Out-of-office blood pressure (BP) monitoring is a critical component of modern hypertension diagnosis and management. Measuring BP outside of clinic reduces the stress response that contributes to white coat hypertension and also allows for the identification of masked hypertension, yielding more accurate cardiovascular disease (CVD) risk assessment and improved CVD prevention. Home BP monitoring and 24-hour ambulatory BP monitoring outperform office BP in predicting CVD outcomes and are cost-effective aspects to cardiovascular health promotion by preventing unnecessary treatment, reducing cli…

KEY POINTS: Here, we report the relationship between arterial and venous blood gas parameters (pH, pressure of carbon dioxide, and bicarbonate) on the basis of 5419 samples collected concurrently. The data show that the venous blood gas can be transformed with reasonable confidence intervals to arrive at a corresponding arterial blood gas.
BACKGROUND: Arterial blood gas (ABG) analysis remains the gold standard for diagnosis and management of acid-base disorders. Obtaining arterial blood through arterial puncture, however, is cumbersome, at times difficult and carries potential risks. For thes…

OBJECTIVE: To evaluate the relationship between standardized arm and calf blood pressure (BP) measurements obtained in the habitual posture of hemodialysis (HD) and to derive calf BP values corresponding to abnormal arm BP thresholds.
METHODS: This cross-sectional study included 277 HD participants. Arm and calf BP were measured immediately before dialysis in the habitual HD posture. Linear regression estimated calf values corresponding to arm BP thresholds of 140/90 mmHg and 130/80 mmHg. Receiver operating characteristic (ROC)/Youden analysis identified optimal calf cutoffs. Cutoff-specific …

To address critical challenges in clinical blood shortages and limited preservation periods, particularly affecting emergency transfusions, transgenic pig red blood cells (RBCs) emerge as a promising solution. We performed multi-gene editing on pig RBCs by knocking out three Xeno antigen genes (GGTA1, B4GALNT2, CMAH) and introducing high expression of human CD55 and CD47, generating 5-gene edited (5GE) RBCs. After co-incubating with human blood, we assessed their survival rate, along with physicochemical indicators related to hemolytic reactions and inflammatory responses. We further compared …

ABO typing is essential for preventing ABO incompatibility transfusion reactions. Discrepancy exists when reactions in forward grouping do not match with reverse grouping. Any discrepancies reported should be investigated so that correct blood group is reported minimizing the chances of transfusion reaction. The most common causes of ABO discrepancy are cold autoantibodies and missing serum reactivity. We report a rare alloantibody anti-PP1Pk discovered during the resolution of a grouping difficulty with a positive control. Anti-PP1Pk is associated with hemolytic transfusion reactions. In our …

Red blood cells (RBCs) and platelets contribute to the coagulation capacity in bleeding and thrombotic disorders. The thrombin generation (TG) process is considered to reflect the interactions between plasma coagulation and the various blood cells. Using a new high-throughput method capturing the complete TG curve, we were able to compare TG in whole blood and autologous platelet-rich and platelet-poor plasma to redefine the blood cell contributions to the clotting process. We report a faster and initially higher generation of thrombin and shorter coagulation time in whole blood than in platel…

BACKGROUND AND OBJECTIVES: Blood donation can be a potentially stressful event, leading to the activation of an acute stress response. Knowing and identifying potential stressors could help in optimizing the donation experience. The present study aimed to measure the physiological and psychological stress changes before, during and after blood donation.
MATERIALS AND METHODS: Physiological and psychological stress response was assessed in 70 blood donors. To evaluate physiological stress response, pulse rate, respiratory rate, blood pressure (BP), beat-to-beat BP and lead II electrocardiogram…

While differences among donors has long challenged meeting quality standards for the production of blood components for transfusion, only recently has the molecular basis for many of these differences become understood. This review article will examine our current understanding of the molecular differences that impact the quality of red blood cells (RBC), platelets, and plasma components. Factors affecting RBC quality include cytoskeletal elements and membrane proteins associated with the oxidative response as well as known enzyme polymorphisms and hemoglobin variants. Donor age and health sta…

Computational models based on recent maps of the RBC proteome suggest that mature erythrocytes may harbor targets for common drugs. This prediction is relevant to RBC storage in the blood bank, in which the impact of small molecule drugs or other xenometabolites deriving from dietary, iatrogenic, or environmental exposures ("exposome") may alter erythrocyte energy and redox metabolism and, in so doing, affect red cell storage quality and posttransfusion efficacy. To test this prediction, here we provide a comprehensive characterization of the blood donor exposome, including the detection of co…

BACKGROUND AND OBJECTIVES: Based on the Council of Europe directive which dictates regulatory requirements in Australia, blood donors are currently deferred from donating for 4 months after an endoscopic procedure if either polyps were removed or a biopsy sample was taken. We aimed to assess the incidence of blood-borne viruses (BBVs) (HIV, hepatitis B and C) in blood donors who donated after an endoscopic procedure and evaluate the risk to blood safety through risk modelling.
MATERIALS AND METHODS: Donors from 1/1/2013 to 31/12/2017 with an endoscopy deferral on their blood donor file with p…

Malaria has had a major effect on the human genome, with many protective polymorphisms-such as the sickle-cell trait-having been selected to high frequencies in malaria-endemic regions1,2. The blood group variant Dantu provides 74% protection against all forms of severe malaria in homozygous individuals3-5, a similar degree of protection to that afforded by the sickle-cell trait and considerably greater than that offered by the best malaria vaccine. Until now, however, the protective mechanism has been unknown. Here we demonstrate the effect of Dantu on the ability of the merozoite form of the…

Multiple blood pressure (BP) measurements allow an evaluation of BP variability and BP‑derived arterial stiffness indices. Periodic variations in BP are well known, from beat‑to‑beat BP readings in intra‑arterial measurement to seasonal variations in BP. Diurnal BP variation has been investigated in relation to its prognostic value. People with night‑to‑day BP ratio of 1 or higher, that is, those with a higher nocturnal than daytime BP, were older than those with normal dipping status at baseline and had a greater risk of cardiovascular mortality and morbidity. Short‑term BP variability was ev…

INTRODUCTION: It has been suggested that blood donation reduces risks of developing cardiovascular diseases such as heart failure, atherosclerosis, and stroke. Although there are known benefits of blood donation, the inclination of people of the Kurdistan Region of Iraq to donate blood is not known. Therefore, the aim of this study was to determine demograpic and blood biochemical profiles of regular and first-time blood donors in the Sulaimani province of North Iraq.
METHODS: A cross-sectional study was conducted at the Sulaimani Blood Bank, during the period of April 1, 2016 to March 28, 20…

BACKGROUND: Clinical practice guidelines endorse automated office blood pressure (AOBP) measurement as the preferred in-office measurement modality. However, recent data indicate that this method may underestimate daytime ambulatory BP. The objective of this study was to further assess the comparability of mean AOBP and daytime ambulatory BP in clinical practice.
METHODS: A retrospective cross-sectional chart review was conducted of 96 consecutive patients referred from primary or specialty care practices to a tertiary care ambulatory BP monitoring service. Six AOBP readings were taken using …

This study evaluated the accuracy and reproducibility of a human portable blood glucose meter (PBGM) for canine and feline whole blood. Reference plasma glucose values (RPGV) were concurrently measured using glucose oxidation methods. Fifteen healthy dogs and 6 healthy cats were used for blood sampling. Blood glucose concentrations and hematocrits were adjusted using pooled blood samples for our targeted values. A positive correlation between the PBGM and RPGV was found for both dogs (y = 0.877, x = -24.38, r = 0.9982, n = 73) and cats (y = 1.048, x = -27.06, r = 0.9984, n = 69). Acceptable re…

BACKGROUND: Studies have repeatedly demonstrated that blood donors experience lower mortality than the general population. While this may suggest a beneficial effect of blood donation, it may also reflect the selection of healthy persons into the donor population. To overcome this bias, we investigated the relation between blood donation frequency and mortality within a large cohort of blood donors. In addition, our analyses also took into consideration the effects of presumed health differences linked to donation behavior.
