A patient with lumbar disc herniation complicates cauda equina syndrome after epidural steroid injection: A case report.

RATIONALE: While epidural steroid injections (ESI) are a routine component of lumbar disc herniation management, the procedure carries inherent risks. Certain clinical presentations may signal an elevated risk for catastrophic complications, such as cauda equina syndrome. This report examines the significance of severe positional pain as a pre-procedural warning sign. PATIENT CONCERNS: We present the case of a 65-year-old male who exhibited a critical physical sign: severe low back pain prohibiting a supine position. Despite this red flag, an interlaminar ESI was administered at the L2-3 leve…

Spinal Venous Engorgement Secondary to Congenital Absence of the Infrarenal Inferior Vena Cava Mimicking Cauda Equina Syndrome.

This case report contributes to the limited literature by describing a man in his early forties who presented with symptoms suggestive of cauda equina syndrome (CES). Initial investigations focused on identifying a possible spinal lesion as the underlying cause. However, further imaging and multidisciplinary team assessment revealed a rare vascular anomaly: agenesis of the infrarenal segment of the inferior vena cava (IVC), a condition with a distinct etiology that requires a fundamentally different approach to management.

Suspected and surgically managed cauda equina syndrome nationwide: epidemiological trends and socioeconomic factors influencing access to care.

BACKGROUND: Cauda equina syndrome (CES) is a critical condition requiring timely intervention to prevent severe morbidity. This study investigates the epidemiology and socioeconomic factors influencing access to CES care in USA Emergency Departments. METHODS: Data was used from the Nationwide Emergency Department Sample (NEDS) from 2016-2020. Encounters for patients presenting with suspected CES were queried using ICD 10 codes. Incidence estimates for suspected and surgically managed CES were constructed. Encounter characteristics were tabulated to describe aspects of a typical CES presentati…

Minimally invasive decompression alone versus fusion surgery for acute lumbar disk herniation combined incomplete cauda equina syndrome.

STUDY DESIGN: Retrospective single center study. BACKGROUND: Cauda equine syndrome (CES), which caused by acute lumbar disc herniation (LDH), often requires urgent surgical treatment. Currently, there is no standard defining the type of surgery, and approaches to the treatment of patients are based on the experience of the surgeon. PURPOSE: to compare the clinical efficacy of minimally invasive decompression alone (Decompression alone group) and transforaminal lumbar interbody fusion (Fusion group) in the treatment of the incomplete CES, caused by acute LDH. PATIENTS AND METHODS: 89 patient

Piriformis syndrome as a result of intramuscular haematoma mimicking cauda equina effectively treated with piriformis tendon release.

We present a case of piriformis syndrome in a woman in her 30's following low energy trauma, presenting with unilateral lower limb weakness, altered sensation and urinary retention. CT imaging revealed a bulky piriformis muscle which was further clarified on MRI as an intramuscular haematoma within the left piriformis causing compression of the left lumbosacral plexus. Haematoma formation was exacerbated due to use of an antiplatelet medication the patient was taking for Moyamoya disease, which carries an increased risk of cerebrovascular accident. Surgical exploration of the piriformis and sc

Two cases of myelin oligodendrocyte glycoprotein antibody-associated disease presenting with Cauda Equina Syndrome without conus myelitis.

Myelin oligodendrocyte glycoprotein antibody-associated disorder (MOGAD) is a central nervous system inflammatory disorder associated with MOG antibodies. Two patients with clinical symptoms of cauda equina syndrome had positive serum MOG antibody tests, and spinal magnetic resonance imaging showed cauda equina enhancement. They were diagnosed with incomplete cauda equina syndrome associated with MOGAD. A few cases of lumbosacral radiculomyelitis associated with MOGAD have been reported; however, this is the first report of isolated lumbosacral radiculitis associated with MOGAD without transve…

Proximal Nerve Root Involvement in immunoglobulin M Anti-Myelin-Associated Glycoprotein Neuropathy Presenting as Cauda Equina Syndrome.

Typical distal symptoms in anti-myelin-associated glycoprotein (anti-MAG) neuropathy are believed to be due to the binding of immunoglobulin M to distal nerve terminals. We describe the case of a 56-year-old man diagnosed with immunoglobulin M anti-MAG neuropathy in the setting of Waldenström macroglobulinemia, which developed acute neurological worsening presenting as cauda equina syndrome. Lumbosacral magnetic resonance imaging revealed enlarged nerve roots with diffuse heterogeneous gadolinium enhancement. Treatment with steroids resulted in substantial clinical improvement. Increased recog…

[Hypothesis concerning the anatomical basis of cauda equina syndrome and transient nerve root irritation after spinal anesthesia].

OBJECTIVE: Cauda equine syndrome is a rare neurological complication associated with subarachnoid anesthesia, and particularly with the use of 5% hyperbaric lidocaine and small gauge catheters. Our aim was to study a possible anatomical factor that might impede adequate dilution of local anesthetic and explain the development of cauda equine syndrome and transitory radicular irritation. MATERIAL AND METHOD: The spinal dura matters and their contents from two male human cadavers were examined after organs had been extracted for transplantation. Both men had recently died at ages 56 and 65 year

Posterior vertebral body erosion by arachnoid diverticula in cauda equina syndrome: an unusual manifestation of ankylosing spondylitis.

Cauda equina syndrome is an uncommon complication of longstanding ankylosing spondylitis. It is associated with dorsal arachnoid diverticula, which may erode the lamina and spinous processes of the bony lumbosacral spine. We describe a patient who developed cauda equina syndrome associated with the unusual finding of erosion of the posterior aspect of 2 vertebral bodies by arachnoid diverticula. This was clearly revealed by magnetic resonance imaging of the spine.

1991 Volvo Award in experimental studies. Cauda equina syndrome: neurologic recovery following immediate, early, or late decompression.

An animal model of cauda equina syndrome was developed. Neurologic recovery was analyzed following immediate, early, and delayed decompression. Five experimental groups, each containing six dogs, were studied. Compression of the cauda equina was performed in all 30 dogs following an L6-7 laminectomy. The cauda equina was constricted by 75% in each group. The first group was constricted and immediately decompressed. The remaining groups were constricted for 1 hour, 6 hours, 24 hours, and 1 week, respectively, before being decompressed. Somatosensory evoked potentials were performed before and a…
