A comparison of UK and US guidelines for drug treatment in borderline personality disorder.

The UK and USA differ considerably in their guidance regarding the use of drug treatment for borderline personality disorder, but generally agree over the use of psychological treatment. The 2009 UK guidelines from the National Institute for Health and Clinical Excellence (NICE) do not recommend any form of drug treatment except in a crisis with the intention of ceasing such treatment shortly afterwards. The US guidelines from the American Psychiatric Association (APA), published in 2001, are much more positive and suggest that there is a place for selective serotonin reuptake inhibitors (SSRI…

A goal-oriented limited-duration approach for borderline personality disorder during brief inpatient hospitalizations.

As part of their overall care, patients with borderline personality disorder may require inpatient hospitalization for containment and stabilization in the context of severe distress, increased risk for self-harm, or after self-harming acts. Though data suggests that such patients are frequently hospitalized, the benefits of brief hospitalizations can sometimes be questionable. In some cases, the hospitalization environment may increase patient distress. Additionally, frequent serial hospitalizations can disrupt or sidetrack longer-term outpatient treatments. The present paper presents a Goal-…

Platelet [³H] paroxetine binding in female patients with borderline personality disorder and their sisters.

Paroxetine binding could be a vulnerability marker for traits associated with borderline personality disorder (BPD). To study this relationship, we examined [³H] paroxetine binding in female patients with BPD and their sisters. The sample consisted of 54 sibling pairs in which a proband met criteria for BPD. All subjects were given the Diagnostic Interview for Borderlines, revised (DIB-R), the Diagnostic Assessment for Personality Pathology: Brief Questionnaire (DAPP-BQ), the Barratt Impulsivity Scale (BIS), the Affective Lability Scale (ALS), the Hamilton Rating Scale for Anxiety (HAM-A), the…

Processing of visual stimuli in borderline personality disorder: a combined behavioural and magnetoencephalographic study.

BACKGROUND: Behavioral studies on facial emotion recognition yielded heterogeneous results in patients with Borderline Personality Disorder (BPD). Extrastriate cortex hyperactivation has been demonstrated in imaging studies in patients with BPD during face recognition, but electrophysiological studies are lacking. The aim was to investigate temporal processes following face perception in patients with BPD. METHODS: Magnetoencephalography (MEG) was used in eleven non-medicated patients with BPD and nine age-matched healthy subjects. Behavioral responses to visual stimuli and an emotion discrim…

Predictors of dropout from inpatient dialectical behavior therapy among women with borderline personality disorder.

Inpatient dialectical behavior therapy (DBT) is an effective treatment for borderline personality disorder (BPD), but often treatment is ended prematurely and predictors of dropout are poorly understood. We, therefore, studied predictors of dropout among 60 women with BPD during inpatient DBT. Non-completers had higher experiential avoidance and trait anxiety at baseline, but fewer life-time suicide attempts than completers. There was a trend for more anger-hostility and perceived stigma among non-completers. Experiential avoidance and anxiety may be associated with dropout in inpatient DBT. L…

Affective instability: measuring a core feature of borderline personality disorder with ecological momentary assessment.

Ecological momentary assessment (EMA; Stone & Shiffman, 1994) was used to characterize and quantify a dynamic process--affective instability in borderline personality disorder (BPD). Sixty outpatients (34 with BPD and affective instability; 26 with current depressive disorder but not with BPD or affective instability) carried electronic diaries for approximately 1 month and were randomly prompted to rate their mood state up to 6 times a day. Results indicated that BPD patients (a) did not report significantly different mean levels of positive or negative affect; (b) displayed significantly mor…

[Is assessing for borderline personality disorder useful in the referral after a suicide attempt?].

BACKGROUND: Management of suicide attempters accounts for 10% of the psychiatric activity in the emergency room. In this population, the prevalence of borderline personality disorder (BPD) is high (10 - 55%). These patients present poorer psychosocial outcome and more frequent suicide attempts repetitions. However, the utility of the assessment of BPD in the referral to a specific treatment plan has not been yet studied. OBJECTIVE: To examine the association between the assessment of a diagnosis of BPD after a suicide attempt and the referral from the emergency room to a specific treatment pl…

Topiramate treatment for women with borderline personality disorder: a double-blind, placebo-controlled study.

Borderline personality disorder is a common and severe psychiatric illness. The goal of this study was to determine whether topiramate can influence patients' borderline psychopathology, health-related quality of life, and interpersonal problems. Women meeting the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition Structured Clinical Interview II criteria for borderline personality disorder were randomly assigned in a 1:1 ratio to topiramate titrated from 25 to 200 mg/d (n = 28) or placebo (n = 28) for 10 weeks. Primary outcome measures were changes on the Symptom-Checklist,…

Reduced size and abnormal asymmetry of parietal cortex in women with borderline personality disorder.

BACKGROUND: Evidence is accumulating that suggests borderline personality disorder (BPD) and posttraumatic stress disorder (PTSD) are related to small hippocampal size. Psychotic symptoms are frequent in both disorders. Psychotic spectrum disorders are known to be related to abnormalities of temporoparietal cortices. METHODS: Using structural magnetic resonance imaging (3D-MRI), parietal cortex and hippocampal volumes were assessed in 30 young women with BPD who had been exposed to severe childhood sexual and physical abuse and in 25 healthy control subjects. RESULTS: Compared with control s…

Effects of depression and borderline personality traits on psychological state and eating disorder symptomatology.

The incidence of current or lifetime affective disorder and borderline personality characteristics were measured in bulimia nervosa patients. The relationship of these variables to the severity of eating disorder symptomatology (Eating Disorder Inventory [EDI]) and general psychiatric symptoms (Hopkins Symptom Checklist [SCL]) was examined. Categorical diagnostic assessments of affective disorder and borderline personality disorder (BPD) were made by Structured Clinical Interviews for DSM-III-R (SCID-I and -II). Affective disorder diagnosis (both current and lifetime) strongly influenced EDI a…

[Validation of a screening procedure for borderline personality disorder based on the Alternative DSM-5 Model for Personality Disorders].

Objectives The fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) includes an Alternative Model for Personality Disorders (DSM-5), which defines personality disorders based on two dimensional criteria. Criterion A corresponds to the severity of personality dysfunction in the areas of self and interpersonal functioning, while Criterion B comprises five pathological domains including a total of 25 facets. Six specific disorders, including borderline personality disorder (BPD), are defined in the AMPD based on Criteria A and B. However, there is currently very litt…

Levels and associations of borderline personality features and early maladaptive schemas in bipolar disorder: A comparative network analysis of patients with and without severe borderline personality features.

OBJECTIVES: To compare the levels of borderline personality features (BPFs) and early maladaptive schemas (EMSs) and the patterns of associations among BPFs and EMSs between patients with bipolar disorder and severe BPFs (BPF+ group) and those with bipolar disorder and nonsevere BPFs (BPF- group). METHODS: Korean psychiatry department outpatients aged 18-49 years and with bipolar I or II disorder were enrolled. The levels of BPFs and EMSs were compared between the BPF+ and BPF- groups using t-tests. Two undirected networks were estimated for both groups, and differences between the networks w

Antagonistic Narcissism in Women With Borderline Personality Disorder Assessed by Direct and Indirect Measures.

Antagonistic narcissism (AN), involving devaluation, aggression, and impulsivity, is a key feature of narcissism and overlaps with borderline personality disorder (BPD). This study compared explicit (self-reported) and implicit (indirectly measured) AN in women with BPD and a matched clinical control (CC) group and examined associations with aggression, interpersonal problems, and emotion dysregulation. Fifty-one women with BPD and 51 CC participants completed the AN Implicit Association Test (AN-IAT), the Narcissistic Admiration and Rivalry Questionnaire (NARQ), and clinical assessments. Grou…

Use of multisensory information as facilitator of group schema therapy for borderline personality disorder.

BACKGROUND: Group schema therapy (GST) has been found to be an effective form of treatment for borderline personality disorder (BPD) patients. Yet high drop-out rates and heterogenous treatment responses remain challenges in the real-life clinical care of BPD. METHOD: We suggest a novel method for facilitating BPD group therapy using recordings (videos) made by patients at home according to the therapists' instructions and watched together in the group session. Patient examines his/her observations on the video in the group under the guidance of psychotherapist. These video recordings are use…

Borderline personality disorder and other psychiatric, somatic, and behavioral conditions: a nationwide family study.

Borderline personality disorder (BPD) often co-occurs with other health conditions, but the role of genetics vs. environment is unclear. In this multigenerational cohort study using Swedish national registers, the authors quantified the co-aggregation of BPD with other phenotypes in families and estimated the genetic vs. environmental contributions. In a birth cohort of individuals born 1973-2001 and their twins, siblings, cousins, parents, and aunts/uncles, the exposure was BPD in the proband. Odds ratios were estimated for 19 psychiatric, 18 somatic, and seven behavioral/injury outcome pheno…

Impulsivity in borderline personality disorder: A scoping review of affective, neurobiological and methodological findings.

INTRODUCTION: Impulsivity is a defining feature of borderline personality disorder (BPD), yet it remains unclear whether it reflects a stable deficit in inhibitory control or a context-dependent breakdown in behavioural regulation under conditions of emotional arousal. Existing models variably emphasise trait, cognitive or affective mechanisms, with limited integration of contextual influences. This scoping review synthesised empirical evidence examining affective mechanisms, neurobiological and methodological accounts of impulsivity in BPD. METHODS: Following PRISMA-ScR guidelines, CINAHL, E…

Mentalization, Emotional Dysregulation, and Emotion-Related Impulsivity in Young Adults with Borderline Personality Disorder.

Background: Borderline personality disorder (BPD) is characterized by emotional dysregulation and emotion-related impulsivity, particularly during late adolescence and early adulthood. Although mentalizing difficulties have been proposed as an important mechanism underlying borderline psychopathology, their contribution to emotion-related impulsivity beyond alexithymia and anxiety-depressive symptoms remains unclear. This study examined the relationships among mentalizing difficulties, emotional dysregulation, and emotion-related impulsivity in young individuals with BPD. Methods: A total of 1…

A Longitudinal Prediction of Suicide Attempts in Borderline Personality Disorder: A Machine Learning Study.

Borderline personality disorder (BPD) is associated with a high risk of suicide. Despite several risk factors being known, identifying vulnerable patients in clinical practice remains a challenge so far. The current study aimed at predicting suicide attempts among BPD patients during disorder-specific psychotherapeutic interventions exploiting machine learning techniques. The study took into account several potential predictors relevant to BPD psychopathology: emotion dysregulation, temperamental and character factors, attachment style, impulsivity, and aggression. The sample included 69 patie…

Peer-support and group psychoeducation for patients with borderline personality disorder: A feasibility study.

OBJECTIVE: Borderline personality disorder (BPD) is a common disorder associated with poor mental health and socio-professional outcomes. Even though it is known to be a treatable condition, almost half of the patients are still not responding to current psychotherapeutic treatment programs. Peer-support interventions are thought to be an interesting way to improve these treatments, but data regarding the feasibility of such interventions are scarce. METHODS: The present naturalistic study aimed to assess the feasibility and acceptability of incorporating a peer-support worker (PSW) with a li…
