Surgical restoration of arm and hand function in people with tetraplegia.

Improved hand and arm function is the most sought after function for people living with a cervical spinal cord injury (SCI). Surgical techniques have been established to increase upper extremity function for tetraplegics, focusing on restoring elbow extension, wrist movement, and hand opening and closing. Additionally, more innovative treatments that have been developed (implanted neuroprostheses and nerve transfers) provide more options for improving function and quality of life. One of the most important steps in the process of restoring upper extremity function in people with tetraplegia is…

TRENDS AND ISSUES IN RELATION TO ALCOHOL DEPENDENCE IN THE ARMED FORCES.

Alcohol dependence syndrome has an estimated prevalence of 1533% of all psychiatric hospitalizations in a year. There is an increasing trend in incidence among JCOs/ORs. The index of admission was highest in Armoured Corps, Mechanized Infantry and Army Medical Corps (AMC). Generally a service personnel becomes dependent at 35 years of age after about 11 years of drinking. The average consumption of alcohol dependent individuals was 6-7 pegs/day. A family history of alcohol abuse/dependence was found in 31 % cases. Alcoholics had high anxiety, depression, extroversion, neuroticism and psychopat…

Delineation of subtelomeric deletion of the long arm of chromosome 6.

Pure subtelomeric deletion of the long arm of chromosome 6 is rare. The frequency of this deletion accounts for approximately 0.05% of subjects with intellectual disability and developmental delay with or without dysmorphic features. Common phenotypes associated with this deletion include intellectual disability, developmental delay, dysmorphic features, seizure, hypotonia, microcephaly and hypoplasia of the corpus callosum. The smallest overlapped region is approximately 0.4 Mb, and contains three known genes. Of these genes, TBP has been considered as a plausible candidate gene for the pheno…

Etiology and Management of Zygomaticomaxillary Complex Fractures in the Armed Forces.

BACKGROUND: The nature of duties in the Armed Forces predisposes its personnel to various kinds of injuries. The rise in violence coupled with an exponential increase in motorized population has contributed towards maxillofacial injuries. The aim of this study was to determine the incidence, cause and pattern of various injuries resulting in fracture of the zygomaticomaxillary complex in Armed Forces personnel and their families. METHODS: A study was conducted between January 2001 - December 2002 in maxillofacial surgical wing of an Army Dental Centre with specific regard to age, sex, locatio…

Intravascular mucormycosis as a cause of arm ischemia in an immunocompromised patient.

Mucormycosis is an opportunistic fungal infection primarily affecting patients with immunosuppression. We present a case of upper limb ischemia secondary to mucormycosis. A 59-year-old man with myelodysplasia presented with an acutely ischemic arm. The patient underwent numerous revascularisation attempts. Following the final procedure, he developed a cerebral infarct and subsequently died. The most common presentations of mucormycosis are of local character. Once established, it can spread rapidly with vascular invasion and in situ thrombosis. Mortality rate is 76%-96%. In this case, it led t…

[Immunoconjugates, drug-armed antibodies to fight against cancer].

Monoclonal antibodies constitute a growing class of therapeutic agents. They are classically used in combination with chemotherapeutic drugs for cancer treatment. The concept of coupling a cytotoxic agent to an antibody can be viewed as a means to confer a selectivity for tumoral cells to highly cytotoxic drugs which cannot be used in human, or a higher power to antibodies which have a low anti-tumoral activity on their own. Gemtuzumab ozogamicin is the only drug-armed antibody available on the market, for the treatment of acute myeloid leukaemia. Other immunoconjugates are currently under cli…

The influence of arm position on thoracic response in side impacts.

The purpose of this study was to investigate the influence of arm position on thoracic response and injury severity in side impacts. A total of sixteen non-destructive side impact tests and four destructive side impact tests were preformed using four human male cadavers. Single-axis strain gages were placed on the lateral and posterior regions of ribs three through eight on the impacted side, and the lateral region of ribs three through eight on the non-impacted side. Thoracic rods attached to ribs five, seven, and nine were used to measure lateral rib deflection. For the non-destructive tests…

Innovative single-armed suture technique for microsurgical vasoepididymostomy.

OBJECTIVES: Vasoepididymostomy outcomes are heavily dependent on the surgeon's microsurgical experience and skill. To avoid back-walling the tubular lumen, the needles are generally placed inside-out through the vasal lumen using double-armed microsutures. These double-armed sutures for infertility microsurgery are very expensive and may be difficult to obtain. We describe a randomized trial that used a novel single-armed suture placement pattern for vasoepididymostomy. METHODS: Male adult Wistar rats underwent vasectomy. Two weeks later, vasoepididymostomies were performed using either a sin…

Variant structures potent to compress the median nerve in the arm.

The most commonly known nerve compression syndrome is carpal tunnel syndrome, in which the median nerve is compressed at the wrist. Along with the pathological processes, some of the anatomically variant structures can also compress the nerves and often leads to confusion in the diagnosis and treatment if one is unaware of such possible anatomical variants. Here, we present a case of possible median nerve compression by an additional belly of coracobrachialis and a fascial sheet from the brachialis, and we also discuss the other possible structures that can compress the median nerve.

Harlequin syndrome with crossed sympathetic deficit of the face and arm.

Harlequin syndrome is characterized by unilateral hyperhidrosis and flushing, which are predominantly induced by heat or exercise. Usually, the sympathetic deficits confine to the face. Rarely, the autonomic deficits involve the arm or the parasympathetic neurons in the ciliary ganglia. We report a 43-yr-old woman who presented with facial flushing and sweating in the right side, which were mainly induced by exercise. The facial flushing accompanied relative coldness in the right arm. Valsalva maneuver, cold pressure and 0.125% pilocarpine test, and computed tomography of the chest were normal…

Severe lymphedema of the arm as a potential cause of shoulder trauma.

The aim of this study was to determine whether lymphedema of the arm is associated with traumatic injury to the shoulder and to assess the role of lymphatic physiotherapy in reducing disabling shoulder pain. The study group consisted of 10 women aged 58-81 years (mean 66.9) with arm lymphedema after surgery for breast cancer. The average interval between the operation and the appearance of lymphedema was 9.8 years. All patients complained of shoulder pain. Five patients had a tear in the supraspinatus muscle diagnosed by ultrasound examination, and 5 had chronic bursitis; the nonaffected arm s…

[The problems in replantation of limbs amputated through the arm region].

The problems in replantation of arms were analysed on the ground of nine such replantations performed in Center of Replantations of Limbs in Trzebnica in the same number of patients (7 men, 2 children, 1 woman) during the seven year period 1993-2000. The range of age of the patients was from 12 to 62 years (35.5 on average). Among the problems discussed were such as qualification for the operation, operative technique (limb shortening, perfusion of vessels, half-open anastomosis of veins, neurotisation of remaining nerves) evaluation scale. Eight of the amputations were the results of crush-av…

[Physiopathology of repetitive movements and vibrations of the hand-arm system].

This paper is a review of some recent findings in physiopathology of work related upper limb disorders and hand-arm-vibration-syndrome (HAVS). In a first section the data relative to work related upper limb musculoskeletal disorders are collected in three groups referring respectively to the muscle, to the tendon and to the peripheral nerve. Furthermore the physiopathological data are analyzed with respect to the Strain Index proposed by Moore and Garg. In a second section the physiopathological mechanisms of HAVS are summarized.
