Extent of pleural effusion on chest radiograph is associated with failure of high-flow nasal cannula oxygen therapy.

PURPOSE: The purpose of the study was to determine whether pleural effusion (PE) is associated with a failure of high-flow nasal cannula (HFNC) therapy. MATERIALS AND METHODS: We conducted a single-center retrospective study. Seventy-three patients with acute respiratory failure given HFNC therapy between January 2012 and December 2014 were reviewed. HFNC failure was defined as intubation or noninvasive positive pressure ventilation following HFNC therapy. The numbers of quadrants with consolidation or ground glass opacity were counted on chest radiographs performed within 24 hours before sta…

High-Flow Nasal Cannula and Aerosolized β Agonists for Rescue Therapy in Children With Bronchiolitis: A Case Series.

Asthma and bronchiolitis are episodic obstructive pulmonary diseases characterized by bronchoconstriction, airway wall inflammation, increased mucus production, and air-flow obstruction. We present the cases of 5 infants treated for acute bronchiolitis with respiratory distress using a combination of high-flow nasal cannula oxygen (HFNC) and an Aerogen nebulizer to deliver aerosolized β-agonist therapy. In all infants, we found that HFNC resulted in a greater heart rate increase than delivery via a facemask. We also found that patients tolerated inhaled therapy better with HFNC than a facemask…

Use of a cannula insert in erratic emptying Heidenhain pouch dogs: effect of single and cumulative doses of omeprazole.

A novel cannula insert was used to correct erratic emptying in Heidenhain pouch dogs. The insert restored the recovery of histamine or pentagastrin stimulated gastric acid secretion to that obtained from normal emptying pouch dogs, probably by displacing obstructing tissue within the pouch. Use of the insert did not cause any discomfort to the animals, nor did it alter basal secretion. There was no significant difference between the antisecretory activity of single doses of omeprazole in erratic emptying animals fitted with the insert (ED50 = 0.21 mumol/kg) and in dogs with a normal pouch (ED(…

Cardiopulmonary effects of butorphanol tartrate intravenously administered for placement of duodenal cannulae in isoflurane-anesthetized yearling steers.

Cardiopulmonary effects of IV administered butorphanol tartrate (BUT) were assessed in 7 yearling steers medicated with atropine and anesthetized with guaifenesin, thiamylal sodium, and isoflurane in O2 for surgical placement of duodenal cannulae. Heart rate, respiratory rate, arterial blood pressures, pHa, PaCO2, PaO2, arterial [HCO3-], esophageal temperature, and end-tidal isoflurane concentrations were measured before and after IV administration of BUT (10 mg). Mean respiratory rate increased significantly (P less than 0.05) only at 45 and 60 minutes after BUT administration. Mean respirato…

High-flow nasal cannula in a patient with prolonged air leak following cardiothoracic surgery: A case-based observation.

Prolonged air leak (PAL) following cardiothoracic surgery severely limits available ventilatory strategies. We report a 68-year-old male who developed bilateral PAL after coronary artery bypass grafting and mitral valve replacement. Despite maximal ventilatory support, critical hypoxaemia persisted. Following early extubation at hour 72, high-flow nasal cannula (HFNC) was initiated, achieving rapid oxygenation recovery (ΔPaO2 +22 mmHg, ΔPaCO2 -15 mmHg within 24 hours). Combined with awake physiotherapy and intermittent noninvasive ventilation, HFNC facilitated clinical recovery without re-intu…

From Floor to Intensive Care Unit: Unpacking the Real Impact of High-Flow Nasal Cannula on Bronchiolitis Triage.

BACKGROUND: High-flow nasal cannula (HFNC) therapy is increasingly used for infants with bronchiolitis on general pediatric wards, yet factors associated with pediatric intensive care unit (PICU) transfer and escalation beyond HFNC remain poorly defined. Our aim was to characterize clinical trajectories of infants started on HFNC on the pediatric floor and identify factors associated with PICU admission and escalation of respiratory support. METHODS: We conducted a retrospective cohort study of children younger than 24 months of age admitted with bronchiolitis to a tertiary children's hospita…

High-flow nasal cannula improves pulmonary gas exchange during endoscopic retrograde cholangiopancreatography: A single-center randomized controlled trial.

BACKGROUND: Sedation during endoscopic retrograde cholangiopancreatography procedures (ERCP) increases the risk of hypoxemia and the need for respiratory support in high-risk patients. High flow nasal cannula (HFNC) provides humidified gas at high flow rates, improving alveolar ventilation. We aim to assess whether ventilatory support with HFNC improves gas exchange compared to standard low-flow nasal cannula (NC). METHODS: Single-center, randomized controlled trial, in adults undergoing ERCP. After providing informed consent, participants were randomized in a 1:1 ratio following obstructive …

Effectiveness of maintaining oxygenation of a new high flow nasal cannula for endoscopic retrograde cholangiopancreatography during intravenous anesthesia.

BACKGROUND: Intravenous anesthesia during endoscopic retrograde cholangiopancreatography (ERCP) is associated with a high incidence of respiratory depression. High-flow nasal cannula (HFNC) therapy uses a mild positive pressure load to enhance carbon dioxide washout and reduce rebreathing, thereby improving respiratory function. Therefore, it is widely used to prevent hypoxemia and hypercapnia. We aim to investigate the efficacy of a new HFNC device (AIRVO 3™) in improving respiratory status during sedated ERCP. METHODS: In a multicenter randomized controlled study involving two groups-the HF…

Effect of Cannula Fenestration Area on Fat Graft Architecture and Histology: A Double-Blind, Split-Body Clinical Trial.

BACKGROUND: Fat grafting is widely used in reconstructive and aesthetic surgery due to its autologous nature and biocompatibility; however, resorption rates of 40-50% remain a limitation. Adipocyte survival depends on harvesting, processing, and injection techniques. This study aimed to evaluate whether liposuction cannula diameter and fenestration area influence macroscopic and histologic characteristics of harvested adipose tissue. MATERIALS AND METHODS: A double-blind, non-randomized, split-body study was conducted. The abdomen was divided into four quadrants. Four cannula configurations w…

Ward-Based High-Flow Nasal Cannula Led by the Medical Emergency Team: A Pragmatic Model for Resource Stewardship.

High-flow nasal cannula (HFNC) for acute hypoxemic respiratory failure is typically restricted to ICUs. We evaluated a ward-based, medical emergency team (MET)-supervised HFNC protocol (flow ≤ 40 L/min, Fio2 ≤ 0.40) with 2-/4-/8-hour nursing and respiratory therapist reassessments. Among 82 ward HFNC initiations (2021-2024), 38 (46%) required immediate ICU transfer (IMT) and 44 (54%) were Ward-Managed After MET (WMAM). Of WMAM patients, 18 transferred to ICU within 48 hours, and 26 remained on ward. WMAM patients accrued a median 1.46 ICU bed-days saved (interquartile range, 0.73-2.67); bootst…

Impact of drainage cannula type on patients' fluid balance in venovenous extracorporeal membrane oxygenation: A historical cohort study.

Objective: Positive fluid balance in patients on veno-venous extracorporeal membrane oxygenation (VV ECMO) is associated with increased mortality. Drainage insufficiency on VV ECMO is a common occurrence and managed with fluid administration. The use of multi-stage drainage cannulas may result in decreased drainage insufficiency, but it is unclear the effects this may have on patient fluid balance.Design: A historical cohort study was conducted on adult patients undergoing VV ECMO for acute respiratory failure in femoral-jugular configuration.Setting: Intensive Care Unit.Patients: Adult patien…

High-Caliber Femoral Dual-Lumen Cannula for ECCO 2 R in Hypercapnic Respiratory Failure: Efficacy and Safety Evaluation.

Extracorporeal carbon dioxide removal (ECCO₂R) prevents intubation and facilitates extubation in patients with hypercapnic respiratory failure. However, low-flow systems increase shear stress and need full anticoagulation, increasing the risk of circuit-related complications. We assessed the safety and efficacy of a high-caliber dual-lumen cannula, designed for jugular veno-venous extracorporeal membrane oxygenation (VV ECMO), repurposed for femoral vein insertion, with the aim of achieving higher blood-flow rates and more efficient CO₂ clearance. We retrospectively analyzed 16 intensive care …
