BACKGROUND: Reports place the frequency of phrenic nerve injury after cardiac operations between 10% and 85%, emphasizing the importance of an accurate anatomic description of the diaphragm's innervating nerves to reduce iatrogenic injury, length of hospitalization, and associated costs. The aim of our study was to explore the anatomic variations of the accessory phrenic nerve and relate these findings to phrenic nerve injury.
METHODS: Eighty adult formalin-fixed cadavers were dissected, resulting in 160 nerve specimens. Fifty nerve specimens were also examined laparoscopically with findings

We analyzed the relation between meningioma and the brain in 50 surgical cases. So-called capsule formation was seen in 20 meningiomas, of which 13 were categorized as thin and 7 as thick. In 21 meningiomas the arachnoid membrane was intact, and 10 meningiomas had no underlying arachnoid membrane. The other 19 tumors showed partial disruption of the arachnoid membrane. The degree of arachnoid disruption correlated with the tumor grade, perifocal edema, pial blood supply on angiography, and tumor size. The existence of brain invasion correlated with the tumor grade and partially with tumor size

To probe the feasibility and utility of neuroendoscopic inspection of the anatomy of the cerebellopontine angle (CPA) and of neuroendoscopic assisted microneurosurgery (NEAMN) for CPA lesions via a retrosigmoid approach, we used retrosigmoid NEAMN in 28 patients with CPA lesions. Prior to this, we undertook anatomical observation of bilateral CPA in two adult cadaver heads using the neuroendoscope. NEAMN tumour resection was performed in eight acoustic neuromas, one meningioma and 14 cholesteatomas and NEAMN vascular decompression was performed in five patients with trigeminal neuralgia. Both …

The advent of ultra-high-frequency sonographic transducers has significantly enhanced our ability to image superficial structures. As a result, sonography now can be used to assess injuries of the tendons in the wrist and hand. A clear understanding of normal sonographic anatomy is required to prevent misdiagnosis and ensure optimal patient care. The anatomy of the wrist and hand is best described by considering the extensor and flexor surfaces separately. The carpal extensor retinaculum divides the dorsal extensor tendons into six separate synovial compartments, which are demarcated by the po…

The process of atherosclerosis begins with endothelial dysfunction from impaired bioavailability of NO and progresses through multiple stages of plaque development. Impaired endothelial function decreases NO and increases oxidized macromolecules such as low-density lipoprotein cholesterol. Oxidized low-density lipoprotein cholesterol accumulates in subendothelial space, forming plaques that may eventually compromise the lumen. Acute coronary events result from plaque rupture and consequent thrombus formation and abrupt occlusion of the vessel lumen. New diagnostic methods such as flow-mediated

Vagal and spinal afferent innervation of the portal hepatic area has not been studied as thoroughly as the innervation of other important organs. It is generally agreed that unlike noradrenergic sympathetic efferent nerve fibers, sensory nerve fibers of either vagal or dorsal root/spinal origin do not directly innervate hepatocytes, but are restricted to the stroma surrounding triades of hepatic vasculature and bile ducts, and to extrahepatic portions of the portal vein and bile ducts. For vagal afferent innervation, retrograde and anterograde tracing studies in the rat have clearly shown that

OBJECT: The purpose of this study was to define the topographic anatomy, arterial supply, and venous drainage of the insula and sylvian fissure.
METHODS: The neural, arterial, and venous anatomy of the insula and sylvian fissure were examined in 43 cerebral hemispheres.
CONCLUSIONS: The majority of gyri and sulci of the frontoparietal and temporal opercula had a constant relationship to the insular gyri and sulci and provided landmarks for approaching different parts of the insula. The most lateral lenticulostriate artery, an important landmark in insular surgery, arose 14.6 mm from the apex…

Anosognosia for hemiplegia is the denial of the contralesional motor deficits that may follow brain damage. Although this disturbance has been reported in the neurological literature since the beginning of the last century, only few longitudinal studies have addressed the issue of the anatomical substrate of the disorder. Here we present a comprehensive review of the literature on anosognosia for hemiplegia from 1938 to 2001, taking into account some of its clinical, epidemiological and anatomical aspects. In particular, an attempt has been made to identify the intra-hemispheric lesion locatio

Variations in the anatomy of the inferior alveolar nerve were seen in 2 of the 20 dissections of the infratemporal fossa in 10 cadavers. A connecting nerve branch that originated from the auriculotemporal nerve joined the inferior alveolar nerve on both sides. The second part of the maxillary artery passed between the mandibular nerve, the root of the inferior alveolar nerve, and the connecting nerve branch which formed a loop. The maxillary artery seemed to be entrapped. Neurovascular entrapment can cause pain and numbness. Anatomical variations in this region should be kept in mind, particul…

The outstanding progress achieved in MRI and arthroscopic techniques has forced orthopedic surgeons to consider the anatomy of the shoulder joint in detail. The superior labrum complex shows a broad range of variations, which is the reason for some difficulties regarding diagnostic conclusions as well as therapeutic considerations. In particular, the differentiation between the normal and pathological adherence of the labrum is a serious clinical problem. The glenoid labrum consists of circularly arranged collagenous fibers attached to the osseous margin of the glenoid by a fibrocartilaginous …

INTRODUCTION: Reconstruction of the MPFL for recurrent and acute lateral dislocation of the patella is becoming more accepted. This study aimed to further define the medial patellofemoral ligament (MPFL) anatomy and to describe the most suitable sites for graft attachment when reconstructing the MPFL.
MATERIALS AND METHODS: Anatomical dissection was performed on 25 embalmed knee specimens. Four separate specimens were used to assess graft fixation points. Sites were tested for isometricity from 0 to 120 degrees of knee flexion. Length patterns were considered isometric if there was less than …

The parenchymal cells of the mammalian pineal gland are the hormone-producing pinealocytes and the interstitial cells. In addition, perivascular phagocytes are present. The phagocytes share antigenic properties with microglial and antigen-presenting cells. In certain species, the pineal gland also contains neurons and/or neuron-like peptidergic cells. The peptidergic cells might influence the pinealocyte by a paracrine secretion of the peptide. Nerve fibers innervating the mammalian pineal gland originate from perikarya located in the sympathetic superior cervical ganglion and the parasympathe

This study provides an accurate, anatomical description of the rabbit's lumbosacral spinal cord. Following humane euthanasia, 64 rabbits were radiographed and dissected. The number of thoracic (T) and lumbar (L) vertebrae was 12T/7L in 43.8%, 13T/6L in 32.8%, and 13T/7L in 23.4% in the 64 rabbits studied. The 13th ribs were rudimentary, asymmetric, or both in 15 animals. The spinal cord terminated within the second sacral vertebra (S2) in 79.3%, within the first sacral vertebra (S1) in 19.0%, and within the third sacral vertebra (S3) in 1.7% of the sample. Spinal nerve contributions to the fem

Basic knowledge of macroscopic anatomy is needed to understand the clinical signs and distinct syndromes resulting from the disturbed function of the tracks, segments and roots of the spinal cord. The cord is contained within the vertebral canal, continuing the medulla down to the conus medullaris. Descending tracks are motor (corticospinal and extrapyramidal), and ascending tracks are sensory (spinothalamic and lemniscal). Spinal cord involvement must be considered when a sensorimotor spinal tract and a radicular spinal syndromes are combined. Syringomyelia, Brown-Séquard syndrome and subacut…

Carpal tunnel syndrome is one of the most commonly diagnosed disorders of the upper extremity. The etiology of the neuropathy is known to be associated with many disorders, with the etiology of carpal tunnel syndrome mainly attributable to ischemia of the median nerve. The purpose of this study was to determine the presence of neural elements within the transverse carpal ligament. Fourteen transverse carpal ligaments were harvested from seven male and seven female fresh frozen cadavers with an average age of 76 years. The tissues were stained with S-100 using a standard immunoperoxidase techni
