Undergrowth: brachydactyly.

Brachydactyly simply means "short finger", but it has a clinical spectrum ranging from minor digital hypoplasia to complete aplasia. The index and small fingers are the most commonly affected digits, and the middle phalanx is the most commonly affected bone. Presentation of brachydactyly is highly variable-a digit may be just slightly shorter because of reduced growth of one phalanx or the metacarpal, or there may be complete absence of one or more phalanges, or there may be complete aplasia of the digit or multiple digits. This article discusses the results of conventional reconstruction usin

[Distraction osteogenesis of the mandible in craniofacial abnormalities].

In recent years, lengthening the human mandible by distraction osteogenesis has become an accepted treatment to correct severe mandibular hypoplasia. Using intraoral unidirectional and extraoral bidirectional distraction devices we report about our experiences and results in the application of the bone distraction technique in four selected cases of syndromal disease, including various forms of mandibular hypoplastic malformations. The patients involved were a boy with Pierre Robin syndrome, a girl with unilateral facial hypoplasia in Goldenhar's syndrome, a case with Nager's syndrome, and a r

[Comparison of Modifications of Femoral and Tibial Lengthening in Children].

PURPOSE OF THE STUDY Limb lengthening has always belonged to the most complex surgical techniques in paediatric orthopaedics. In our study, we compared the results of femoral and tibial lengthening using three different surgical techniques. The presented study aimed to shorten the duration of external fixation to a minimum and to reduce the complication rate. MATERIAL AND METHODS The retrospective study compared 74 patients (38 boys, 36 girls) who had undergone stepwise progressive lengthening of the femur or tibia between the years 2007 and 2019. The most frequent indication was the proximal

Can polyaryletherketone cage be used to achieve union and maintain correction in anterior calcaneal lengthening osteotomy for treatment of flexible flatfoot?

A structural graft is often used to maintain correction and achieve union after anterior calcaneal lengthening osteotomy for treatment of flexible flatfoot. Autograft, the current gold standard, is limited in availability and configuration and is associated with donor site morbidity in as much as 48%, whereas the alternative allograft carries risks of disease transmission and collapse. Polyaryletherketone cage, with a healing rate similar to that of autograft, high stability, and no donor-site morbidity, has been used in spine surgery. However, its use has not been documented in foot and ankle

Limb salvage using acute shortening and gradual lengthening after failed free flap in a hemodialysis patient with Gustilo IIIB open tibial fracture: a case report.

BACKGROUND: Limb salvage in Gustilo IIIB open tibial fractures is particularly challenging in patients with chronic kidney disease undergoing hemodialysis, owing to increased susceptibility to infection, impaired bone healing, and vascular fragility. When microsurgical reconstruction fails, treatment options become extremely limited, particularly in cases with compromised vascularity such as single-vessel limbs. Acute shortening and gradual lengthening (ASGL) using the Ilizarov method represents a vascular-independent reconstructive strategy. CASE PRESENTATION: A 65-year-old man undergoing he

Rigid intramedullary nailing of lower limb segments in children and adolescents with metabolic bone disease.

Children and adolescents with metabolic bone disease present to the orthopedic surgeon with pain, fractures (which may be impending), and deformity. Different modalities of orthopedic management are available. Scant literature exists on the use of rigid intramedullary nailing in this population. This study sought to evaluate the utilization of this treatment modality in the pediatric cohort, focusing on indications, techniques, and outcomes of the procedures. A retrospective review was performed over an 11-year period at a single tertiary pediatric institution in the UK. Medical records and ra

Local injection of abaloparatide promotes mandibular condyle lengthening in adolescent rats via enhancing chondrogenesis and ossification.

BACKGROUND: Mandibular condylar hypoplasia negatively affects patient's facial appearance and dentofacial function. OBJECTIVE: To investigate the effect of local injection of the drug abaloparatide (ABL), an analogue of parathyroid hormone related protein (PTHrP), on promoting lengthening of the mandibular condyle. METHODS: Thirty adolescent male Sprague-Dawley rats were randomly divided into two groups, which received the injection of ABL or normal saline (the control) every 3 days in the temporomandibular joint (TMJ) cavity. Cone-beam computed tomography and immunohistochemistry assays wer

Metacarpophalangeal pattern profile analysis in clinical genetics: an applied anthropometric method.

The hand is a complex anatomical structure with the component bones susceptible to a combination of environmental and genetic factors that may affect the bone length and width. The alterations may involve a single bone or specific group of bones. The metacarpophalangeal pattern profile (MCPP) developed by Poznanski, Garn, and others (Poznanski et al. Birth Defects VIII (5): 125-131, 1972) is a graphic representation of the relative lengthening and shortening of the 19 tubular bones of the hand useful for diagnosis, comparison of dissimilar patients, and gene carrier detection. The profile hand

Guided Growth: A Novel Treatment for Anterolateral Bowing of the Tibia.

BACKGROUND: Tibial pseudoarthrosis is a source of considerable morbidity in children with neurofibromatosis. Preventing the progression of anterolateral bowing of the tibia (ALBT) to tibial pseudoarthrosis is difficult. The aim of this paper is to report the successful use of guided growth to prevent tibial pseudoarthrosis in a child with neurofibromatosis. METHODS: With local ethical approval, we reviewed the case notes and radiographs of a child with pronounced ALBT who was treated with guided growth to correct the deformity and prevent fracture. RESULTS: At 4-year follow-up guided growth

[Treatment of tibial pseudoarthrosis. Complications after intramedullary, allogeneic fibular grafting].

A 24-year-old woman underwent cosmetic bilateral tibial lengthening with severe complications. In all, 15 operations, including allogeneic fibular grafting of both tibia, were required to treat unstable bilateral non-union, malalignment, and osteomyelitis of the right tibia. The present article describes the surgical strategy of revision to achieve good recovery with full consolidation and proper alignment of the lower leg. Furthermore, the indications for allogeneic bone grafting, which was described by Erich Lexer 100 years ago, are discussed. For surgical revision, a T-external fixator was u

Femoral nailing to treat fractures after lengthening for congenital femoral deficiency in young children.

Congenital femoral deficiency in children can be treated with femoral lengthening. A common complication is fracture soon after removal of the external fixator, often despite prophylactic hip spica cast application. These fractures present special challenges because the patients have tight soft tissues and sclerotic intramedullary canals. We treated nine such fractures in eight children (average age, 5.4 years). Most were 'spontaneous' events resulting in transverse fracture through regenerate bone or pin sites. All were stabilized with intramedullary Rush pins using special insertion techniqu

Somatosensory evoked potential evaluation of acute nerve injury associated with external fixation procedures.

The effects of different mechanisms of acute nerve injury on peripheral nerve function during K wire application and 1 stage limb lengthening were evaluated prospectively in 24 goats using somatosensory evoked potentials. Stable somatosensory evoked potential recordings throughout 3-day experiments were obtained in animals with wires placed at a distance from or directly adjacent to a nerve but without producing any tension or pressure. Complete loss of the peroneal nerve somatosensory evoked potentials occurred if this nerve was perforated by wire, underwent excessive pressure by wire, or had

Prophylactic Intramedullary Rodding Following Femoral Lengthening in Congenital Deficiency of the Femur.

INTRODUCTION: Femoral fracture after lengthening of congenital femoral deficiency (CFD) is a common complication with a high incidence, up to 50%. The purpose of this study is to determine whether prophylactic intramedullary Rush rodding after lengthening for CFD is an effective method to prevent femoral fracture and to assess any complications that may result. METHODS: Forty-five femoral lengthenings (43 CFD patients) were performed using external fixation. At the time of frame removal (except for 3 cases, 8 d later), prophylactic intramedullary Rush rods were inserted. Special reaming techn

Creation of the bone bank of the Rabat and Casablanca region in Morocco.

For a long time the use of bone grafting has demonstrated its interest in orthopedic surgery and traumatology. The autografts which are still very frequently used present various problems. On the one hand, it is necessary to find a correct mechanical quality and a sufficient quantity of bone. On the other hand, the graft removal lengthens the operative time and generally painful in postoperative. These disadvantages of autografts have led to the development of bone allografts. Indeed, the low immunogenic power of the bone, the good integration of the graft and the ease of bone preservation tec

Evans Calcaneal Osteotomy in Flexible Flatfoot Deformity: A Podiatry Institute Perspective.

The Evans calcaneal osteotomy is a powerful lateral column lengthening procedure that creates multiplanar correction in the flexible flatfoot deformity. The procedure was popularized within podiatric medicine and surgery by Dr James V. Ganley. The Podiatry Institute faculty modified the procedure over the years. It has stood the test of time as a reliable procedure with good bone healing and predictable radiographic correction. This article describes its history along with its indications, surgical technique, and possible complications.

[Lengthening, axial correction and segmental reconstruction of the extremities using the distraction method].

Between 1988 and 1992 lengthenings axial, corrections and reconstructions have been performed by distraction on lower arms, upper and lower legs in 25 patients because of deviations in length due to either congenital or acquired (multiple cartilaginous exostoses, posttraumatic conditions, congenital pseudarthrosis deformations or defects (fibula-defect, amnion-band-complex, amputation). On the average elongation amounted to 4 cm for an average installation time of the fixator system of 22 weeks and a mean duration of 25 weeks for the adaptation to full strength. A modified Wagner technique for femoral lengthening in skeletally mature patients with poliomyelitis.

A modified Wagner technique using an Orthofix was employed for femoral lengthening in 62 skeletally mature patients with residual poliomyelitis. The first stage was similar to that of the original Wagner technique. However, distraction was started after 5 to 7 days at a rate of 1.5 mm per day. At the second stage, a neutralization plate was applied and a corticocancellous iliac strut graft inserted in the distracted site, preferably under compression. Cast immobilisation was not used and a long leg brace with a thigh corset allowed early knee movement and weightbearing. The average age at oper

Distraction lengthening following growth cessation due to thumb replantation in a child.

Growth disturbance due to premature physeal closure as a sequela of digit replantation, on occasion, can cause shortening, resulting in suboptimal function. A simple method for correction using only local tissues is possible via gradual bony distraction. This was attempted in a toddler, whose dominant thumb was amputated by a riding lawn mower when he was an infant, because replantation caused significant growth cessation. The first metacarpal distraction over 64 days netted 22 mm of lengthening. Because a further immobilization period to allow spontaneous consolidation by neo-osteogenesis was

Stability in correction of hypertelorbitism and Treacher Collins syndromes.

In the authors' experience, correction of hypertelorbitism either by facial bipartition or by moving only the orbits has proved to be very stable. This is based on the design of the osteotomies and the use of the frontal bar. On the contrary, the rotation of the midfacial segment combined with mandibular lengthening for the correction of Treacher Collins has a strong tendency to relapse because of the backward pull of the soft tissues. Primary stability in these cases depends on the quality of bone grafts, their placement, and skeletal fixation.
