The atrioventricular conduction axis of hearts with isolated ventricular septal defects.

We studied the surgical anatomy of 14 hearts with isolated ventricular septal defects and the precise relationship of the atrioventricular conduction axis to their rims. Ten of these hearts were investigated by serial sectioning of their atrioventricular conduction systems. All defects were divided into perimembranous or muscular types as previously suggested by Soto et al. The distinguishing feature of the perimembranous defect was that the central fibrous body formed part of its rim, this fact indicated that the conduction tissue was always to the right of the surgeon's hand when the defect

[Hyperthyroidism and atrioventricular heart block (author's transl)].

Two cases of thyrotoxicosis including a first-degree heart block are reported; a study of action potentials of the bundle of His was achieved in the second case. The association atrioventricular block-hyperthyroidism is unusual but does not seem to be a pure coincidence. The direct effect of thyroid hormon on the myocardium is supposed to be responsible for the conduction disturbance; its disappearance with the cure of the thyrotoxicosis is a favourable argument for a relation of causality. The therapeutic consequence of this association is to exclude completely the use of betablockers.

Strength-duration curves for left bundle branch area pacing.

BACKGROUND: Despite growing clinical use of left bundle branch pacing (LBBP), data regarding the fundamentals of this pacing modality, including chronaxie and rheobase, are scarce. OBJECTIVE: The purpose of this study was to calculate strength-duration curves with chronaxie and rheobase values for LBBP and left ventricular septal pacing (LVSP), and to analyze battery current drain and presence of selective LBBP at very short pulse duration (PD). METHODS: A group of 141 patients with permanent LBBP were studied. LBBP and LVSP capture thresholds were assessed at 6 different PDs to calculate th

Terminal end of retro-aortic root branch: An unrecognized origin for "proximal left anterior fascicle" premature ventricular complexes with narrow QRS duration.

BACKGROUND: Premature ventricular complexes (PVCs) with narrow QRS duration, inferior frontal plane QRS axis, and right bundle branch block (RBBB) pattern generally originate from the proximal segment of the left anterior fascicle (LAF). OBJECTIVE: The purpose of this study was to investigate the exact origin of this category of PVCs. METHODS: Twenty-two patients with assumed proximal LAF-PVCs were enrolled in the study. Detailed mapping of fascicular potentials (FPs) was performed during sinus rhythm (SR) and PVCs. RESULTS: During SR, a cluster of FPs could be found at the most superior po

Reversible T-wave inversions during left bundle branch area pacing.

BACKGROUND: Our clinical observation found that T-wave inversions (TWIs) appeared during left bundle branch area pacing (LBBAP); however, the incidence and influencing factors were unclear. The study aimed to investigate the effects of LBBAP on T-wave and explore possible factors associated with TWIs. METHODS: This was a retrospective cohort study. An electrocardiogram (ECG) was acquired at baseline and after LBBAP. Baseline characteristics, ECG parameters, LBBAP parameters, and troponin T (TnT) levels were compared between the non-TWIs and TWIs groups. Multivariable logistic analyses were pe

Clinical presentation of ventricular-Hisian and ventricular-nodal accessory pathways.

BACKGROUND: Narrow QRS tachycardia or premature beat with bystander atrial activation or ventricular-atrial dissociation is an unusual arrhythmia that can present diagnostic and therapeutic challenges. When ventricular-atrial conduction is robust, these arrhythmias can be difficult to distinguish from atrioventricular nodal reentry. OBJECTIVE: The purpose of this study was to describe the electrophysiology characteristics, diagnostic maneuvers, and treatment of these types of arrhythmias in a series of 7 cases. METHODS: Standard multipolar recording and pacing electrodes were used during ele

Is It Complete Left Bundle Branch Block? Just Ablate the Right Bundle.

Complete left bundle branch block (LBBB) is established according to standard electrocardiographic criteria. However, functional LBBB may be rate-dependent or can perpetuate during tachycardia due to repetitive concealed retrograde penetration of impulses through the contralateral bundle "linking phenomenon." In this brief article, we present two patients with basal complete LBBB in whom ablating the right bundle unmasked the actual antegrade conduction capabilities of the left bundle. These cases highlight intriguing overlap between electrophysiological concepts of complete block, linking, ex

Tachycardia-dependent second degree AV-block in a patient with right bundle branch block.

A 55-year old male patient, with dizzy spells during everyday activity and a complete right bundle branch block as the sole electrocardiographic abnormality, reproducibly demonstrated tachycardia-dependent Mobitz Type II- and 2:1 second degree atrioventricular block. An electrophysiologic study revealed a provocable block within the distal portion of the bundle of His without evidence of a split His potential. Because of the truly tachycardia-dependent AV-block, beta-blocker medication was initiated to prevent high sinus rates during everyday activity. This therapy abolished symptoms totally.

Mobitz II atrioventricular block due to infra-Hisian block and bundle branch extrasystoles.

An asymptomatic elderly male presented with complete right bundle branch block, left anterior fascicular block and Mobitz Type II second degree atrioventricular block. In addition, he was noted to have ventricular premature systoles. Electrophysiologic studies demonstrated two apparently different mechanisms for the second degree A-V block--infra-Hisian block and A-V block due to bundle branch extrasystoles arising in the affected right bundle branch. Concealed conduction of the bundle branch extrasystoles as the proximate cause of infra-Hisian block could not be excluded. However, both forms

Diagnosis, management, and long-term results of patients with congenital complete atrioventricular block.

Sixty-five patients with congenital complete atrioventricular block have been studied. The median age at time of diagnosis was 7 months with 26 infants being diagnosed prior to 1 month of age. Anatomic heart disease was present in 25/65 (39%) infants, with ventricular inversion/L-transposition of great arteries complex occurring in 20/25 infants (80%). The site of block was above the bundle of His in 11 of 18 (61%) patients studied, in the bundle of His in three of 18 (17%), and below the bundle of His in two of 18 (11%). Permanent pacemakers were implanted in 17 (26%). Ten patients (15%) have…

Clinical outcomes of His-Purkinje conduction system pacing.

His-Purkinje conduction system pacing (HPCSP) in the form of His bundle pacing (HBP) and left bundle branch pacing (LBBP) allows normal left ventricular activation, thereby preventing the adverse consequences of right ventricular pacing. HBP has been established for several years with centers from China, Europe, and North America reporting their experience. There is international guidance as to how to implant such systems with the differing patterns of His bundle capture clearly described. LBBP is a more recent innovation with potential advantages including improved pacing parameters. HPCSP ha
