Dissociative Multiplicity and Psychoanalysis.

This paper examines the relationship between psychoanalysis and trauma, focusing specifically on dissociation-and, more narrowly, on dissociative multiplicity, the most challenging form for psychoanalysis to address. Dissociative multiplicity involves the presence of more than one center of consciousness, more than one "I," a feature of multiple personality, now called Dissociative Identity Disorder or DID, and OSDD-1, or subthreshold DID. The central question is whether classic psychoanalytic constructs, such as splitting, repression, various triadic views (oedipal, topographic, structural, F

Advancing minimally invasive surgery for elderly colorectal cancer patients: Bridging evidence to practice.

The recent study by Min et al provides evidence supporting laparoscopic radical resection of colorectal cancer (LRRCC) as a superior surgical approach for elderly patients. Their retrospective analysis of 104 patients demonstrated that LRRCC offers higher therapeutic efficacy, reduced postoperative complications, faster recovery, and attenuated systemic inflammatory responses compared to open surgery. While the study underscores the clinical advantages of minimally invasive techniques, it also highlights critical gaps, such as single-center design and limited long-term follow-up. This editoria

[Late diagnosis of spinal muscular atrophy in a patient with Down syndrome].

A case of a 6-year-old girl with Down's syndrome is presented. She was referred to our center due to a history of generalized muscle weakness of chronic evolution, associated to her inability to walk. Her mother claimed that the girl's muscle weakness always called her attention as well as the difficulties to the development of motor skills shown by her daughter compared to other children, whether they were healthy or with Down's syndrome. There was information in her medical record and physical exam that strongly suggested the possibility of suffering a neuromuscular disorder. We asked for a

The Prevalence of Antimicrobial Resistant Neisseria gonorrhoeae in Papua New Guinea: A Systematic Review and Meta-Analysis.

Neisseria gonorrhoeae antimicrobial resistance (NG AMR) has become an urgent concern globally. The World Health Organization, the United States of America Centers for Disease Control, and other regulators have called to improve resistance-testing methods to enhance NG AMR surveillance. NG AMR surveillance remains critical in informing treatment; unfortunately, this is often lacking in settings with limited resources, such as Papua New Guinea (PNG). We conducted a systematic review and a prevalence meta-analysis, and provided an overview of NG AMR in PNG. We showed the lack of NG AMR data in th

Death: The Simple Clinical Trial Endpoint.

Death, as a biological phenomenon, is well understood and a commonly employed endpoint for clinical trials. However, death identification and adjudication may be difficult for pragmatic clinical trials (PCT) that rely upon electronic health record and patient reported data. We propose a novel death identification and verification approach that is being used in the ToRsemide compArisoN with furoSemide FOR Management of Heart Failure (TRANSFORM-HF) PCT. We describe our hybrid approach that includes gathering information from clinical trial sites, a centralized call center, and National Death Ind

Bortezomib in combination with dose-adjusted EPOCH (etoposide, prednisone, vincristine, cyclophosphamide, and doxorubicin) induces long-term survival in patients with plasmablastic lymphoma: a retrospective analysis.

Plasmablastic lymphoma (PBL) is a rare and aggressive form of B-cell non-Hodgkin lymphoma. This subtype of lymphoma has a post-germinal center cell-of-origin called the plasmablast, and the immunophenotype is more consistent with that of a plasma cell than a lymphocyte. Because of these unique features, PBL is notoriously difficult to treat. Case reports and small reviews have evaluated the addition of agents directed against plasma cell disorders in combination with traditional lymphoma-directed regimens. We describe the largest case series to date, with the longest follow-up, evaluating bort

A Call for Research on Individuals Who Opt Out of Living Kidney Donation: Challenges and Opportunities.

In light of the transplant community's efforts to increase living donation, preserving donor autonomy is essential. Understanding how often and the reasons why potential living kidney donors opt out of the donor evaluation process for nonmedical reasons is important for assessing donor voluntariness. The experiences of individuals who opt out of donation may reveal avenues for enhancing donor protections and identify modifiable deterrents to donation. A limited number of centers have reported rates of opting out between 0.2% and 22%; the reasons for declining to donate are not well studied. Th

Abdominal wall defects: prenatal diagnosis, newborn management, and long-term outcomes.

Omphalocele and gastroschisis represent the most frequent congenital abdominal wall defects a pediatric surgeon is called to treat. There has been an increased reported incidence in the past 10 years mainly due to the diffuse use of prenatal ultrasound. The early detection of these malformations, and related associated anomalies, allows a multidisciplinary counseling and planning of delivery in a center equipped with high-risk pregnancy assistance, pediatric surgery, and neonatology. At present times, closure of defects, even in multiple stages, is always possible as well as management of most

Reducing central line-associated bloodstream infections in the blood and marrow transplantation population: a review of the literature.

Healthcare-associated infections are a significant cause of patient morbidity and mortality. Most healthcare-associated bloodstream infections are related to the presence of central venous catheters and are called central line-associated bloodstream infections (CLABSIs). A review of the literature showed a decrease in CLABSIs in intensive care units (ICUs) since the implementation of guidelines from the Centers for Disease Control and Prevention. This review demonstrated the effectiveness of daily chlorhexidine gluconate (CHG) bathing toward reducing CLABSIs as well as infections from methicil

Intrahepatic cholestasis following abuse of powdered kratom (Mitragyna speciosa).

INTRODUCTION: Kratom (Mitragyna speciosa) is a common medical plant in Thailand and is known to contain mitragynine as the main alkaloid. According to an increase in published reports and calls at German poison control centers, it has been used more frequently as a drug of abuse in the western hemisphere during the last couple of years. Despite this increase, reports of severe toxicity are rare within the literature. CASE REPORT: We describe a case of a young man who presented with jaundice and pruritus after intake of kratom for 2 weeks in the absence of any other causative agent. Alkaloids

Rapid serial processing in patients with multiple sclerosis: the role of peripheral deficits.

This study compared speed of information processing in patients with relapsing-remitting or secondary progressive multiple sclerosis (MS) and healthy controls using the Stroop Test and a Picture Naming Test (PNT). While both tests evaluated processing speed within a format calling for rapid serial processing of stimulus information, the PNT included trials designed to impose greater verbal-motor and ocular-motor challenges by using novel rather than repeated pictures and by presenting the pictures in distributed locations rather than always centered on the screen. The results confirmed that a

Inaccuracy of the migration percentage and center-edge angle in predicting femoral head displacement in cerebral palsy.

Six patients (seven hips) with total-body-involvement cerebral palsy were assessed for a hip reconstruction. Four patients (five hips) underwent three-dimensional computerized tomographic (3D CT) scans, and in two patients, faux profile views were obtained. The femoral head was subluxed or dislocated anteriorly in all seven hips. The migration percentage (MP) and center-edge angle (CEA) underestimated the pathology. We conclude that the MP and CEA do not exclude an anterior displacement of the femoral head and calls into question the accuracy of methods relying on two-dimensional imaging in as

[Identification of an unusual stabbing weapon based on the shape of injuries of the kidney and humerus].

The body of a 38-year-old man was found buried. The first step of the discovery was an anonymous telephone call. The body had 5 stab wounds (3 in the back, 1 in the left upper arm and 1 in the right leg). The cause of death was considered to be loss of blood due to a stab wound in the lung. The wounds in the kidney and the left humerus had peculiar shapes. The wound on the kidney was 1.1 cm long and had notches in the center of both margins of the wound. A V-shaped nick was noted on the humerus. Based on the above findings it was assumed that the murder weapon was a tapering 4-edged instrument

Cartilage healing: A review with emphasis on the equine model.

Articular cartilage is a remarkably resilient tissue capable of withstanding considerable stress and repeated loading. Since this tissue has no blood vessels, nerve elements, or lymphatics, it is not surprising that it has a limited capacity for repair when damaged. In the horse, cartilage damage occurs as an occupational hazard. Furthermore, developmental defects such as osteochondrosis can lead to osteochondritis dissecans. Resultant cartilage flaps, fissures, and poorly organized subchondral bone produce disruption of joint surfaces.Veterinarians are often called upon to intervene when dama

[Effect of neonatal androgenization on the septal neurons of the brain in female rats in early postnatal ontogeny].

Newborn female rats were androgenized, and the reaction of neurons of brain septum on excessive quantity of exogenous androgens, introduced during so-called "crucial" period of formation of centers of gonadotropic regulation of sexual cycles, has been studied in 3, 5, 7, 10, 20, 30, and 60 days old animals. Morphometry of brain septum cell nuclei revealed that most neuron nuclei shrink after androgenization. Monoamine content was significantly increased in septum nuclei of experimental animals. Neonatal androgenization led to the increased capacity of septal complex neurons to bind 3H-estradio

Air transport of the neonate with a congenital diaphragmatic hernia.

A congenital diaphragmatic hernia is the prenatal herniation of abdominal contents into the chest cavity; the infant usually has a hypoplastic lung on the side of the hernia. The neonate presents with cyanosis, tachypnea, retractions, and a scaphoid abdomen. A simple chest X-ray will confirm the diagnosis by revealing hollow viscera in the chest cavity. Survival is dependent upon rapid transport to a major surgical center equipped to perform the necessary surgery and provide the post-operative care. The newborn should be kept warm, well oxygenated, and normocapneic to avoid acidosis. Acidosis

Influenza-Associated Acute Necrotizing Encephalopathy in US Children.

IMPORTANCE: Acute necrotizing encephalopathy (ANE) is a rare, but severe, neurologic condition for which epidemiologic and management data remain limited. During the 2024-2025 US influenza season, clinicians at large pediatric centers anecdotally reported an increased number of children with influenza-associated ANE, prompting this national investigation. OBJECTIVE: To understand the clinical presentation, interventions, and outcomes among US children diagnosed with influenza-associated ANE. DESIGN, SETTING, AND PARTICIPANTS: This study was a multicenter case series of children diagnosed wit

Automated Calls Added to SMS Reminders Reduce Missed Appointments among High-Risk Patients.

Missed outpatient appointments (or no-shows) pose a significant challenge to health systems by reducing access to care, wasting resources, and negatively impacting patient outcomes. Traditional interventions - such as overbooking, transportation services, phone calls, and financial incentives - have yielded mixed results and face limitations in terms of scalability. Predictive modeling offers a promising tool for identifying patients at high risk of missing their appointment, allowing for targeted outreach. However, targeted outreach typically needs to be paired with manual outreach efforts to

Decadal Trends in Emergency Department Head and Neck CTA Utilization: A Comprehensive Analysis of Volume, Trauma, and Clinical Severity.

BACKGROUND AND PURPOSE: Neuroradiology services report rising ED CTA head and neck demand. Although late-window thrombectomy trials increased interest in perfusion-based selection, relative growth of CTA head and neck versus CT perfusion (CTP), and how these trends compare with overall neuro-ED imaging volume, ED throughput, and coverage periods, remains incompletely characterized. We evaluated decade-long neuro-ED imaging trends at a tertiary care academic medical center. MATERIALS AND METHODS: Retrospective observational study including all neuroradiology ED examinations from two hospital E
