Openness to Vegetarian Diets Among Patients With Chronic Disease: Racial Parity Between Black and White Patients.

Many lifestyle medicine and primary care practitioners are actively seeking to leverage the benefits of plant-based diets in preventing and managing chronic diseases among their patients. It is important to understand the key barriers that underlie an individual's openness to adopting a vegetarian diet in an urban, metropolitan area with a predominately Black population. This study used a cross-sectional survey design to assess barriers and openness to vegetarian diets in three outpatient clinics in the Baltimore area. Barriers surveyed included anticipated stigma, tastiness, financial cost, c…

Alzheimer's disease neuropathology and longitudinal change in subjective cognitive complaints.

INTRODUCTION: Subjective cognitive complaints often precede declines in objective measures of cognitive performance. Associations of incipient Alzheimer's disease (AD) neuropathology with subjective cognitive complaints may be detectable earlier than associations with neuropsychological testing among cognitively normal individuals. METHODS: We examined the independent associations of positron emission tomography measures of amyloid beta and tau pathologies with longitudinal subjective complaints and memory among 91 cognitively normal Baltimore Longitudinal Study of Aging participants using li…

Having naloxone when you need it: Factors associated with owning and carrying naloxone among people who use opioids.

It is critical to have naloxone readily available in the event of an opioid overdose. This study examines the prevalence of owning and carrying naloxone and factors associated with ownership and carriage among people who use opioids in Baltimore, Maryland, from December 2022 to January 2025. Among the 780 community-recruited study participants who were aged 18 years or older and currently used non-prescription opioids, most (78.2 %) reported owning naloxone, with the majority (69.4 %) keeping it at their residence. While 41.8 % of participants reported always having naloxone available when usi…

Temporal unfolding of declining episodic memory on the Free and Cued Selective Reminding Test in the predementia phase of Alzheimer's disease: Implications for clinical trials.

INTRODUCTION: Free and Cued Selective Reminding Test (FCSRT) performance identifies patients with preclinical disease at elevated risk for developing Alzheimer's dementia, predicting diagnosis better than other memory tests. METHODS: Based on literature mapping FCSRT performance to clinical outcomes and biological markers, and on longitudinal preclinical data from the Baltimore Longitudinal Study of Aging, we developed the Stages of Objective Memory Impairment (SOMI) model. Five sequential stages of episodic memory decline are defined by Free Recall (FR) and Total Recall (TR) score ranges and…

Regular low-calorie sweetener consumption is associated with increased secretion of glucose-dependent insulinotropic polypeptide.

Low-calorie sweeteners (LCSs) are widely used for weight control despite limited evidence of their effectiveness and studies linking LCS consumption with incident obesity. We tested the hypothesis that regular LCS consumption is associated with higher postprandial glucose-dependent insulinotropic polypeptide (GIP) secretion, which has been linked to obesity. We used data from participants in the Baltimore Longitudinal Study of Aging who had completed a diet diary, had at least one visit during which they underwent an oral glucose tolerance test (OGTT), and had no diabetes. Of 232 participants,…

Victimization experiences, substance misuse, and mental health problems in relation to risk for lethality among African American and African Caribbean women.

The purpose of this study was to investigate the association of intimate partner victimization experiences, mental health (MH), and substance misuse problems with the risk for lethality among women of African descent. Data for this cross-sectional study were derived from a large case-control study examining the relationship between abuse status and health consequences. Women were recruited from primary care, prenatal, or family planning clinics in Baltimore and the U.S. Virgin Islands. Logistic regression was used to generate the study findings. Among 543 abused women, physical and psychologic…

Thenar eminence quantitative sensory testing in the diagnosis of proximal median nerve compression.

Diagnosis of proximal median nerve compression (PMNC) remains a clinical challenge. The authors hypothesized that measurement of the sensibi … touch (2PS) were measured with the Pressure-specified Sensory Device (Sensory Management Services, Baltimore, Maryland). There was no significant difference in thenar eminence sensibility between the healthy volunteers and the patients with carpal tunnel syndrome. In contrast, patients with PMNC had higher cutaneous pressure thresholds for 1PS (p<0.001), 2 …

A new xeroderma pigmentosum group C poly(AT) insertion/deletion polymorphism.

We found a common biallelic polymorphism (PAT) in the xeroderma pigmentosum complementation group C (XPC) DNA repair gene consisting of an insertion of 83 bases of A and T [poly(AT)] and a 5 base deletion within intron 9. We developed a PCR assay to resolve the XPC PAT+ and PAT- alleles and found that the PAT+ allele frequency was 0.44 in 156 cancer-free donors from the Johns Hopkins School of Public Health, 0.41 in 263 cancer-free donors from the Baltimore Longitudinal Study of Aging and 0.36 in samples from 216 unselected donors from NIH. We also found a single nucleotide polymorphism in exo…

Reducing cigarette sales to minors in an urban setting: issues and opportunities for merchant intervention.

INTRODUCTION: Intervention studies to reduce cigarette sales to minors have been conducted primarily in suburban settings. Little is known about sociocultural factors influencing cigarette sales to minors in urban settings. This study sought to determine sociodemographic and cultural factors that may play a role in cigarette sales and in efforts to reduce sales to minors in urban areas. METHODS: Merchant education and follow-up surveys were conducted in small local stores in predominantly African-American urban census tracts in Baltimore. The stores had prior evidence of cigarette sales to mi…

Epistaxis: a retrospective review of hospitalized patients.

A 2-year retrospective study (August 1, 1992, to July 31, 1994) reviews medical records of 249 hospitalized epistaxis patients at seven Baltimore-area hospitals. This review represents the first multihospital study of patients hospitalized with epistaxis. The study evaluates multiple factors associated with epistaxis: demographics, underlying medical diseases, use of anticoagulation medication, site of epistaxis, and month of hospital admission. Blood transfusion requirement, hematocrit level, length of stay, treatment, and local/systemic complications are studied as well. The month of hospita…

A case-control and family study of Waldenstrom's macroglobulinemia.

Waldenstrom's macroglobulinemia (WM) is a rare disorder of lymphoid and plasma cells characterized by an immunoglobulin M (IgM) monoclonal gammopathy, clinical and immunopathologic similarities with other lymphoproliferative neoplasms, but the etiology of which is unknown. We undertook the first case-control study of this disorder among 65 cases, comprising 87% of all WM patients diagnosed during 1969-1983 in the greater Baltimore, Maryland area. Compared with 213 hospital controls without cancer, cases were slightly better educated, but there were otherwise no differences in sociodemographic …

Blood and plasma donations among a cohort of intravenous drug users.

We evaluated the blood and plasma donation histories of a cohort of 2921 intravenous drug users in Baltimore, Md, and correlated these histories with their human immunodeficiency virus (HIV) serologic status, numbers of CD4 lymphocytes in the peripheral blood, and stigmata of intravenous drug use (scarred veins). Of the 793 intravenous drug users (27.1%) who had donated blood or plasma, 652 (82.2%) donated after they had started using intravenous drugs. Most subjects donated at commercial plasma centers, where they were paid $10 to $15 per donation. Although the HIV-1 seroprevalence of the ent…

Taxol toxicity. Epithelial necrosis in the gastrointestinal tract associated with polymerized microtubule accumulation and mitotic arrest.

Taxol, an antineoplastic agent with a novel mechanism of action, is currently undergoing Phase I trials at The Johns Hopkins Hospital, Baltimore. The authors recently observed striking mitotic arrest associated with epithelial necrosis and ulceration in an esophageal biopsy specimen. The biopsy specimen was taken from a patient who received taxol the day before endoscopy. Review of our autopsy files revealed four other cases in which taxol had been administered. Sections of esophagus, stomach, small intestine, colon, liver, skin, bone marrow, and testes were examined for evidence of mitotic ar…
