Nutrition in Pediatric Cardiomyopathy.

Pediatric cardiomyopathies are heterogeneous groups of serious disorders of the heart muscle and are responsible for significant morbidity and mortality among children who have the disease. While enormous improvements have been made in the treatment and survival of children with congenital heart disease, parallel strides have not been made in the outcomes for cardiomyopathies. Thus, ancillary therapies, such as nutrition and nutritional interventions, that may not cure but may potentially improve cardiac function and quality of life, are imperative to consider in children with all types of car…

[Peripartum dilated cardiomyopathy].

AIM: To analyze the clinical characteristics and to evaluate the different factors that influences the prognosis of the peripartum cardiomyopathy (PPCM). METHODS: A retrospective review was undertaken on records of women who were diagnosed with peripartum cardiomyopathy at Farhat Hached Hospital (Sousse) between January 1992 and December 2004. RESULTS: Clinically, PPCM shows pulmonary symptoms such as dyspnea and tachypnea. The diagnosis is established by echocardiography that showed decreased systolic function of the left ventricular. Both gynecologist and cardiologist must check the patien

Cardiomyopathy associated with clozapine.

Clozapine is an atypical antipsychotic agent that is more effective than the standard neuroleptics currently used for treating refractory schizophrenia. In addition, clozapine is a drug with few extrapyramidal side effects. However, clozapine is also associated with potentially serious adverse effects, such as cardiac complications as well as agranulocytosis. Clozapine-related cardiomyopathy has not been previously reported in East Asia. This report describes a 31-year-old Korean male patient with schizophrenia who developed dilated cardiomyopathy on treatment with clozapine. The removal of cl

Restrictive cardiomyopathy in dermatomyositis.

The association between dermatomyositis and restrictive cardiomyopathy has not been reported before. We present here the clinical, echocardiographic and muscle biopsy data for a patient with dermatomyositis and restrictive cardiomyopathy. In a 78-year-old male with a history of arterial hypertension, recurrent episodes of atrial fibrillation and syncopes, rupture of an infra-renal aortic aneurysm with complications (recurrent QT-prolongation, lumbo-sacral plexopathy, transient ischaemic attack, peripheral embolism), monoclonal gammopathy, subdural haematoma, focal seizures, megaloblastic anaem

Obstructive hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a primary disease of cardiac muscle characterized by a thickening of the left ventricular wall and often predominantly affecting the interventricular septum. This paper presents a case study of a 53-year-old female with a dynamic and obstructive form of HCM. The study includes a case presentation, clinical findings, investigations, and management. Patient findings were obtained before and after alcohol septal ablation, a novel interventional therapy. The article reviews the various forms of HCM and describes the currently available treatment modalities for

Peripartum cardiomyopathy. A review.

The objective of this article is to review the aetiology, epidemiology, diagnosis, clinical course, treatment and prognosis of peripartum cardiomyopathy (PPCM). The medical literature from 1966 to March 2002 was reviewed through MEDLINE. PPCM is a rare complication in pregnancy. It is seen in late pregnancy or in the early puerperium. The aetiology of this disease remains uncertain. Many possible causes have been proposed, including myocarditis, abnormal immune response to pregnancy, maladaptive response to the hemodynamic stresses of pregnancy and prolonged tocolysis. Risk factors for PPCM in

Mitral reconstruction in cardiomyopathy.

Functional mitral regurgitation (MR) is a significant complication of end-stage cardiomyopathy, and may affect most heart failure patients as a pre-terminal or terminal event as it develops secondary to a change in the annular-ventricular apparatus and altered ventricular geometry; this results in incomplete leaflet coaptation.. Historically, surgery in MR patients was mitral valve replacement, but the adverse effects on LV systolic function of interrupting annulus-papillary muscle continuity was poorly understood. At the University of Michigan (1993-2000), 140 patients with end-stage cardiomy

Is cardiomyopathy an autoimmune disease?

Idiopathic dilated cardiomyopathy (DCM) is one of the leading causes of severe heart failure and the most common cause of heart transplantation due to its ventricular dilatations and contractile dysfuntions. Twenty percent of DCM is in the familiar form and the rest is sporadic. The clinical impact of DCM is far greater than its position in epidemiological terms. Despite recent improvements in therapy, both incidence and mortality are still very high. The main problem is its heterogeneous etiology. So far, three factors have been identified to be potentially important: enteroviral infection, i

Pathogenesis and treatment of cardiomyopathy.

Dilated cardiomyopathy is a disease of diverse causes. Untreated, this condition will likely progress to advanced heart failure, sudden death, or both. There have been a number of significant advances in the understanding of the mechanisms involved in the pathogenesis of this disease. Treatments that counteract ventricular remodeling with neurohumoral blockade have substantially improved the outcome for many patients. Although cardiac transplantation is of benefit to patients with advanced disease, the growing donor heart shortage limits this option. New treatments are clearly needed, and many

Cardiomyopathy and encephalopathy in AIDS.

HIV encephalopathy has been in the past years the most typical CNS disorder in patients with AIDS. Histologic abnormalities consist in astrocytosis, myelin pallor, infiltration by infected macrophages, resident microglia and multinucleated giant cells, generally in absence of direct infection of neurons. Mononuclear phagocytes in the brain are the main target of HIV-1 infection and the site of productive viral replication, and viral stimulation leads to the release of neurotoxic products causing neurologic damage. Subclinical cardiac abnormalities are common in HIV+ patients and several studie

[Dilated thyrotoxic cardiomyopathy].

We report a case of reversible, dilated cardiomyopathy due to thyrotoxicosis, which occurred in a young male without any underlying heart disease. The patient presented a clinical picture of cardiogenic shock related to severe left ventricular dilation and dysfunction and with new-onset atrial fibrillation and very high ventricular rate. In spite of vigorous medical therapy, there was only a mild improvement of clinical and hemodynamic status and ventricular rate persisted inappropriately elevated. Subsequently, laboratory test results allowed for recognition of thyrotoxicosis (secondary to Gr

The inheritance of hypertrophic cardiomyopathy.

The inheritance of hypertrophic cardiomyopathy is autosomal dominant, "sporadic" cases are now recognised to be uncommon. In the past 5 years molecular genetic advances have isolated >100 different mutations in genes encoding sarcomeric proteins. The cardiac morphology and prognosis are linked to the underlying genetic defect, but intra and interfamilial variation suggest polymorphism or mutation of other genes, or environmental factors. Penetrance is age related and negative echocardiographic screening cannot guarantee absence of disease. Currently mutation analysis is not routine in clinical

Hypertrophic cardiomyopathy in children.

Hypertrophic cardiomyopathy is a congenital disease that may be manifested in infancy, childhood, adolescence, or young adulthood. It is heterogeneous both genotypically and phenotypically, and as such, it requires an individualized approach to medical management. The symptomatic patient can have progression of symptoms but can be treated with appropriate medical or surgical therapy. The symptomatic infant usually has a grave prognosis and should be given early consideration for heart transplantation. The future holds promise because genotyping may recognize pedigrees with more "malignant" cou

Arrhythmias in hypertrophic cardiomyopathy.

Supraventricular and ventricular arrhythmias, particularly nonsustained ventricular tachycardia, and ventricular premature beats are a common finding in patients with hypertrophic cardiomyopathy. Several investigations have demonstrated that nonsustained ventricular tachycardia on Holter monitoring is associated with an increased risk of sudden cardiac death. It has been a long lasting controversial discussion whether suppression of these arrhythmias with drugs, such as amiodarone is capable to reduce the incidence of sudden cardiac death. While recent studies have indicated that nonsustained

Apical hypertrophic cardiomyopathy.

Illustrated by a case report, which was rather suggestive of coronary ischemic heart disease, the principle characteristics of apical hypertrophic cardiomyopathy are outlined. Outside East Asia, it remains a very uncommon variant of hypertrophic cardiomyopathy. The electrocardiogram in basal conditions showed giant negative T-waves (-14 mm) in the precordial leads, while the ventriculogram in RAO projection revealed the typical, so called "ace of spades" configuration. Based on the present literature and the follow-up of these patients a review of apical hypertrophic cardiomyopathy is presente

Arrhythmia and hypertrophic cardiomyopathy.

Atrial fibrillation is the commonest arrhythmia observed in hypertrophic cardiomyopathy, and is associated with an acute deterioration in symptoms. Digoxin is the drug of choice in established atrial fibrillation and amiodarone the drug of choice in paroxysmal atrial fibrillation and ventricular arrhythmia. Non-sustained ventricular tachycardia occurs in 20% of patients and is the single best predictor of sudden death in adults. Sustained monomorphic ventricular tachycardia occurs only rarely. The mechanism of sudden death is likely to involve initiating factors such as arrhythmia and peripher
