Flow volume mismatch dramatically affects transient neurologic symptoms after direct bypass in Moyamoya disease.

Transient neurological events (TNEs) occur after bypass surgery in Moyamoya disease (MMD); however, their pathology remains unknown. To elucidate the pathophysiology of TNEs, we investigated their relationship with perioperative superficial temporal artery (STA) blood flow volume, which was evaluated using ultrasonography. Forty-nine patients with MMD, who underwent direct bypass surgery, were included and stratified into TNE and non-TNE groups, respectively. The STA blood flow volume was evaluated at four time points (preoperatively and 2-4, 7, and 10-14 days postoperatively), and a change in

Surgical Resection and Clipping of an Atherosclerotic Partially Calcified Middle Cerebral Artery Trifurcation Aneurysm with Clipping of an Anterior Communicating Artery Aneurysm: 2-Dimensional Operative Video.

It is estimated within the western population that 10%-13% of patients possess multiple intracranial aneurysms1 and are linked to certain risk factors. Thrombotic aneurysms are a rare subgroup of complex aneurysms characterized by an organized intraluminal thrombus.2,3 They differ from typical saccular aneurysms in terms of morphology, natural history, symptomatology, and difficulty in treatment with conventional strategies.2,4 The risk of rupture is poorly characterized and assumed to be comparable with that of nonthrombotic aneurysms.2 A subset of thrombotic aneurysms can be treated surgical

Preoperative 3D Image Evaluation of EC-IC Bypass by 3D Visualization Analysis Software Amira®.

BACKGROUND: Careful evaluation of the preoperative imaging for extracranial-intracranial bypass performed for conditions like intracranial stenosis and Moya disease is important. The traditional use of 2D imaging has a significant limitation for neurosurgeons, primarily to determine the optimal location of the recipient artery for performing the surgical bypass. Therefore, many neurosurgeons use 3D angiograms more frequently to overcome these shortcomings. MATERIALS AND METHODS: We performed the preoperative evaluation of the possibility of performing an anastomosis between the superficial te

[Emergency Superficial Temporal Artery-Middle Cerebral Artery Bypass for Atherosclerotic Ischemic Stroke].

Endovascular recanalization is the primary strategy for the treatment of acute embolic stroke. However, atherosclerotic occlusions are often challenging to recanalize, and only medical therapy can be performed. In these cases, even the best medical treatment may not be effective, and the cerebral infarction progressively worsens. We believe that an emergency superficial temporal artery-middle cerebral artery(STA-MCA)bypass could be effective in these situations, after careful case selection. We use the following eligibility criteria: (1)atherosclerotic infarction; (2)cerebral ischemia with blo

Angiographic disease progression in medically treated adult patients with ischemic moyamoya disease without cerebral misery perfusion: supplementary analysis of a 5-year prospective cohort.

Angiographic disease progression reportedly develops in adult moyamoya disease (MMD). However, more than half of patients analyzed underwent revascularization surgery. The present supplementary analysis of a 5-year prospective cohort with follow-up using magnetic resonance angiography (MRA) and cerebral blood flow (CBF) measurements was to elucidate the incidence and clinical features of angiographic disease progression in adult patients receiving medical management alone for ischemic MMD. Sixty-eight patients without misery perfusion in the symptomatic cerebral hemispheres underwent MRA and C

Occlusion of Extracranial-Intracranial Bypass Anastomosis-Associated Aneurysms Following Contralateral High-Flow Extracranial-Intracranial Bypass in a Patient with Impaired Cerebrovascular Reserve.

BACKGROUND AND IMPORTANCE: Extracranial-intracranial (EC-IC) bypass anastomosis-associated aneurysms are rare sequelae of cerebral revascularization surgery. Although treatment paradigms are not well defined, clipping, trapping with revision bypass, and donor vessel ligation represent the most common microsurgical approaches. CLINICAL PRESENTATION: A 53-yr-old male presented with cognitive decline, left extremity weakness, and left visual field blurriness. Computed tomographic angiography of head/neck demonstrated bilateral cervical internal carotid artery occlusion and magnetic resonance ima…

Sphenoid Wing Meningioma with Surgical Revascularization of an Injured Anterior Temporal Artery.

A 79-year-old woman presented to the emergency department following multiple falls in the previous week. Her past medical history was unremarkable except for chronic obstructive pulmonary disease. She was neurologically intact on arrival with normal laboratory data. Brain imaging revealed a right-sided sphenoid wing meningioma with considerable edema around it. Owing to her cognitive decline and recent symptoms, it was decided to proceed with surgical resection of the lesion. Considering her age and underlying disease, we planned to avoid probable complications of an aggressive resection. She

Occipital artery to upper posterior circulation bypass through the presigmoid approach in revascularization of the posterior circulation: An anatomical study.

We describe a novel extracranial (EC)-to-intracranial (IC) bypass technique between the occipital artery and upper posterior circulation (UPC) for revascularization of the posterior circulation through the presigmoid approach. Five formalin-fixed human heads were examined to demonstrate the detailed anatomy of the occipital artery and UPC and illustrate the step-by-step bypass procedure. The occipital artery, a branch of the external carotid artery, can be anastomosed to the P2/P3 segment of the posterior cerebral artery and S1/S2 segments of the superior cerebellar artery as an alternative to

Middle Meningeal-Middle Cerebral Artery Anastomosis for Moyamoya Disease.

BACKGROUND: We describe a unique anastomosis procedure involving successful middle meningeal artery (MMA) to cortical middle cerebral artery (MCA) bypass, as well as superficial temporal artery (STA) to cortical MCA bypass. CASE DESCRIPTION: A 46-year-old woman with hemorrhagic Moyamoya disease underwent an anastomosis procedure with the MMA and STA serving as donor arteries. We performed a small craniotomy at the areas closest to the donor STA, MMA, and recipient cortical MCA, and completed STA-MCA and MMA-MCA direct bypass procedures. CONCLUSIONS: This is a rare case of successful MMA-to-M

Reversal of the reversed 'Robin Hood syndrome' after superficial temporal-Middle cerebral artery bypass for severe intracranial stenosis.

Background: Intracranial arterial stenosis is a common cause of ischemic stroke. Recent attempts with intracranial stenting and superficial temporal-middle cerebral artery (STA-MCA) bypass failed to show benefit. Perhaps, better strategies are needed for selecting suitable patients. Acetazolamide-challenged single photon emission computed tomography (SPECT) can demonstrate reversed Robin Hood syndrome (RRHS) due to intracranial steal phenomenon and identify high-risk patients. Methods: We describe the clinical and SPECT findings of RRHS.Results: In our patient with severe and symptomatic severe

Urgent Bypass Surgery Following Failed Endovascular Treatment in Acute Symptomatic Stroke Patient With MCA Occlusion.

INTRODUCTION: Although the benefits of extracranial-intracranial bypass surgery remain controversial, there is some surgical rationale for the augmentation of cerebral blood flow in cases of acute ischemic stroke with hemodynamic instability. CASE REPORT: We report a case of a 62-year-old woman who suddenly developed right hemiplegia and global aphasia. Initial magnetic resonance imaging and magnetic resonance angiography revealed a small acute ischemic lesion in left parietal lobe with occlusion at the left middle cerebral artery. We performed an endovascular thrombectomy, which failed. Her

Variant angina in moyamoya disease--a correlative etiology and different presentation: a case report.

INTRODUCTION: Moyamoya disease is characterized by progressive steno-occlusive changes of the distal internal carotid and developed collateral vasculature, so called 'moyamoya' vessels at the base of the brain. Variant angina is a rare occurrence in patients with moyamoya disease. CASE PRESENTATION: Here we report the case of a 41-year-old Korean woman who developed chest pain after indirect revascularization surgery of moyamoya disease. A treadmill test and an exercise stress echocardiograph showed positive results, but there was no significant major coronary arteries stenosis. Suspicious of

Microsurgical neurovascular anastomosis: the example of superficial temporal artery to middle cerebral artery bypass. Technical principles.

The superficial temporal artery to the middle cerebral artery (STA-MCA) bypass is a good example of cerebrovascular anastomosis. In this article, we describe the different stages of the procedure: patient installation, superficial temporal artery harvesting, recipient artery exposure, microsurgical anastomosis, and closure of the craniotomy. When meticulously performed, with the observance of important details at each stage, this technique offers a high rate of technical success (patency>90%) with a very low morbi-mortality (respectively 3% and 1%). Some anesthetic parameters have to be consid

[(12)Revascularisation and aneurysms of the posterior circulation].

This is the last article, namely, the twelfth topic, in the series "personal view and historical backgrounds of operative neurosurgery." In this article, revascularisation and aneurysms of the posterior circulation are thematized. As an example for classic posterior circulation EC-IC bypass, a case of basilar stenosis is presented. The stenosis turned out to occlusion by good patency of the STA-SCA bypass, and although it initially showed clinical improvement, it manifested as severe pontine infarction several months later. Considerations on such malignant conversion are discussed together wit

[Moyamoya disease].

Moyamoya disease is characterized by progressive steno-occlusive changes at the terminal portions of the bilateral internal carotid arteries with arterial collateral vessels at the base of the brain. As the incidence of moyamoya disease is highest in Japanese and Asians, Japanese researchers have made enormous efforts to illuminate the pathogenesis, the epidemiology, the clinical features and the treatment of this disease for five decades. The disease was called variously according to each researcher's assumption of the pathogenesis in the early period. After presentation of the initial nation

Elevation of nitric oxide metabolites in the cerebrospinal fluid of patients with moyamoya disease.

BACKGROUND: To investigate whether nitric oxide (NO) contributes to formation of abnormal collateral circulation in patients with moyamoya disease. METHODS: Cerebrospinal fluid (CSF) samples were obtained from the subarachnoid space of the Sylvian fissure during combined bypass surgery for moyamoya disease and kept frozen until NO metabolites, nitrate and nitrite, were measured using a Griess method. RESULTS: Compared with control CSF obtained from 18 patients with hemifacial spasm, unruptured aneurysm, etc., concentrations of NO metabolites in 26 CSF samples of 18 patients with moyamoya dis

Complications of intracranial bypass for vertebrobasilar insufficiency.

The authors' initial experience with intracranial revascularization of vertebrobasilar insufficiency, reported previously, fortunately yielded reasonably good results with high patency rates but in some cases there were significant, albeit temporary, complications. Since that time, major brain-stem strokes have occurred in two patients following superficial temporal to superior cerebellar artery bypass procedures. This occurrence has caused the authors to reassess their experience with this procedure and review the published literature with regard to complications. This review and the results
