Clinical Profile of Cerebral Venous Sinus Thrombosis.

BACKGROUND AND OBJECTIVES: Cerebral venous sinus thrombosis (CVT) occurs due to occlusion of the veins and sinuses which drain the brain parenchyma. It is an uncommon form of stroke, predominantly found in young patients1 . Despite advances in the recognition of CVT in recent years, diagnosis and management can be difficult because of the diversity of underlying risk factors, presenting features and the absence of a uniform treatment approach. This study aimed to ascertain the clinical presentation, various aetiologies and prognostic indicators of cerebral venous sinus thrombosis. METHODOLOGY

Acute cerebellitis in paediatric patients: Our experience.

INTRODUCTION: Acute cerebellitis is a rare inflammatory disease with a highly variable clinical course that ranges from benign self-limiting symptoms to a fulminant presentation associated with a high risk of death due to compression of the posterior fossa, acute hydrocephalus, and intracranial hypertension. METHODS: We reviewed clinical, laboratory, and radiological findings from children diagnosed with acute cerebellitis between May 2007 and November 2016. We analysed treatments and clinical and radiological progression. RESULTS: Nine children met the diagnostic criteria for cerebellitis.

Multifocal complex glioneuronal tumor in an elderly man: an autopsy study: case report.

OBJECTIVE: The clinicopathological spectra of a dysembryoplastic neuroepithelial tumor (DNT) and a rosette-forming glioneuronal tumor (RGNT) are expanding. We report here the autopsy findings of a case of complex glioneuronal tumor with combined histological features of both a DNT and an RGNT. CLINICAL PRESENTATION: A 79-year-old man presented with a 1-month history of confusion and gait difficulties. A magnetic resonance imaging scan revealed obstructive hydrocephalus attributed to a mass in the posterior third ventricle. INTERVENTION: A third ventriculostomy was performed. Postoperatively,

Distinct Role of Dural and Leptomeningeal Macrophages in Maintaining Cerebrospinal Fluid Drainage to Meningeal Lymphatic Vessels.

Cerebrospinal fluid (CSF) drains along the perivascular space, known as the glymphatic system, to the meninges, where meningeal lymphatic vessels (MLVs) remove toxic products with excess CSF from the brain. Macrophages are widely present in the leptomeninges and dura mater of meninges. However, whether leptomeningeal and dural macrophages play the same or distinct roles in maintaining optimal CSF drainage remains unclear. Intracisterna magna injection of clodronate liposomes indicated a comprehensive depletion of leptomeningeal macrophages, a selective reduction in dural sinus-associated macro

Misleading Rare Case of Idiopathic Hypertrophic Pachymeningitis.

Idiopathic hypertrophic pachymeningitis (IHP) is a rare disease with diffuse thickening of the dura mater that has no specific clinical symptoms and manifestations and it causes neurosurgeons to misdiagnose. A 4-year-old girl presented at the emergency room of our hospital with speech difficulty and severe headache. Head computed tomography scans (CT scan) on admission revealed a large fluid collection over the right temporoparietal region with mass effect, and the neurosurgeon drained it with the initial diagnosis of subdural hematoma. However, the postoperative CT scan demonstrated the failu

Intracranial Hemorrhage Due to Potential Rupture of an Arteriovenous Malformation after BNT162b2 COVID-19 mRNA Vaccination in a Young Korean Woman: Case Report.

Inoculation with the Pfizer-BioNTech coronavirus infection-19 (COVID-19) vaccine (BNT162b2) has been approved in Korea. Although it is generally safe, several possible side effects have been reported. The present report describes a 28-year-old woman who developed an intracerebral hemorrhage in her right temporal lobe after the first dose of the Pfizer-BioNTech COVID-19 vaccine. The patient complained of a persistent headache for four days after the first dose, along with right third nerve palsy and drowsiness. Non-enhanced brain computed tomography confirmed a 5.0 × 3.7 × 5.0 cm3-sized intracr

Tentorial venous anatomy of mice and humans.

We recently described a transtentorial venous system (TTVS), which to our knowledge was previously unknown, connecting venous drainage throughout the brain in humans. Prior to this finding, it was believed that the embryologic tentorial plexus regresses, resulting in a largely avascular tentorium. Our finding contradicted this understanding and necessitated further investigation into the development of the TTVS. Herein, we sought to investigate mice as a model to study the development of this system. First, using vascular casting and ex vivo micro-CT, we demonstrated that this TTVS is conserve

Basal subarachnoid hemorrhage by rupture of arteriovenous malformation at the cerebellopontine angle.

A man in his late forties had lived as a recluse for more than ten years. He was found dead in his room. At autopsy, subarachnoid hemorrhage (SAH) was detected at the base of the brain, which weighed 1333 g. The cerebellar tonsil was swollen. The cerebral ventricle was enlarged and filled with blood. A hematoma was observed in the upper part of the left side of the cerebellar hemisphere. The location and size of SAH in this case indicated that the rupture of a cerebral aneurysm (CA) had occurred; however, CA was not detected. A mass of blood vessels buried in the hematoma was observed at the l

MR features of intracranial hemangioblastomas.

The contribution of MR in 10 patients with surgically proven intracranial hemangioblastomas was retrospectively evaluated and correlated with the computed tomographic findings. Angiography was obtained in eight cases. Multiple lesions were shown in only two patients, but none proved to have von Hippel-Lindau disease. In six patients the tumor typically appeared as a cystic lesion with an intensely enhancing mural nodule at the pial surface of the cyst; three patients presented with a solid lesion with a central cyst and one with a solid lesion only. Thirteen tumors were located in the posterio

[Raghib's syndrome associated with cor triatriatum--a rare surgical case report].

A 27-year-old male with a history of congenital heart disease was admitted for pre-operative evaluation of a cardiac malformation. Echocardiography and cardiac catheterization revealed an incomplete endocardial cushion defect with a persistent left superior vena cava which drained into the left atrium but echo-cardiographic evidence of an abnormal intra-atrial septum was not found. The patient was placed on cardiopulmonary bypass and prepared for the surgical correction of his primary cardiac lesion. Intra-cardiac examination during reconstruction of Raghib's syndrome also revealed the presenc

Magnetic resonance imaging of cerebral arteriovenous malformations.

Magnetic resonance imaging (MRI) was performed in 15 patients with small to very large supratentorial arteriovenous malformations (AVMs). Unlike non-contrast infused computed tomographic (CT) scanning, MRI demonstrated the AVM in all cases. The nidus had a characteristic honeycomb appearance, and in all cases feeding arteries and deep or superficial draining veins were demonstrated without intervening bone artifact. The relationship of the AVM to surrounding cortical areas, deep nuclei, important white matter tracts, and the ventricular system was well appreciated. These characteristics make M

A rare case of multiple brain abscesses caused by Nocardia abscessus co-infection with tuberculous meningitis in an immunocompetent patient.

BACKGROUND: Nocardial brain abscesses are extremely rare and predominantly affect immunocompromised patients, exhibiting a high overall mortality rate. Tuberculosis infections, although they can occur in immunocompetent individuals, are more prevalent in those with compromised immune systems. Tuberculous meningitis (TBM), the most severe manifestation of tuberculosis, is associated with a high fatality rate. Co-infection with both pathogens is unusual. To our knowledge, this is the first reported case of multiple brain abscesses caused by Nocardia abscessus (N. abscessus) in a young immunocomp

Comparison of Accuracy Between Image-Guided and Freehand External Ventricular Drain Bedside Placement in Association With Time Efficacy.

BACKGROUND AND OBJECTIVES: A common neurosurgical procedure is the placement of an external ventricular drain (EVD), often as a time-sensitive treatment for hydrocephalus and/or increased intracranial pressure. The etiology ranges from traumatic brain injury to spontaneous intracranial hemorrhages, frequently causing change to ventricular anatomy, making them difficult targets. The goal of this study was to determine whether the use of image-guidance for bedside EVD placement improves accuracy and decreases number of passes with the same time-effectiveness. METHODS: Adult patients with bedsid

Cranioplasty of Hemispherical Defects Using High Impact Methylmethacrylic Plate.

UNLABELLED: Decompressive craniectomy is the most common neurosurgical procedure performed in today's scenario, usually posttrauma or a cerebrovascular event. Cranioplasty is the repair of a cranial defect or deformation. In the last decade, there has been an increase in the number of cranioplasties performed because of an increase in the number of decompressive craniectomies. Although the main purpose of cranioplasty is to protect the brain and restore aesthetics, it has been proved beyond doubt that there is also an improvement in function and patient self-esteem.Reconstructing the skull aft

Cognitive bias and diagnostic overshadowing in trauma: The case of substance use disorder in patients with traumatic brain injury.

BACKGROUND: Diagnostic overshadowing is a type of cognitive bias where a pre-existing condition overshadows the evaluation of other diagnoses. We aimed to explore diagnostic overshadowing in trauma, using as a proxy the effects of substance and alcohol use disorder on time to surgical intervention in patients with traumatic brain injury. METHODS: The 2017-2020 American College of Surgeons-Trauma Quality Improvement Program database was used to identify patients of at least 18 years of age with isolated traumatic brain injury who underwent craniotomy, intraventricular drain, or intracranial pr

Sociodemographic and clinical predictors of quality-of-life outcome in children and young people with primary brain tumour in Karachi, Pakistan: a prospective cohort study.

BACKGROUND: Children and young people (CYP) with primary brain tumour (PBT) are at high risk for developing late effects, potentially affecting long-term quality of life (QoL). In low-income and middle-income countries, QoL has not been studied in depth in CYP. In the present study, CYP treated for PBTs in Pakistan were evaluated regarding (A) mean change in QoL scores pretreatment and 12 months post-treatment and (B) predictors of change in QoL scores 12 months post-treatment. METHODS: A prospective cohort study was conducted from November 2020 to July 2023. CYP aged 5-21 years, with newly d

Indications for pediatric external ventricular drain placement and risk factors for conversion to a ventriculoperitoneal shunt.

BACKGROUND/AIMS: The external ventricular drain (EVD) is commonly used for the treatment of hydrocephalus and intracranial pressure (ICP) monitoring. In this study, we retrospectively reviewed indications for EVD placement over the last 20 years in pediatric patients at our institution. Additionally, we evaluated the rate that an EVD needed to be converted to a ventriculoperitoneal shunt (VPS) and ascertained how hydrocephalus etiology impacted long-term drainage requirements. METHODS: Patients that received an EVD between 1990 and 2010 at our institution were identified using billing codes.
