C1 lateral mass screw insertion with protection of C1-C2 venous sinus: technical note and review of the literature.

STUDY DESIGN: This is a technical note and review of the literature. OBJECTIVE: We propose to describe a revised surgical technique of C1 lateral mass screw insertion with protection of C1-C2 venous sinus surrounding the C2 nerve root. SUMMARY OF BACKGROUND DATA: During C1 lateral mass screw insertion and in posterior C1-C2 fixation, iatrogenic injury of C1-C2 venous sinus results in bleeding, which is troublesome. Appropriate management of the venous sinus in this region is critical to successful surgery in this complex anatomic region. METHODS: We reviewed 48 patients who underwent poster

Occult congenital anomaly of the atlas presenting in the setting of acute trauma.

Congenital absence of the posterior arch of the atlas is rare. Its detection in the setting of acute trauma may be confusing and mimic a fracture. This case serves to highlight the adjunctive use of computed tomography in equivocal cases. Most patients can be managed conservatively, however operative treatment should be reserved for patients in whom atlanto-axial instability has been shown.

Assimilation of the atlas with associated syringomyelia and Chiari 1 malformation (Klippel-Feil type 2).

This case report describes an unusual congenital anomaly of the craniovertebral junction in a 14 year old male. Cervical radiographs showed the bony anomaly, while thin slice CT (with 2-D and 3-D reformations), MR imaging, and angiography were particularly useful in accurately assessing the bony, soft tissue and vascular anatomy, prior to and following surgery.

Vertebro-PICA aneurysms: midline suboccipital approach and laminectomy of the atlas.

Vertebro-PICA aneurysms represent a particular technical challenge because of the extremely narrow operative field and the presence of numerous vital neural and vascular structures. The lack of exposure still remains the limiting factor in most cases. During the 9-year period from January 1980 to October 1989, 8 vertebro-PICA aneurysms have been treated microsurgically. Of these, 2 underwent a pure suboccipital approach while 6 underwent a midline suboccipital approach with deliberate removal of the arch of the atlas in the park bench position. Seven patients did well and regained full activit

[Treatment by a direct surgical approach of a false arterio-venous traumatic aneurysm between the vertebral artery and the internal jugular vein at the level of the atlas (author's transl)].

The authors report a case of traumatic arteriovenous fistula between the vertebral artery and the internal jugular vein at the level of the atlas, treated successfully by a direct surgical approach which permitted exclusion after simple dissection. The relative ease of the surgical approach of the suboccipital segment of the vertebral artery may permit one to extend this technique to other more common indications, in particular due to atheroma.

Aberrant TRPC1 expression reflects stromal cervical invasion, lymphovascular invasion, elevated FIGO stage, and poor survival in resectable endometrial carcinoma patients.

BACKGROUND: Transient receptor potential channel 1 (TRPC1) promotes tumor growth and metastasis in endometrial carcinoma (EC) cell lines, whereas its clinical role in EC management remains unclear. Therefore, this study aimed to investigate the association of TRPC1 protein expression with the clinical features and survival of EC patients, then was further validated by TRPC1 mRNA measurement and data from The Human Protein Atlas. METHODS: TRPC1 protein expression in tumor tissues and normal endometria of 176 resectable EC patients was determined using immunohistochemistry. Besides, TRPC1 mRNA

Torticollis in children caused by congenital anomalies of the atlas.

Hemi-atlas is a rare congenital anomaly in the formation of the first cervical vertebra. It may cause a rather severe and progressive torticollis. When a child is young, the neck, despite the deformity, is flexible and the torticollis can be passively corrected. However, in some patients it becomes increasingly severe and fixed. I describe the findings in seventeen patients, seven of whom were operated on between 1975 and 1983. Treatment with a brace was shown to be ineffective. In patients with severe deformities, fusion of the upper part of the cervical spine is recommended. Fusion was obtai

[Calcium pyrophosphate dihydrate deposition disease in the transverse ligament of the atlas].

Calcium pyrophosphate dihydrate (CPPD) deposits, answerable for chondrocalcinosis, are frequently observed in elderly people. Involvement of the spine is not rare. We present the case of a woman hospitalised for an acute arthritis of the right knee related to CPPD crystal deposition disease who suffered from acute neck pain. The computerised tomography showed calcified deposits in the transverse ligament of the atlas highly suggestive of CPPD deposits. This localisation seems to be very common for CPPD deposits. These CPPD crystal deposits may induce pseudo-meningitic attacks or chronic mechan

Evidence of occipitoatlantoaxial malformations independent of the HOXD3 mutation in an Arabian horse.

Occipitoatlantoaxial malformations (OAAM) are developmental disorders occurring at the occiput, atlas and axis. OAAM type 1 is a familial disorder affecting Arabian horses. Currently, a single genetic mutation in the HOXD3 gene region can be tested to confirm the diagnosis. This case describes a 3-year-old Arabian gelding affected with OAAM. Clinical examination revealed a proximal cervical deformation on the right side and severe stiffness of the neck in the sagittal plane without associated pain. Neurological examination demonstrated a generalized proprioceptive ataxia. Radiographs revealed

Contributions from forensic imaging to the investigation of upper cervical fractures.

Injuries to the upper cervical spine (UCS) are common in traumatic deaths and postmortem computed tomography (PMCT) may contribute to the forensic investigation. This study presents PMCT in comparison with autopsy in the examination of UCS injury. Thirteen consecutive cases with UCS fracture and ⁄ or cranio-cervical dislocation were examined with PMCT and autopsy, and the findings were correlated. Neither of the techniques identified all UCS injuries. Fractures of atlas and axis were best visualized with PMCT whereas cranio-cervical dislocation was better identified during autopsy. Serious inj

Prognosis of neurological deficits associated with upper cervical spine injuries.

We investigated the type of injury and neurological prognosis in 82 patients with an upper cervical spine injury with neurological deficits, from a total of 247 such patients that we treated, from which 11 patients who were dead on arrival had been excluded. The incidence of neurological deficits in upper cervical spine injury was 33%. They were classified into three signs; cord, upper cervical nerve root, and cranial nerve signs. The types of injury accompanied by neurological deficits were burst fracture of the atlas, type II dens fracture, body fracture of the axis, type II traumatic spondy

Anatomical Variants of the Cervical Spine and Sports-Related Injury in Athletes: A Narrative Review.

Congenital and developmental cervical spine variants, as well as acquired or sport-adapted cervical morphologies, may alter canal reserve, segmental stability, foraminal dimensions, and the cervicothoracic outlet in athletes. This narrative review aimed to integrate the anatomically and methodologically heterogeneous literature linking these findings with sports-related injury and return-to-play decisions. PubMed/MEDLINE was searched from database inception through 6 July 2026 using controlled vocabulary and free-text terms for cervical anatomy, specific variants, athletic exposure, neurologic

Unilateral Biportal Endoscopic Resection of the Posterior Arch of the Atlas for Crowned Dens Syndrome.

BACKGROUND Crowned dens syndrome (CDS) is a rare disorder caused by calcium pyrophosphate deposition around the odontoid process. Although most patients respond favorably to conservative management, some develop refractory myelopathy requiring surgical intervention. We present a minimally invasive surgical technique for CDS complicated by spinal cord compression. CASE REPORT An 86-year-old woman with CDS exhibited progressive myelopathy, including thoracoabdominal girdle sensation, gait disturbance ("walking on cotton"), and impaired fine motor function in the upper extremities. Her symptoms w
