Groin injuries comprise 2 to 5 percent of all sports injuries. Early diagnosis and proper treatment are important to prevent these injuries from becoming chronic and potentially career-limiting. Adductor strains and osteitis pubis are the most common musculoskeletal causes of groin pain in athletes. These two conditions are often difficult to distinguish. Other etiologies of groin pain include sports hernia, groin disruption, iliopsoas bursitis, stress fractures, avulsion fractures, nerve compression and snapping hip syndrome.

MR imaging provides clinically useful information in assessing the elbow joint for osteochondral and articular lesions. Post-traumatic osseous abnormalities well seen by MR imaging include radiographically occult fractures, stress fractures, bone contusions, osteochondritis dissecans, and chondral defects. Intraarticular loose bodies can be identified with MR imaging, especially if fluid or contrast material are present within the elbow joint. MR imaging can also provide additional information regarding synovial osteochondritis, osteoarthritis, and bursitis about the elbow.

The diagnosis of septic infections of closed body cavities requires a careful search. Traditional laboratory tests such as Gram's stain, white cell count, and protein and glucose levels are often inconclusive. Measurement of lactic acid in cerebrospinal, synovial, pleural, ascitic, and bursal fluids has been utilized to distinguish bacterial from nonbacterial infections. The present review summarizes the current status of lactic acid measurement in the differential diagnosis of meningitis, arthritis, empyema, bacterial peritonitis, and bursitis.

The aim of this review is to evaluate the current available literature evidencing on peri-articular hip endoscopy (the third compartment). A comprehensive approach has been set on reports dealing with endoscopic surgery for recalcitrant trochanteric bursitis, snapping hip (or coxa-saltans; external and internal), gluteus medius and minimus tears and endoscopy (or arthroscopy) after total hip arthroplasty. This information can be used to trigger further research, innovation and education in extra-articular hip endoscopy.

OBJECTIVE: To identify sonographically the changes of the forefoot in patients with metatarsalgia.
METHODS: Sonography of the foot was performed in 112 patients with metatarsalgia and in 50 healthy controls. Metatarsophalangeal (MTP) joints, intermetatarsal web spaces, flexor and extensor tendons, and plantar aponeurosis were examined.
RESULTS: Sonography showed intermetatarsophalangeal bursitis in 20.5% of cases, Morton's neuroma in 15.2%, and effusion of MTP joints in 11.7%.
CONCLUSION: Sonography gives useful information about the possible alteration responsible for metatarsalgia.

Ultrasonography is a useful technique to detect soft tissue changes of rheumatoid arthritisnot only synovitis, but also tenosynovitis, bursitis, and enthesitis-even at a subclinical stage. However, radiologists tend to focus on synovitis in daily practice, and unusual peri- or extraarticular manifestations of rheumatoid arthritis are difficult to detect at the initial presentation. This pictorial essay describes a broad spectrum of ultrasonographic findings in tendons, bursae, ligaments, subcutaneous tissues, bones, and nerves to assist in the accurate diagnosis of rheumatoid arthritis.

We present an overview of imaging and intervention of the extensor mechanism of the knee. Particular focus is placed on the evaluation of patellofemoral tracking disorders, patellar and quadriceps tendinosis and tears, patellar fracture, lateral patellar condyle patellar friction syndrome, and prepatellar bursitis. Anatomical and biomechanical factors contributing to these disorders are considered. Imaging evaluation is presented in a clinical context, and therapeutic options for these disorders are discussed. Image-guided therapy options for symptomatic patellar tendinosis are also described …

UNLABELLED: Pain in the posterosuperior portion of the calcaneus can be caused by a retrocalcaneal bursitis, enlargement of the superior bursal prominence of the calcaneus, insertional Achilles tendinopathy, stress fracture of the calcaneus, or inflammation of an adventitious bursa between the Achilles tendon and the skin. Hypertrophied tendon impinging on the posterosuperior calcaneal tuberosity can be a cause of retrocalcaneal pain after repair of acute rupture of the Achilles tendon. This can be effectively treated by endoscopic calcaneoplasty.

The early diagnosis of osteomyelitis of the patella in children can be difficult due to the rarity of this infection and to the vagaries of presentation which may be either acute with systemic signs or insidious with mild local signs. We present four cases of patellar osteomyelitis to illustrate the spectrum of this disorder. There should be a high index of suspicion with persistent pain and swelling above the knee, septic prepatellar bursitis, and septic knees that do not respond to conventional management. Management parallels treatment of osteomyelitis of other bones.

The article considers a question of the differential diagnosis and treatment of back pain. The authors recommend the use of generally accepted term «spinal osteoarthritis» to describe changes in the spine, which in the Russian medical literature referred to as «osteochondrosis». The article describes the diagnosis and treatment of myogenic pain, pain due to facet syndrome, herniated disc, cervical and lumbar radiculopathy, spinal stenosis, interspinous bursitis. The authors recommend the use of dexketoprofen (flamadex) and adjuvants (baclofen, flupirtine) for the nociceptive back pain treatmen…

Olecranon bursitis is a common clinical problem. It is often managed conservatively because of the high rates of wound complications with the conventional open surgical technique. Conventional olecranon bursoscopy utilizes an arthroscope and an arthroscopic shaver, removing the bursa from inside-out. We describe an extrabursal endoscopic technique where the bursa is not entered but excised in its entirety under endoscopic vision. A satisfactory view is obtained with less morbidity than the open method, while still avoiding a wound over the sensitive point of the olecranon.

There has been recent interest in tuberculosis of the hand because of a rising incidence owing to increasing numbers of immigration, an aging population, and immunosuppressed people including affected patients with human immunodeficiency virus. In this article, we review the epidemiology, bacteriology, pathophysiology, diagnosis, and principles of treatment of tuberculosis of the hand. The second part of the report emphasizes the classification of hand tuberculosis (cutaneous lesions, tenosynovitis, bursitis, osteomyelitis, arthritis, and tuberculous hypersensitivity reactions) along with the …

Adult-onset Still's disease (AOSD) is an inflammatory disorder characterized by high-spiking fever, arthritis, and an evanescent rash. The pattern of chronic arthritis is a universal feature of AOSD. However, chronic bursitis with the formation of numerous rice bodies is a very rare presentation in a patient with AOSD. To our knowledge, no case of formation of numerous rice bodies in AOSD has been reported thus far. We describe the case of a 28-year-old man with AOSD with rice bodies in his left shoulder joint.

Haglund's syndrome is a cause of retrocalcaneal pain. The clinical diagnosis of Haglund's syndrome is often confusing as the clinical picture may mimic other causes of hindfoot pain such as isolated retrocalcaneal bursitis or hindfoot involvement from more systemic arthropathies such as Reiter's syndrome or rheumatoid arthritis. With the increasing frequency of employing sonography as a diagnostic tool in the evaluation of foot and ankle pathology, recognition of the sonographic appearance of Haglund's complex is important. We report a case of Haglund's syndrome diagnosed and treated with sono…

This review discusses the many etiologies of hip pain and how to clinically evaluate. Both primary hip disorders and conditions that refer pain to the hip are included in the differential diagnosis. The key components to a history are reviewed with clinically relevant examples. A comprehensive physical exam is described that includes disease and injury-specific tests. Imaging studies and diagnostic tests available are outlined. Conditions of bursitis, snapping hip syndromes, myofascial pain, fibromyalgia, arthropathies, fractures, neurogenic pain, metabolic diseases, and neoplastic disorders w…

Inequality of leg lengths is associated with low back pain, greater trochanteric bursitis, and degenerative hip disease. The pelvic obliquity that results from a discrepancy of leg lengths may contribute to degenerative changes in the lumbar spine. The diagnosis of leg length inequality may be overlooked. If this occurs, extensive and costly investigations are then often ordered to evaluate these secondary manifestations of leg length inequality. This is unfortunate because a simple shoe lift is all that was needed to provide symptomatic relief for selected patients.

Classic signs of joint inflammation at physical examination of patients with rheumatoid arthritis (RA) are joint swelling and joint tenderne … and osteitis, and more recently, interosseous tendon inflammation in the hand, and intermetatarsal bursitis in the forefoot. The question now arises as to whether the signs and symptoms observed during clinical assessment of RA are purely driven by synovitis, or if, and to what extent, they can be explained by other inflamed tissues. Therefore, we have su …

The prevalence of asymptomatic shoulder pathology has been shown to be high on both ultrasound and magnetic resonance imaging (MRI). The most common shoulder pathologies identified in asymptomatic, non-athlete individuals include rotator cuff pathology, acromioclavicular (AC) joint pathology, labral tears, subacromial bursitis, and calcific tendinitis. The data in the current literature suggest that asymptomatic rotator cuff tears are diagnosed on ultrasound and MRI at high rates, suggesting that rotator cuff tears may be considered an age-related, normal, degenerative change. However, there a…
