A systematic approach to the surgical management of acoustic neuroma.

Contemporary otomicrosurgical techniques have made total removal of acoustic tumor with preservation of the seventh and sometimes the eighth cranial nerves possible. The four approaches currently used in acoustic tumor surgery are the middle cranial fossa, the translabyrinthine, the suboccipital, and the combined translabyrinthine-suboccipital. This review examines the surgical results in the removal of more than 600 acoustic tumors and outlines a rationale for the choice of approach. Tumor size on computed tomographic scan and auditory reserve establish the parameters used in planning the sur

[An implant for stimulation of the acoustic nerve with 12 channels].

A portable hearing aid for direct stimulation of the eighth cranial nerve was developed and translated into reality. The system (Fig. 1) consists of two components, the portable transmitter and the receiver to be implanted in the mastoid. The speech signal, received in a microphone, is divided into 12 frequency bands in the transmitter (Fig. 2). The outputs are transferred via a vocoder system with pulse amplitude modulation and using a small transmitting device for signals and for energy to the receiver. The implantable receiver (Figs. 3 and 8) transfers the signals in the 12 channels into th

[Visual and acoustic evoked potentials in multiple sclerosis].

A central topic of recent research on multiple sclerosis (MS) concerns the development of sensitive neurophysiological techniques with the ability to detect lesions at the earliest possible stage. Specifically, the recording of evoked potentials has during the past decade demonstrated its diagnostic value by disclosing clinically silent lesions especially of the optic nerve and brainstem region. The visual evoked potential (VEP) was registered in 177 patients with definite, probable and possible MS (McAlpine). Abnormal latencies were obtained in 86/46/34% of these three patient groups, respect

[A case of acoustic neurinoma presenting as subarachnoid hemorrhage].

A case of acoustic neurinoma presenting with subarachnoid hemorrhage is reported. The patient, a 33-year-old female, had suffered from left hearing disturbance and tinnitus for several years prior to admission. She had sudden onset of severe headache in the left posterior auricular region, nausea and vomiting while watching a play-going. Immediately she was brought to a neighboring hospital by ambulance. Lumbar puncture demonstrated xanthochromic cerebrospinal fluid with high opening pressure of 380 mmH2O or more and she was diagnosed as having subarachnoid hemorrhage (SAH). As her level of co

Pathology of acoustic nerve Schwannoma in vivo and in vitro.

The ultrastructural pattern of the acoustic nerve Schwannoma was studied on surgically excised tumours and on explants cultured in vitro. The typical neoplastic cell is elongated, spindle-shaped, coated with basement membrane and numerous protruding cellular processes. Cells demonstrating numerous microfilaments are also seen, sometimes intermixed with the typical spindle-shaped cells. Fibroblasts are not uncommon; nor are cells with a round nucleus and rich in organelle cytoplasm, resembling endothelial cells. Numerous collagen fibres and "Luse bodies" are seen in the extracellular space, esp

[Computer tomography in the diagnosis of neurinomas of the acoustic nerve].

Computer-aided tomography (CT) was used in the examination of 259 patients with neurinoma of the acoustic nerve. Characteristic direct and indirect signs of the tumor and the degree of their manifestation depending on the size and predominant direction of the growth of the neurinoma were determined. In 46 patients the size of the tumour established by computer-aided tomography was compared with that revealed on operation. It was established that computer-aided tomography demonstrates exactly the true size of tumors whose compactness is greater than that of the brain matter. It is difficult to

Tracheal stenosis measured by the acoustic reflection technique.

We examined the usefulness of the acoustic reflection technique for measurement of airway area in 6 patients with tracheal stenosis. In each patient, we obtained airway area by acoustic reflections in the upright position, maximal expiratory and inspiratory flow-volume curve, and radiographs of the trachea. We identified acoustic and radiographic stenotic segments and compared their length, their distances from the glottis, and their cross-sectional areas. We found that (1) in all subjects except one, flow-volume curves did not suggest upper airway obstruction, (2) tracheal stenosis was confir

Normative characteristics of ipsilateral acoustic reflex adaptation.

Contralateral and ipsilateral acoustic reflex adaptation (decay) were examined using 500-, 1000-, and 2000-Hz tonal activators for stimulation periods of 30 seconds in 30 normal subjects. For these normal subjects, ipsilateral adaptation always exceeded contralateral adaptation, although differences between the two conditions were not significant. In addition, contralateral and ipsilateral acoustic reflex adaptation of four subjects with varying degrees of sensorineural hearing loss were compared to the normative data. Results for these impaired ears indicated that a longer period of activatio

Acoustic differentiation of laryngeal, esophageal, and tracheoesophageal speech.

This investigation was designed to determine if a multivariate acoustic classifier could effectively discriminate group membership for 15 tracheoesophageal, esophageal, and laryngeal speakers. Seven intensity, 10 frequency, and 13 duration measures were quantified from recorded voice samples. Using principal components analysis, a subset of the 13 least redundant acoustic and temporal measures was systematically selected from the 30 original measures and analyzed singly and jointly in terms of its ability to discriminate among the three speaker groups. Discriminant function analysis revealed p

Aspirin abolishes spontaneous oto-acoustic emissions.

Spontaneous oto-acoustic emissions (OAEs) were measured prior to, during, and following administration of aspirin. The dose schedule was three 325-mg tablets every 6 h for a total of 16 doses (3.75 days). In every subject studied, all spontaneous OAEs gradually diminished and then disappeared during the drug regimen. Emissions that were initially small disappeared within 14-20 h of beginning the drug regimen (3-4 doses), while initially large emissions took 40-70 h (7-12 doses) to disappear completely. In contrast, the initial size of an emission appeared unrelated to the time required for it

The effects of alcohol on the acoustic Reflex Relaxation Index.

Normal-hearing young adults (6 M, 5 F) ingested 1.5 ml alcohol per Kg body weight over 90 min (blood alcohol level then being 0.104 +/- 0.016%), then tested for contralateral acoustic reflex (AR) threshold using an otoadmittance meter, and given the acoustic Reflex Relaxation Index (RRI) test of Norris, Stelmachowicz and Taylor. Each stimulus for the RRI was a 4-sec train of pulses at a level 10 db above AR threshold at .5, 1, or 2 kc/s, with equivalent on-time of 166.7 msec and off-time of 200 msec. AR responses were displayed on an X-Y recorder. RRI was taken as the percentage of total relat

Evaluation and integration of acoustic features in speech perception.

Identification of synthetic stop consonants as either /bae/, /pae/, /dae/, or /tae/ was examined in two experiments in which the stimuli varied independently on voice onset time (VOT), the consonantal second and third formant (F2-F3) transitions and, in experiment 2, the intensity of the aspiration noise during the VOT period. In both experiments, the patterns of the resulting identification probabilities were complex, but systematic, functions of each of the independent variables. Of most interest was the fact that the likelihood of identifying a stimulus to be /bae/ or /pae/, rather than /da

Acoustic immittance findings in acute otitis media.

This study sought to evaluate the sensitivity of tympanometry and acoustic reflex measurement for identifying acute otitis media. Acute otitis media was diagnosed via case history and biphasic pneumo-otoscopy in 103 children (161 ears) ranging in age from four months to 17 years (mean age, 5.6 years) who had subjective complaints of only one to three days. Of the 161 ears with diagnosed acute otitis media, the tympanogram was categorized as flat in 102 ears (63%), shallow/rounded in 18 ears (11%) and negative in 17 ears (11%). Of particular interest was that 15 ears (9%) were classified tympan

Function of the acoustic reflex in discrimination of intense speech.

It is known that the middle ear acoustic reflex (AR) alters the transmission characteristics of the auditory system; however, disagreement exists on how these changes may influence the ears; transfer of speech stimuli. In this study, speech with and without a competing message was delivered at supra-AR levels to cat ears that demonstrated an active and a pharmaceutically inactivated middle ear reflex mechanism. Resulting cochlear potentials were tape recorded and subsequently presented to 18 human subjects with normal hearing. Findings suggest word and sentence discrimination is enhanced in th

Decay and recovery of the acoustic stapedius reflex in humans.

The time course of decay and recovery of ipsilateral and contralateral stapedius reflex responses to 2 000 Hz pure tone stimulation was studied in 10 normal-hearing subjects. Reflex responses were found to follow a closely similar time course with respect to both decay and recovery in simultaneous bilateral recordings. The similarity is compatible with the assumption that decay and recovery originate in the afferent auditory system, and not in the muscles. Recovery was 50% complete 250 ms after the end of the stimulus and most subjects had reached their initial amplitude after 1-3 s. The indiv
