Large arteries contribute to the regulation of cerebral blood flow. The goal of this study was to examine the effects of changes in diameter of the basilar artery on blood flow to the brain stem. We measured blood flow with laser-Doppler flowmetry in anesthetized rats. MRI has become indispensable to the positive diagnosis of brain stem tumours which develop preferentially in young people. The prognosis of these tumours, especially infiltrating gliomas, being relatively sombre, it is important to make a precise morphological diagnosis of the lesions (increase in size, signal abnormalities) which will help both neurosurgeons and radiotherapists. As regards post-therapeutic monitoring, MRI with gadolinium contrast injection is harmless and will make it possible to follow these patients regularly and to detect recurrences.

The authors have studied by CT brain infarcts of different ages in 16 patients, two of whom were also examined by MRI. The early and CT signs of wallerian degeneration of the descending tracts within the brain stem are described. Three cases of crossed cerebellar atrophy are reported; in one of them, the cerebral lesion had been present for less than 8 years. The physiopathology of pontine hemiatrophy and crossed cerebellar atrophy is discussed.

This study was carried out to investigate the effects of an expanding supratentorial mass on auditory brain-stem responses (ABRs). A balloon was inserted into the supratentorial epidural space of seven baboons (in two cases, in the right occipital area; in five cases, in the right temporal area). Twenty-five hyperlipidemic, neurologically and audiologically asymptomatic patients were compared to 20 normolipemic control subjects regarding different interpeak latency differences (IPLD) of auditory brain stem evoked potentials (ABEP). A case of acute polyradiculoneuritis accompanied by rare complications of the VII, VIII and IX cranial nerve disorder was described. There was an elevation of the rubella virus antibody titer, which suggested the case had been infected before. Thirty-eight year-old woman complained of bilateral tinnitus and was proven audiometrically to have sensori-neural hearing loss. Electroacoustical examination disclosed that all the waves were suppressed and that wave I and V were barely detected, while wave II, III and IV were absent. The auditory nerve and bran stem responses recorded at the vertex and mastoids were closely examined in normal subjects using a larynx electrode as a neutral reference point. With monaural stimulation, significant activity was found at the vertex and both mastoids, and the wave-forms at the ipsi- and contralateral mastoids were consistently different. With binaural stimulation the small amplitude mastoid response were largely in phase with the vertex response. The implications of the findings are discussed.

Twenty cases which showed partial loss of ABR waves were reported. Only wave I was recorded in 18 ears tested. Wave I and II; I-III; and I-IV responses were observed in four, eight, and five ears tested, respectively. Cases included pontine glioma (8), acoustic tumor (4), West syndrome (2), leukodystrophy (2), facial neurinoma (1), epidermoid tumor (1), pontine bleeding (1), and unknown cause (1). Fourteen of 20 were tumor cases. This fact indicates that if the ABR shows partial loss of waves, the examiner should first suspect the space-occupying lesions of the brain stem.

The auditory brainstem electric response yields information on both the neurological and the audiological status of infants and adults. These early waves have particular properties as: non-habituation, absence of latency modifications between sleep, awake state, or under high doses of barbiturates. We have developed a procedure for extracting each type of information by measuring the latencies of wave I (auditory nerve) and wave V (inferior colliculus), then the speed of conduction between these two waves.
