nightmares.

OBJECTIVE: To compile and assess the English-language literature on drug-induced nightmares, excluding nightmares secondary to drug withdrawal or drug-associated night terrors. DATA SOURCES: Published articles, letters, case reports, and abstracts in English were identified by MEDLINE (1966-May 1998) searches using the search term nightmares, chemically induced. Additional articles were obtained from bibliographies of retrieved articles. DATA EXTRACTION: All case reports of drug-induced nightmares were evaluated using the Naranjo algorithm for causality. Clinical studies of drugs that report

[Behavior disorder of the REM sleep in 2 documented cases with polysomnography].

We report a 69 years old male with a parkinsonian syndrome and a 50 years old female without neurological problems who showed violent behavior during REM sleep. Polysomnography showed that both bad tonic or phasic muscular activity during REM sleep and a REM sleep behavior disorder was diagnosed. Clonazepam was used in both, with good clinical response. This condition is frequently unrecognized and confused with nightmares, nocturnal delirium or other parasomnias.

Diagnostic aspects of narcolepsy.

The diagnostic criteria for narcolepsy continue to evolve as more is learned about the features of this and other sleep disorders. A variety of symptoms have been said to distinguish narcolepsy from other sleep disorders, including cataplexy, character of daytime sleepiness, sleep paralysis, hypnagogic hallucinations, and automatic behavior. Other diagnostic assessments, such as determination of human leukocyte antigen (HLA) haplotype and findings of sleep laboratory assessments, also contribute to the differential diagnosis. As diagnostic and analytic techniques have become more sophisticated

Sleep-related erectile activity.

Sleep-related erections occur in all healthy, potent men in close temporal association with REM sleep. These erections are naturally occurring and can be assessed quantitatively. The nocturnal tumescence cycle seems unaffected by behavioral factors. Furthermore, sleep-related erections are present across the life span with only a slight decline in older healthy people. Analogous phenomena are present in women. The function of sleep-related erections is not known; however, they clearly involve vascular, neurologic, and hormonal mechanisms. Sleep-related erection testing provides a physiologic,

[Sleep disorders].

There has been a great development in sleep medicine during the past 20-25 years and the International Classification of Sleep Disorders now lists about 90 relatively well-defined conditions related to sleep. Several of them are severely underdiagnosed. This review considers frequent and/or disabling sleep disorders, in particular disorders for which treatment is available.

The effect of regularly scheduled naps on sleep attacks and excessive daytime sleepiness associated with narcolepsy.

To investigate the effectiveness of "nap therapy" for improving alertness and reducing the frequency of sleep attacks, 16 narcoleptic subjects were studied before, during, and after a 1-month program of three regularly scheduled naps. The frequency and timing of sleep attacks, naps, and nocturnal sleep periods were recorded daily in sleep logs for 5 weeks. The severity of five common symptoms of narcolepsy and daytime alertness were assessed before and after the 1-month treatment period. Medications and usual nocturnal sleep habits were not changed during the study period. Mean sleep latency w

The parasomnias of childhood.

Parents are rarely questioned in detail about the sleep characteristics of their children except during the infancy period. Somnambulism, commonly known as sleepwalking, affects approximately 15% of young children. Safety is the biggest concern for a family whose child sleepwalks. Other parasomnias include night terrors, nightmares, and sleeptalking. This article explores the incidence and characteristics of parasomnias, reviews the physiology of sleep, and offers pediatric nurse practitioners information for history taking and anticipatory guidance.

Narcolepsy.

The symptom of excessive sleepiness in children and adolescents does not necessarily cause great concern to families and professionals involved in their care. Children may deny the symptom and minimise the adverse effects. These factors contribute to an underdiagnosis of narcolepsy in this age group when clinical diagnosis is difficult as associated symptoms may not have appeared or are hard to elicit. In this paper three children whose difficult behaviour contributed to the presentation of their sleep disorder are described.

Nonepileptic paroxysmal events in childhood.

Seizures are the most commonly occurring paroxysmal events of childhood. There are a large number of other disorders, however, that present with discrete attacks separated by symptom-free intervals. The differentiation of these conditions from seizures depends on a willingness to consider alternate diagnoses and a thorough review of the history of the events. Laboratory tests, and especially the electroencephalogram, are aids to diagnosis, but reliance on laboratory data has many pitfalls. The clinical features of the important nonepileptic paroxysmal disorders of childhood are reviewed.
