Blindness in leprosy patients of Kaduna State, Northern Nigeria.

We examined the eyes of 311 leprosy patients to determine the prevalence of blindness and impaired vision, and the causes of blindness in leprosy patients in Northern Nigeria. There was impaired vision or blindness in 74 eyes. Both eyes were affected in 5.1% of patients while 2.9% were totally blind. Leprosy alone was responsible for total blindness in 1.3% of patients, while other ocular diseases contributed to blindness in 1.6%. Exposure keratitis (21.3%), corneal opacities (13.5%) and chronic uveitis (10.1%) were the commonest leprotic cause of blindness. Primary care and early intervention…

[Etiology of blindness in remote area: a clinic report].

This paper analyzes the cause for blindness in 408 patients of Xinfeng People's Hospital from 1977 to 1991. The results show that cataract is the major cause for blindness (32.84%). The others are eye injury in patients below age 46, cataract and glaucoma in patients above age 46. It is suggested that the stress of prevention and treatment for blindness should be put on the prevention of eye injury in juveniles and middle aged people, and the treatment for senile cataract in remote mountain areas.

Affected females in X-linked congenital stationary night blindness.

Most heterozygous (carrier) females in families with X-linked congenital stationary night blindness are asymptomatic. Several anecdotal cases of manifesting females in X-linked congenital stationary night blindness have been reported, but few clinical details are available. The authors report clinical, electroretinographic, and dark adaptation studies of four affected females from a five-generation family with X-linked congenital stationary night blindness. Each of the manifesting females was the daughter of a different, asymptomatic, carrier mother. None of the 14 daughters of the 9 affected …

Pneumosinus dilatans as the aetiology of progressive bilateral blindness.

Pneumosinus Dilatans is a rare condition of the craniofacial skeleton which was diagnosed in an adolescent male who presented with progressive bilateral blindness and many features of osteodysplasty (Melnick-Needles Syndrome). The clinical course and unusual pathology of this case which included the compression of both optic nerves within long tubes of bone are described, together with the surgical intervention performed to arrest the patient's loss of vision.

Postpartum transient blindness probably related to undiagnosed brain tumor.

Nausea and vomiting are so frequent under regional anesthesia during parturition that these symptoms can be misleading and may cause the clinician to overlook a potential intracranial lesion. In the case we have reported, it was not until the onset of blindness that an intracranial lesion was suspected. Although the incidence of brain tumor during pregnancy is low, it does occur with various manifestations and has been associated with blindness. Therefore, prompt neurologic investigation may be warranted in patients complaining of visual changes, even though transient blindness can occur in th…

Autosomal dominant stationary night-blindness. A large family rediscovered.

In 1909, 2 years after the famous publication by Nettleship, a large family with congenital stationary night-blindness of the 'Nougaret type' was published by the Danish district surgeon, Sigurd Rambusch. In 1990 the 'Rambusch family', still resident in the original area, was sought out and rediscovered, at which time the reconstructed part of the pedigree comprised more than 200 affected persons in 11 generations. Dark adaptometry and electroretinography were performed on a few affected family members, including a descendant with a uniocular affection. The pedigree is presented and recordings…

Cortical blindness during chemotherapy: clinical, CT, and MR correlations.

Two patients treated by chemotherapy for a non-Hodgkin malignant lymphoma developed focal neurological symptoms including disorientation, hemoplegia, and cortical blindness 5 and 15 days after the end of a polychemotherapy course, including methotrexate and vindesine. In both patients ECG and blood pressure were normal. Case 1 had a slight increase of protein level without cells on CSF examination and presented with a paralytic ileus. Case 2 developed an inappropriate antidiuretic hormone secretion (IADHS) syndrome. In both cases, noncontrast CT scans showed bilateral, symmetrical low density …

Variable expressivity in X-linked congenital stationary night blindness.

X-linked congenital stationary night blindness (CSNB) is a well-documented disorder in which the most striking clinical features are impaired night vision, nystagmus and myopia. Recent reports have highlighted differing features between families, and it has been suggested that these discrepancies may be the result of two loci on the X chromosome or of two mutant alleles. We outline the clinical and visual function findings in 42 affected members from 10 families and 1 adopted person. There was a relative unawareness of the disorder in clinical practice. At least one of the main features of CSN…

A simple field test for identification of night-blindness.

A simple and replicable field test to measure dark adaptation time has been developed. It required a darkened room with a 5 watt bulb covere … gh income group (PC); underprivileged school-age children (n = 44) with a positive history of night-blindness (NB), and their age/sex pair-matched (n = 44) non-night-blind controls (NNB). The NB group had a significantly higher mean dark adaptation time as compared to the NNB and PC groups (14.9 sec vs 11.9 and 11.3 sec respectively). The mean serum vitamin …

Monocular blindness after penetrating trauma to the carotid artery.

Near-total or total blindness caused by chronic ocular ischemia is a well-recognized complication of severe atherosclerotic occlusive disease of the carotid artery (CA) but has not been previously reported in the English-language literature as a sequela of an occlusive CA injury. This report describes a patient who sustained a gunshot wound to the neck, which injured the ipsilateral internal CA, external CA, and vertebral artery. The location of the injuries precluded arterial repair. The patient was neurologically intact after operative exploration. Twenty months after the injury a marked los…

Temporary blindness and eclampsia. A report of 2 cases.

Temporary blindness in 2 patients with eclampsia was associated with oedema of the occipital lobe of the brain. In addition 1 patient had retinal detachment. Management included the use of mannitol, dexamethasone and furosemide to reduce cerebral oedema. Both patients recovered their vision within 48 hours of this management.

The Eisdell pedigree. Congenital stationary night-blindness with myopia.

A pedigree of X-linked congenital stationary night-blindness, originally published by Nettleship, is presented with abstracts from his private correspondence. An affected descendant has supplied letters and the original working pedigree which led to the publication of the extended pedigree in 1912.

World Health Organization's programme on the prevention of blindness.

The WHO Programme for the Prevention of Blindness was established in 1978, reflecting the need for action against the burden of avoidable blindness, which constitutes an increasingly serious socioeconomic problem in many countries. A vast majority of the world's blind live in developing countries, where unoperated cataract, trachoma, xerophthalmia and onchocerciasis are the main causes of visual loss. Thus, more than two-thirds of blindness encountered in such areas is either preventable or curable. The objectives of the WHO Programme for the Prevention of Blindness are to reduce the amount of…

Night blindness revisited: from man to molecules. Proctor lecture.

Night blindness is a frequent concomitant of retinal disorders, many of which are of genetic origin. Through the use of quantitative noninvasive test procedures it has been possible to study patients with these hereditary conditions and to show that the visual abnormalities often result from defects in the functional properties of the rod photoreceptors. More important, the uniqueness of the functional disturbance in the various types of night-blinding disorders suggests that each involves a specific aspect of the rod's internal machinery, i.e., the molecular processes devoted to transduction,…

A 6-month-old baby girl with congenital unilateral blindness had an eyelid coloboma and clear evidence of an old vascularized corneal perforation, indicating that the cause of the blindness was an ocular trauma sustained during a midtrimester amniocentesis.
