Endoscopic sphincterotomy (EST) is regarded as an alternative therapeutic approach to the surgical treatment of choledocholithiasis. To clarify the indication for each of these two methods, a prospective study has been performed comparing patients with calculi of the common bile duct who had undergone endoscopic or surgical treatment. The diagnosis of gallbladder and liver diseases depends primarily on noninvasive techniques (ultrasound and computer tomogram). Gallstones can be detected by ultrasonography in more than 90% of such patients and this is a sufficient indication for operation. The preoperative diagnosis of stones of the biliary tract by sonography is however, unreliable. Even a preoperative endoscopic retrograde cholangio-pancreaticography cannot justify foregoing an intraoperative cholangiogram. It is reported on the very rare occurrence of a benign tumor in common hepatic bile duct of a 33-year-old woman, associated with obstructive jaundice. Clinical diagnosis and treatment are described, histological typing like fibroblastic tumor with neurogenic parts are discussed and a short review of the literature is given.

The incidence of common bile duct (CBD) pathology in a group of patients with benign biliary disease (n = 505) was found to be 23.2 per cent. The spectrum included 111 patients (90.2 per cent) with CBD stones, 37 of whom (33.3 per cent) had no symptoms or findings pre-operatively indicating CBD involvement. A case is reported with a rare anomaly of biliary ducts--lengthened hepatic ducts, that are connected right above papilla of Vater--established by endoscopic retrograde cholangiography and confirmed at operation. The patient was twice operated on the occasion of cholelithiasis: cholecystectomy was performed with the first operation, the gall bladder being connected to the right hepatic duct, but the second biliary duct was not established. The other biliary duct--the left hepatic duct was obstructed by a calculus and dilated, established at the second operation.

"White bile" is the colorless fluid occasionally found in occluded biliary systems. The absence of pigments in this "bile" was not satisfactorily explained. The objectives of this study were to assess its etiology. In dogs, "white bile" developed whenever both the common bile duct and the cystic duct were ligated. In comparison, dark green ("black") bile occurred when only the common bile duct was ligated leaving the gallbladder in communication with the obstructed ducts. From 1972 to 1981, 806 patients suffering from diseases of the bile ducts were seen at our Dept. 20% of them were over 70 years of age. Mortality rate, types of surgery, complications and causes of death are dealt with in detail. All the facts given demonstrate the necessity of early surgical intervention in case of surgical diseases of the bile ducts.

The histopathology of 43 cases of carcinoma in the extrahepatic bile ducts was reported. All four cases of noninfiltrating carcinoma were papillary or polypoid in gross shape. Only one of the eight cases of papillary or polypoid cancers penetrated beyond the fibromuscular layer of the bile ducts, while most cases of nodular or flat cancers spread deeper than the fibromuscular layer. The blood supply of the extrahepatic bile ducts was investigated in 15 human specimens. Human embryology explains the origin of the main blood supply in this area by the a. pancreaticoduodenalis posterior superior, the a. cystica and the a. hepatic propria. Other arteries are inconsistent and functionally unimportant. 2 or 3 marginal arteries are predominant in the arterial system surrounding the common bile duct. Surgery of the extrahepatic bile ducts should be performed with respect to this particular arterial network.

Following oral administration of 1.5 g of sodium salicylate or acetylsalicylate to five patients with indwelling T-tubes there was an approximately 50% increase in bile flow. The increase appeared to be in the bile-acid independent fraction of canalicular flow and was not accompanied by a change in lithogenicity. Neither agent would be of use in the medical treatment of cholesterol gallstones.

The prerequisite for the sonographic diagnosis of biliary tract pathology is the recognition of the normal biliary tract even in neonates, as demonstrated in a prospective study on 78 neonates.
