Marginal bone loss, whether it is physiological or pathological, is one of the implant treatment complications. The biomechanical consequences of marginal bone loss could be catastrophic particularly when the abutment screw is at supraosseous level. This study aimed at investigating marginal bone loss influence on the biomechanics of single implant crown using finite element (FE) analysis. Four FE models for a 3.5 × 13 mm implant supported by 4 bone levels (8.5 mm, 10 mm, 11.5 mm, and 13 mm) were subjected to 3 loading conditions: vertical, oblique, and horizontal. The results indicated 5-10 t

The selection of internal fixation as the primary fixation modality for the patient is one of the challenges for the surgeon treating the patient in question. A model of the lateral tibial plateau fracture was established. Three different configurations of internal fixators namely L bone plate, T bone plate, and screw-washer were analyzed. Three stages after surgery were simulated to assess the displacement of bone plates, screws, washers, and the stress shielding ratio in the fracture area.At three stages after surgery, the T bone plate showed better stability for patients during rehabilitati

In our program of bone-anchored hearing aids we experienced two rare complications: a child with a hearing aid fell on her head and got an intrusion of the bone-screw into the intracranial space with a fracture of tabula externa and interna. There were no further intracranial complications, no hematoma or infection. The screw was explanted immediately with oto-microsurgical instruments and diamond burrs. The site of the screw was covered with a local skin flap. There were no complications in wound healing and the child got a second implant on the other side. A patient got an infection of his b

New plating techniques, such as non-contact plates, have been introduced in acknowledgment of the importance of biological factors in internal fixation. Knowledge of the fixation stability provided by these new plates is very limited and clarification is still necessary to determine how the mechanical stability, e.g. fracture motion, and the risk of implant failure can best be controlled. The results of a study based on in vitro experiments with composite bone cylinders and finite element analysis using the Locking Compression Plate (LCP) for diaphyseal fractures are presented and recommendati

INTRODUCTION: Pedicle screw fixation is the standard technique for the stabilization of the spine, a clinically relevant complication of which is screw loosening. This retrospective study investigates whether preoperative CT scanning can offer a predictor of screw loosening.
METHODS: CT-scan attenuation in 365 patients was evaluated to determine the mean bone density of each vertebral body. Screw loosening or dislocation was determined in CT scans postoperatively using the standard radiological criteria.
RESULTS: Forty-five of 365 patients (12.3 %; 24 male, 21 female) suffered postoperative

BACKGROUND: In patients with tibial plateau bone cysts undergoing total knee arthroplasty (TKA), bone defects commonly occur following tibial plateau resection. Current strategies for addressing these defects include bone grafting, bone cement filling, and the cement-screw technique. However, there remains no consensus on the optimal approach to achieve the best surgical outcomes. This study aims to evaluate the most effective repair method for residual bone defects following tibial plateau bone cyst repair during TKA from a biomechanical perspective.
METHODS: The treatment options for tibial

Tibial tubercle osteotomy (TTO) is commonly performed in cases of complicated juxta-articular trauma or revision total knee arthroplasty. However, strategies for firmly fixing the resulting osteotomy bone fragment are not sufficiently understood. This study aims to investigate the effect of the location of the gap between the fragment and the tibia and with various fixed screw configurations on TTO stability, contact force on the fragment, and bone stress by using the finite element method. A TTO model with a 1-mm gap, either above or below the fragment, was developed. Furthermore, five screw

BACKGROUND: The exacerbation of interfacial stiffness differences can lead to localized stress concentration and an increased incidence of pedicle screw loosening. Consequently, the etiology behind the higher occurrence of screw loosening in osteoporotic patients stems not only from a reduction in bone strength but also from a progressive decline in bone elastic modulus. In this context, we hypothesize that constructing pedicle screws using materials with low elastic modulus may help mitigate stiffness discrepancies at the bone-screw interface, potentially serving as an alternative biomechanic

BACKGROUND: Lag screw osteosynthesis for odontoid fractures has a high rate of pseudoarthrosis, especially in elderly patients. Besides biomechanical properties of the different screw types, insufficient fragment compression or unnoticed screw stripping may be the main causing factors for this adverse event. The aim of the study was to compare two screws in clinical use with different design principles in terms of compression force and stability against screw stripping.
METHODS: Twelve human cadaveric C2 vertebral bodies were considered. Bone density was determined. The specimens were matched

Distal femur fractures routinely heal by secondary bone healing, which relies on interfragmentary motion. Periarticular locking plates are commonly used for fixation in distal femur fractures but are associated with a high nonunion rate, likely due to the stiffness of the constructs. Far cortical locking (FCL) screws are designed to allow micromotion at the near cortex while maintaining purchase in only the far cortex. Although clinical data are limited, these screws have been shown in biomechanical studies to provide excellent interfragmentary motion, and animal models have shown increased ca

Spinal instrumentation surgery for older patients with osteoporosis is increasing. Implant loosening may occur due to inappropriate fixation in osteoporotic bone. Developing implants that achieve stable surgical results, even in osteoporotic bone, can reduce re-operation, lower medical costs, and maintain the physical status of older patients. Fibroblast growth factor-2 (FGF-2) promotes bone formation; thus, coating pedicle screws with an FGF-2-calcium phosphate (FGF-CP) composite layer is hypothesized to enhance osteointegration in spinal implants. We designed a long-term implantation pilot s

Background Headless screw fixation is the current gold standard of surgical repair for scaphoid fractures. However, maintaining reduction of certain types of scaphoid fractures is challenging with a compression screw. Plate fixation may offer superior fixation in some scaphoid fractures, particularly those with comminution, nonunion, segmental bony defects, and osteopenic or osteoporotic bone. Purpose This study questions whether method of fixation is a determinant in load to failure in segmental scaphoid fractures, and whether any fixation provides a greater mechanical advantage in simulate
