[Primary hypothyroidism with obstructive sleep apnea syndrome].

The relationship between severity of hypothyroidism and OSA syndrome before and after treatment was analyzed and discussed. All patients had … followed up after treatment (P < 0.001, respectively). The respiratory parameters including maximum apnea time, mean apnea time, respiratory disturbances index and the lowest oxygen saturation % were also markedly different in patients before and after treatment (P < 0.001-0.02, respectively). The serum levels of TT4 and FT4I were significantly correlat …

Anesthetic management of obstructive sleep apnea patients.

Presented in an illustrative case report and a review of the anesthetic management of obstructive sleep apnea patients. Preoperative evaluation should include a thorough airway evaluation and a comprehensive cardiovascular and pulmonary evaluation. With polysomnography, identification of the severity of sleep apnea can be idenified. Although sleep centers vary in their definitions, severe obstructive sleep apnea is diagnosed if the patient demonstrates an apnea index greater than 70 and an oxygen (O2) desaturation less than 80% with cardiovascular sequelae. Severe sleep apnea patients are at e…

[Obstructive sleep apnea treated with uvulopalato-pharyngoplasty].

38 patients with sleep apnea syndrome treated with uvulopalato-pharyngoplasty were interviewed 23 +/- 10.8 months after the operation (mean age 48.0 +/- 8.6 years, range 28-65). 22 were also studied in the sleep laboratory 10 +/- 10.7 months after operation. 3 months after operation 94% reported improvement in snoring and 77% improvement in excessive daytime sleepiness. 1 and 2 years after the operation snoring had decreased to 74% and 65%, respectively, and excessive sleepiness to 65%, for both periods. There was a decrease of at least 50% in the number of apneic periods per hour of sleep in …

Suffocation, recurrent apnea, and sudden infant death.

We reviewed the cases of 27 young children from 27 different families who were suffocated by their mothers. The certainty, or near certainty, of suffocation was based on reliable observation or recording of the suffocation, maternal confession, or successful prosecution in a criminal court. Eighteen of the children are alive, although one has severe brain damage; nine are dead. Twenty-four were reported to have had previous episodes of apnea, cyanosis, or seizure, and 11 had had 10 or more such episodes that were either invented or caused by the mother. Repetitive suffocation usually began bet…

[Sleep apnea syndromes and Arnold-Chiari malformation].

The Arnold-Chiari malformation (MAC), ectopic medulla of the cerebral hemispheres, may be the basis of respiratory problems provoked by compression of the respiratory centre. It is associated with numerous respiratory problems in children; there are however few publications related to the association of MAC and ventilatory problems in adults. It was recently suggested and subsequently shown that the syndrome of sleep apnoea (SAS) could occur in relation to MAC. We report a new observation documenting the association of MAC and SAS. The surgical cure of MAC enabling a remission of SAS suggests

The sleep apnea syndrome. Part I: Diagnosis.

Part I of this two-part article presents a review of the symptoms, pathophysiology, and diagnosis of the sleep apnea syndrome. The more common obstructive type is characterized by disturbed sleep and daytime hypersomnolence. Subjective signs and symptoms should be correlated with objective findings by way of polysomnography (sleep study) to confirm the diagnosis and determine the severity of obstructive sleep apnea. Electrocardiographic monitoring during polysomnography has revealed potentially life-threatening arrhythmias during sleep. Differential diagnosis should include central sleep apnea…

[Sleep apnea syndrome: late sequela of poliomyelitis].

A syndrome of sleep apnoea may appear 15 to 29 years after acute anterior poliomyelitis (PAA). It is generally a mixed syndrome with an association of central type and obstructive apnoea in variable proportions. We report such a case occurring in a patient who had presented 30 years before with PAA, and presenting on this occasion with resting pulmonary artery hypertension, polycythaemia but without disturbance of blood gases. Treatment with positive pressure ventilation was given by the nasal route at 10 cm of water leading to an improvement with a significant decrease in the number and durat

Neonatal apnea: diagnosis by nurse versus computer.

In an effort to characterize significant neonatal apnea and evaluate the nursing diagnosis of apnea, apnea type and frequency were determined in 27 infants by continuous computer recording of heart rate, respiratory impedance, end-tidal CO2, and either or both transcutaneous oxygen and pulse oximetry. Of the 1,266 recorded apneas, 46% were central, 44% were mixed, and 10% were obstructive. Mixed apnea was associated with a longer mean duration and greater mean decrease in heart rate than central apnea. Apnea duration was positively correlated with both a decrease in heart rate and oxygen satur…

Apnea-onset definition significantly affects pneumogram results.

Pneumogram (PG) scoring methods have not been standardized. To determine the extent to which varying definitions for apnea-onset and apnea-termination will result in clinically significant differences in calculated apnea density (A6/D%) and periodic breathing, we analyzed 40 randomly selected PGs. All PGs were initially scored using end-expiration (EE) to mark the beginning of each apnea. As the second definition, peak inspiration (PI) was used to identify the onset of each apnea. All apneas greater than or equal to 6 s in duration and all episodes of periodic breathing were identified. The di…

Naloxone improves sleep apnea in obese humans.

To test the hypothesis that endogenous opiates play a role in the etiology of the sleep apnea syndrome, we administered naloxone, an opiate antagonist, to ten obese humans with sleep apnea. On two separate nights we measured the frequency and severity of sleep apnea during naloxone infusion vs saline control infusion. The number of oxyhemoglobin desaturation episodes was not significantly lowered but the average maximal oxyhemoglobin desaturation fell significantly (P less than 0.01) with naloxone. The desaturation index (average maximal oxyhemoglobin desaturation times desaturations per hour)…

Decreased specific airway conductance in infant apnea.

A disorder of respiratory control is the suspected etiology in a majority of infants with apnea. Although neurologic control of breathing has been evaluated in infants surviving an apneic episode, pulmonary mechanics have not been previously measured. Pulmonary mechanics were measured during sleep in ten infants with apnea, aged 45.4 +/- 1.4 (SE) weeks postconception, and 13 control infants, aged 42.0 +/- 0.8 weeks postconception. Infant apnea patients were defined as those having at least one episode of cyanosis, limpness, and apnea requiring vigorous stimulation or resuscitation to restore n…

Auditory brainstem evoked potentials during sleep apnea.

Auditory brainstem evoked potentials (ABEPs) were recorded from seven subjects with predominantly central sleep apnea. ABEPs were recorded during: (1) waking, (2) non-REM sleep between apneic episodes, (3) first half of apneas, and (4) second half of apneas. Latencies of vertex positive peaks III, V, and VI as well as the V-III interpeak latency difference were determined, and the effect of apnea phase on these measures were evaluated. The measures studied did not reveal chronic or acute functional abnormality of auditory brainstem, which may result from or cause apneic episodes during non-REM…

Brainstem evoked potentials in adult sleep apnea.

Brainstem evoked potentials (BSEP) were recorded in 23 patients with adult sleep apnea (ASA). Three patients were studied with all-night polysomnography prior to our testing. They were categorized as having obstructive, central, or mixed sleep apnea depending on the predominant sleep findings. All patients with central sleep apnea had abnormal BSEP with prolongation of wave V. A majority of the remaining patients with obstructive sleep apnea and mixed sleep apnea had abnormal BSEP, but without specific configurations. These findings substantiate our hypothesis that brainstem dysfunction may pl…

Work Productivity Burden Among Patients With Obstructive Sleep Apnea and Obesity Versus Controls Without Obstructive Sleep Apnea or Obesity.

OBJECTIVES: The aim of this study was to examine work loss, motor vehicle accidents, and serious injuries among patients with obstructive sleep apnea (OSA) and obesity ("patients") versus matched controls without OSA or obesity ("controls"). METHODS: Outcomes were compared for patients and controls after matching on baseline demographics, overall and stratified by OSA severity. RESULTS: Patients were 3 times more likely to have a short-term disability (STD) claim (15.7% vs 5.0%, P  < 0.001) and had higher STD costs ($960 vs $305, P  < 0.001) than matched controls. The prevalence of serious i…

Clinical manifestations of pediatric obstructive sleep apnea syndrome: Clinical utility of the Chinese-version Obstructive Sleep Apnea Questionaire-18.

AIMS: Childhood obstructive sleep apnea syndrome (OSA) affects not only the children's physical health, but also their mental development, behavioral problems and learning difficulties. Therefore, an early diagnosis is important. However, the assessment tools of polysomnography are demanding. The Obstructive Sleep Apnea Questionnaire-18 (OSA-18) is designed to screen OSA and has good reliability and validity. The goal of this study was to validate the Chinese version of the OSA-18, to analyze the frequency of symptoms and find the most common symptoms of OSA in Taiwanese children. METHODS: We…

Relationship between T90 and systemic hypertension in patients with obstructive sleep apnea: Data from the European sleep apnea database.

BACKGROUND: Hypertension (HTN) is one of the most prevalent cardiovascular comorbidities in patients with obstructive sleep apnoea (OSA). Although OSA severity measured by the apnoea-hypopnoea index (AHI) is a known risk factor, the role of hypoxic load in HTN remains less well established. METHODS: We analysed data from 12,141 patients with OSA enrolled in the European Sleep Apnea Database (ESADA). Clinical and sleep study parameters were assessed to identify factors associated with HTN. RESULTS: The cohort included 2650 patients with mild OSA, 3339 with moderate OSA, and 6152 with severe O…

Combined laryngopharyngeal reflux and obstructive sleep apnea (CLOSA) - Salivary pepsin test for laryngopharyngeal reflux in obstructive sleep apnea patients.

OBJECTIVES: Reflux disease including gastroesophageal reflux and laryngopharyngeal reflux (LPR) is often found in obstructive sleep apnea (OSA) patients. Endoscopic examination is a gold standard diagnosis for reflux disease. However, the invasive procedure limits its widespread use. The pathophysiological characteristics of LPR are associated with refluxate components, of which pepsin is known to damage the tissues of the larynx and pharynx. Therefore, the detection of salivary pepsin to diagnose LPR becomes a potentially clinical application with noninvasiveness. In this study, we aimed to (…
