Congenital heart disease and pregnancy.

Patients with congenital heart disease who wish to become pregnant offer a challenge to obstetricians, cardiologists and anesthetists. Although no large randomized trials exist to support the management of this emerging population, small prospective and retrospective studies provide valuable data on the likely success of pregnancy and the risks involved. Recently, there is emerging consensus on the management of this specialized group of patients, and this article aims to provide the practitioner with an overview of patient needs and the issues to be addressed. All patients with congenital hea…

Comparison of Dexmedetomidine In Ultrasound-Guided Supraclavicular Brachial Plexus Block Versus Intramuscular Administration In Upper Limb Surgeries.

Dexmedetomidine is known to prolong sensory and motor blockade by local anesthetics. In this study we compared the efficacy of dexmedetomidine given intramuscularly and perineurally as an additive to supraclavicular brachial plexus block. Sixty-two patients requiring upper limb surgeries were randomly assigned to two groups. All patients received 2% lidocaine (10 mL) + 0.5% levobupivacaine (10 mL) + normal saline (5 mL). In the perineural group (PN), 0.5 µg/kg dexmedetomidine was administered perineurally in the block whereas in the intramuscular group (IM), 0.5 µg/kg dexmedetomidine was admin

Anesthetic Management of Elective Cesarean Delivery in a Pregnant Patient With Dilated Cardiomyopathy and Pulmonary Hypertension: A Multidisciplinary Approach.

Peripartum cardiomyopathy (PPCM) complicated by pulmonary arterial hypertension (PAH) is an uncommon and high-risk condition that presents significant anesthetic and obstetric challenges. This case report contributes to the limited literature by describing the successful anesthetic management of a parturient with severe cardiac dysfunction and PAH, emphasizing the importance of a multidisciplinary approach. A 30-year-old pregnant woman with newly diagnosed dilated cardiomyopathy of unknown etiology and PAH was admitted for elective cesarean delivery. The diagnosis was established in the second

Damage Control Resuscitation of a Patient With Traumatic Aortic Rupture: A Case Report.

Traumatic aortic rupture (TAR) is a highly fatal injury mechanism resulting from blunt deceleration forces against the descending aorta. The mechanism of TAR is directly attributed to the aorta suffering damage by indirect shearing forces. The descending aorta remains fixed to the posterior chest wall, while the heart and ascending aorta are exerted forward, thus causing the intimal tear. A characteristic triad presents as increased blood pressure in the upper extremities, decreased blood pressure in the lower extremities, and a widened mediastinum on radiography. Early recognition of signs an

Cost Analysis of Intravenous Propofol Monotherapy versus Intravenous Combination Sedation in Patients Undergoing Outpatient Gastrointestinal Endoscopy.

In outpatient gastrointestinal (GI) endoscopy, for which postanesthesia care unit (PACU) stays are expected to be brief, sedative choices by anesthesia providers can affect costs. The purpose of this project was to evaluate the cost of propofol monotherapy compared with combination sedation consisting of propofol with any of the following: midazolam, fentanyl, dexmedetomidine, and/or ketamine. A total of 277 patients who underwent outpatient GI endoscopy were included in this retrospective medical record review. Patients were separated into 2 groups: propofol monotherapy (n = 233) or combinati

Case-series of nurse-administered nitrous oxide for urinary catheterization in children.

BACKGROUND: Children undergoing urologic imaging studies requiring urethral catheterization experience considerable discomfort and psychological distress. Nitrous oxide sedation may mitigate these detriments but the requirement for physician administration has limited the applicability of this technique. METHODS: Registered nurses underwent the nitrous oxide training requirements prescribed for state licensure of dentists and dental hygienists, with special emphasis on pediatric sedation principles. To evaluate the safety of nurse-administered nitrous oxide, we consecutively enrolled all chil

A case of subdural block triggered posterior reversible encephalopathy syndrome: A case report.

RATIONALE: Posterior reversible encephalopathy syndrome (PRES) is a relatively rare neurological disorder in obstetrics. Clinically, the most common precipitating factors are hypertension, immunosuppressive agents, or chemotherapy drugs. It is extremely rare for PRES to be induced by the entry of local anesthetic into the subdural space. PATIENT CONCERNS: In this case report, we present an instance where a patient experienced blurred vision, and even blindness, following an accidental subdural block administered by the anesthetist, and was subsequently diagnosed with PRES. The patient was a 3

Erector spinae plane block combined with local infiltration analgesia for total hip arthroplasty: A randomized, placebo controlled, clinical trial.

The erector spinae plane block is an emerging analgesic technique, which is gaining popularity for a large number of procedures. The majority of publications are at the thoracic level and almost all indicate some benefit to patients. However, there have been relatively few randomized controlled trials and even fewer studies at the lumbar level. The aim of this study was to assess whether the erector spinae plane block at the lumbar level would confer early analgesic benefits and improve the quality of recovery in patients undergoing elective unilateral primary hip arthroplasty. Sixty-four pati

Intrinsic Laryngeal Lipoma Treated with Transoral CO₂ Laser Microsurgery: An Unusual Case Report.

BACKGROUND Lipomas, the most common soft tissue tumors, represent almost half of all benign mesenchymal neoplasms and are characterized by the absence of symptoms. However, if they grow in a laryngeal site, they can cause dyspnea and dysphonia which represents one of the major symptoms of benign laryngeal diseases (such as polyps or nodules); these symptoms are often the first symptom of neoplastic diseases. CASE REPORT We present a case of a 28-year-old female patient with a rounded pseudocyst neoformation located in the left aryepiglottic fold that partially hid the homolateral vocal cord an
