PURPOSE: We describe our experience correcting a cohort of infants with contact lenses in the Infant Aphakia Treatment Study.
MATERIALS AND METHODS: Fifty-seven infants 1-6 months of age were randomized to contact lens wear. An examination under anesthesia was performed at the time of enrollment and at approximately 1 year of age. A traveling examiner assessed visual acuity at approximately 1 year of age.
RESULTS: Forty-two treated eyes (74 %) were fitted with silicone elastomer (SE) contact lenses; 12 eyes (21 %) with rigid gas permeable (RGP) contact lenses, and 3 eyes (5%) wore both lens

A simple technique for the accurate surgical alignment of toric phakic and aphakic intraocular lenses (IOLs) is described. The mathematical relationship between toric misalignment and cylinder error is reviewed to demonstrate the importance of toric lens alignment in the setting of increased rotational stability with newer generation lenses. A formula for calculating the exact lens misalignment given a manifest cylinder is also presented.

BACKGROUND: Cataracts pose a serious threat to the visual system in infants and children, requiring prompt surgical intervention and rehabilitation to optimize visual development. Glaucoma is a serious complication following cataract surgery in infants and can lead to loss of vision. Early diagnosis of glaucoma, however, can be quite challenging in these patients.
CASE REPORTS: Three cases are presented in which comeal edema was the initial presenting sign that led to the diagnosis of glaucoma during routine contact lens follow-up examinations for pediatric aphakia.
CONCLUSION: The contact l

PURPOSE: This prospective study investigated characteristics of fit and performance of silicone contact lenses under conditions of permanent wear in cases of aphakia after congenital cataract.
METHODS: Seventeen aphakic children (8 eyes unilateral, 18 eyes bilateral) were fitted with silicone contact lenses on an empirical basis without the use of keratometry. The lenses were worn on a permanent basis, with a routine replacement every 3 months. The follow up was 6 years, with evaluation lens parameters, fitting characteristics, lens condition, replacement rate, wearing time, complications, an

The possibility of YAG laser interventions on the posterior capsule of the lens with subsequent argon laser coagulation of the retina in patients with aphakia and artiphakia is studied. The optimal intervention for secondary cataract in patients with artiphakia is circular resection of the posterior capsule. Intraocular lenses prevent such complications as vitreous hernias and allow creating a larger and better shaped opening in the posterior chamber, thus providing a better access for laser coagulation of the retina.

BACKGROUND: Surgery for congenital cataracts in early infancy usually includes a primary posterior capsulectomy and an anterior vitrectomy. Initially, most of these infants have aphakia after surgery. Over time, remaining equatorial lens epithelial cells produce new cortical fibers, resulting in a ring of cortex trapped between the lens equator and the fused anterior and posterior capsulectomy edges. A potential space is maintained between the anterior and posterior capsular leaflets. We describe a technique for placing a secondary intraocular lens (IOL) within the capsular bag.

BACKGROUND: Late-onset glaucoma can occur after pediatric cataract surgery. However, no large prospective study of the prevalence of ocular hypertension and glaucoma after pediatric cataract surgery has been performed.
METHODS: A prospective glaucoma evaluation was offered to all eligible subjects at least 5 years after automated lensectomy and vitrectomy for pediatric cataracts.
RESULTS: Sixty-two (58%) of 107 eligible subjects received a glaucoma evaluation. One (4.5%) of 22 patients with bilateral cataracts had glaucoma, and a much higher percentage (45%) had ocular hypertension. Five (12

Cataracts are the commonest cause of blindness in Third-World countries and cataract extraction is the commonest eye operation performed anywhere. Patients require optical correction after surgery either in the form of an intra-ocular lens (IOL) or bifocal aphakic glasses. The standard operative procedure in First-World countries is an extracapsular cataract extraction with an IOL. This type of operation has been thought to be contraindicated in black patients because of the complications and lack of adequate postoperative follow-up. During 1987-1989 3,144 cataract extractions, of which 2,157

Diffractive bifocal contact lenses function by diverting incident light to two focal points. Light from near objects is focused by diffraction while that from distant objects is focused by conventional refraction. Both processes occur simultaneously throughout the full aperture of the lens. Fourteen aphakic and pseudophakic contact lens wearers were fitted with diffractive bifocal lenses and observed for an eight week period. Six (42.8%) were satisfied with the visual result. Five subjects (35.7%) discontinued lens wear complaining of poor quality of near vision. We conclude that these lenses

In this study we examined refractive changes of 34 eyes in 34 patients treated with silicone oil injection. Six patients presented with aphakic retinal detachment, another 14 were rendered aphakic after silicone oil injection, one was pseudophakic and 13 were still phakic at the time of writing this report. Silicone oil in the vitreous cavity induced a myopic shift in aphakic eyes and a hypermetropic shift in phakic eyes. This tended to render phakic high myopes and aphakes closer to emmetropia. The resultant refraction was dependent on the shape of the silicone oil bubble inside the eye and i

We measured prostaglandin E2 and leukotriene B4 in the aqueous humor of baboon eyes, 1 or 8 days after phacoemulsification, with or without posterior chamber lens implantation. We also evaluated the effects of steroid eye drops and cyclooxygenase inhibitor eye drops on the synthesis of these mediators of inflammation. Eyes that had undergone phacoemulsification showed significant elevation of prostaglandin E2 at both postoperative periods compared with normal, phakic control eyes. The level of prostaglandin E2 was significantly (P less than .05) higher in eyes with posterior chamber lens impla

The analysis concerned 100 eyes (91 persons) operated for senile (89 eyes), traumatic (7 eyes) and pathological (4 cases) cataracts, with implantation of an artificial anterior chamber lens. The age of the patients oscillated between 32 and 86 years. The period of observation amounted from several months to 3 years. In 83 cases the final visual acuity was over 0.7. The authors stated a dependence of the visual acuity on the coexisting general diseases and on the type of cataract.

The influence of correcting soft contact lenses and pleoptic-orthoptic exercises on the improvement of the visual acuity and acquiring of binocular vision was analyzed in 22 patients aged 17-65 years after unilateral cataract extraction. Visual acuity of between 5/5 and 5/16 was attained in 88.6 p.c. of patients, binocular vision in 85.9 p.c. Poorer results were observed in persons with a diminished transparency of the ocular media caused by injury as well as in younger individuals after congenital cataract surgery.

Analysis of the means by which accuracy of intraocular lens power calculation could be improved with theoretical and linear regression formulas. The postoperative refractive errors with the lenses selected by the surgeon were compared with those observed with the emmetropic calculated power lenses and with a standard 20 diopters lens. A consecutive series of 100 eyes with less than 4.50 diopters of pre-operative myopia or hypermyopia, is evaluated. Postoperative refractive errors are out of the range -4; +4 diopters in 6% for standard lens, 4% for actually implanted lens, and 1% for emmetropic

Soft contact lenses (SCL) were used for the correction of aphakia in young children. Eleven surgeries were carried out in 7 children with congenital cataracts (aspiration in 2 cases and transciliary lensectomy in the rest). SCLs were made individually of gel with 70% water content by mechanical treatment, their parameters differing from the commercial 'aphakic' lenses. These SCL parameters were chosen with consideration for the data of measurements of the corneal curvature and diameter, carried out during surgery and then specified by trials. To determine the optic force of SCL, a special form

In a prospective study 84 patients with aphakic retinal detachment were treated either by local scleral buckling alone or combined with an encirclement. The rate of surgical reattachment of the retina was found to be similar with either technique over a minimum follow-up period of one year. Simplicity and a low incidence of serious complications of the local procedure merit its application as the initial method of repairing aphakic detachments.
