Occupational problems of the foot and ankle have received less attention than those of other body regions, namely the hand, neck, and lumbar spine. Functional requirements of the foot and ankle differ from other body regions, necessitating a different clinical approach to injuries than those indicated in the upper extremity or back. This article reviews the clinical characteristics of the foot and ankle pertinent to the worker and highlights the unique features of the management of foot and ankle problems encountered in occupational orthopedic practice.

The evaluation of ankle injuries is a common challenge for family physicians. Diagnosis is made by using knowledge of the anatomy and function of the ankle joint to aid in taking an adequate history and performing an appropriate physical examination. The patient should be questioned about the mechanism of injury, previous injury, disability, treatment and pain. The ankle should be evaluated for ecchymosis, swelling, areas of tenderness and laxity. These measures help to determine what, if any, additional diagnostic procedures may be needed. Most ankle sprains are lateral, affecting the anterio…

Ankle sprain is a common sports injury and is often regarded as trivial by athletes and coaches. This epidemiological study was conducted among three categories of Hong Kong Chinese athletes: national teams, competitive athletes and recreational athletes. This study shows that as much as 73% of all athletes had recurrent ankle sprain and 59% of these athletes had significant disability and residual symptoms which led to impairment of their athletic performance. This study indicates that a proper approach towards injury prevention and a comprehensive rehabilitation programme are required.

Rheumatoid arthritis of the foot and ankle can be a debilitating problem, particularly for patients who have undergone successful hip or knee arthroplasty. Optimal medical management, use of orthotic devices, and surgical intervention are essential components of patient care. Forefoot involvement with hallux valgus and lesser metatarsophalangeal joint subluxation and dislocation are the most common findings. Reconstruction usually requires lesser metatarsophalangeal joint excisional arthroplasty and first metatarsophalangeal joint arthrodesis. Midfoot tarsometatarsal and intertarsal involvemen…

Lower extremity problems, specifically in the feet and ankles, are common in dancers. This is not surprising considering the repetitive and acute stresses applied to lower limbs during this rigorous performing arts activity. When evaluating a dancer with lower extremity complaints, the nature of these demands must be appreciated. In addition to routine physical examination, static and dynamic biomechanical evaluation is paramount to analysis and treatment of leg, foot, and ankle conditions. Although a particular problem, such as painful bunion, may seem localized, it is often related to other …

Foot and ankle injuries in basketball are discussed in three unrelated categories in this article. This includes a practical differential diagnosis of ankle sprains, acute conditions of the mid and hindfoot, overuse syndromes of nerve entrapment, fascial strain, synovitis, joint subluxation, and inflammation resulting from repetitive stress. The diagnosis and treatment of tendon inflammation of the extrinsic foot musculature is also reviewed.

Acute lateral ankle ligament ruptures are successfully treated nonoperatively with physiotherapy, peroneal strengthening, and coordination training. About 10-20% of patients may develop functional instability despite adequate nonoperative treatment. Chronic functional instability is not always a severe disability, but reconstruction of the lateral ankle ligaments may be necessary for patients with high demands on ankle stability. More than 50 different surgical procedures for the treatment of chronic lateral ankle joint instability have been described. Most of these are tenodeses where one of …

Ankle injuries in the young athlete are quite different from those of an adult. Because the epiphyseal plates of the distal tibia and fibula are significantly weaker than surrounding ligaments, failure on stress occurs through the growth plates rather than through soft tissue. Growth-plate injuries can assume a distinct pattern based on the maturity of the physis. Leg length inequality and angular deformity are potential sequelae from significant injury to the ankle in the skeletally immature but, fortunately, occur infrequently. Soft-tissue injuries about the ankle are rare, but with advanced…

Dancing is not dangerous; however, dancers must recognize their limitations and learn to do the best they can with what they have to work with. This article discusses some of the more common acute and chronic foot and ankle problems in dancers for the benefit of the treating physician.

Severe soft tissue injury occurring in sprains and dislocations may lead to painful disability after plaster-immobilisation for 6-12 weeks. Therefore, primary operation is indicated for ligamentous repair, osteochondral fracture-fixation and internal K-wire stabilization of dislocations after reduction. After 1 week of immobilisation, the patient should begin active daily muscle exercises; in the 2nd week patients are mobilized with support of a dynamic, partial weight-bearing splint, allowing continued muscle exercises until weight-bearing is possible 6-8 weeks after surgery. In 23 patients u

In transmitting the body's weight, the ankle is subject to frequent static and dynamic injury due to concentrated stresses during standing and movement. The frequency of athletic ankle injuries ranges from 10 to 90 percent, with the highest rate occurring in basketball players. There is prolonged disability and recurrent instability for months to years for 25 to 40 per cent of these patients. Because most of this trauma is handled by primary care physicians, this review presents the mechanism of injury, relevant anatomy, physical examination, and appropriate therapeutic intervention in the acu…

The ability to classify ankle fractures allows one to determine which fractures will probably do well with nonoperative treatment and which fractures will fare best with open reduction because of their inherent instability. An understanding of the Lauge-Hansen system also allows one to predict the degree of ligamentous injury on the basis of the osseous pattern of the injury. Operative management of ankle fractures requires a thorough understanding of ASIF technique. Open reduction is best performed with a fracture that is not anatomically reducible or with a fracture type that has been histor…

The ankle joints of 109 extremities with fibula reductions wer examined. The authors' findings led them to conclude that the shape of the ankle joint must be regarded as the result of an adaptation to altered functions. Ball-shaped ankle joints are frequently found in cases of hypoplasia of the fibula; partial and subtotal aplasias of the fibula are more likely to be associated with a valgus deformity of the ankle joint (known as Volkmann's ankle joint deformity) or, if more severe, with kyphosis of the tibia. If there is no laterodorsal restraint by coarse fibrous strands in cases of fibular …

Ankle sprains are common in dancers. They result from working in the positions which allow increased risk of sprain on the lateral side of the ankle for many hours a day. One hundred ankle injuries were evaluated. The mechanisms of injury are presented, as well as a classification of these injuries. The recommended treatment and rehabilitation are discussed. Leg muscle strength must be restored before rehabilitation is considered complete.
