Child Abuse, Misdiagnosed by an Expertise Center: Part I-Medico-Social Aspects.

Child abuse is a dangerous situation for an infant. Professionals need to weigh the risk of failing to act when children are seriously harmed against the serious harm done by carrying out safeguarding interventions. In severe cases, foster care might be advisable. The negative effects for the child's psychosocial development requires that such placement must be based on very solid evidence. Our aim is to identify why Dutch parents whose child may have a medical condition that could mimic symptoms of child abuse have a significant chance of being erroneously convicted and losing custody of thei

Young Victims Telling their Stories of Sexual Abuse in the Emergency Department.

Among young children, child sexual abuse is a common type of maltreatment. Child sexual abuse (CSA) is an important national public health problem that has a devastating impact on the emotional and psychological makeup of the child, family, and society. The purpose of this analysis is to explore the young victim's own stories of disclosure of child sexual abuse and is an exploratory case study using reported assault histories of victims of CSA between the ages of 6 and 14, taken in the immediate wake of the assault. The data were extracted from the hospital records of child victims treated in

[Malignant neuroleptic syndrome in a 15-year old girl after a single injection of a high-dose neuroleptic].

The malignant neuroleptic syndrome is a rare and fatal iatrogenic syndrome which requires rapid and effective symptomatic treatment in a somatic hospital, possibly in an intensive care unit. Relatively little medicine (neuroleptics) is employed in child psychiatry and the syndrome is, therefore, rare in child psychiatric departments. A well-defined syndrome is concerned and it is important to be able to recognise and diagnose this.

Participation Patterns, Family Routines and Environmental Factors Prior to Pediatric Psychiatric Hospitalization.

Children admitted to psychiatric wards often experience limited participation in daily activities vital for development. This cross-sectional study compared participation in home and community settings among hospitalized children (HC) with those with neurodevelopmental disorders (ND) and typically developing peers (TD). The study involved 141 parents using the Participation and Environment Measure for Children and Youth and the Family Routines Inventory. Occupational therapists completed the Short Child Occupational Profile following a visit to the HC homes. Results showed significantly lower

Challenges, Preparations, and Recommendations: Family Perspectives on Pediatric Cardiac Surgery Across Childhood and Adolescence.

Congenital heart disease (CHD), the most common birth defect globally, often requires hospitalization for cardiac surgery. While the stress of infant surgery is well-documented, less is known about the experiences of families with older children. We explored the experiences of families of children aged 3-17 hospitalized for cardiac surgery. Semi-structured interviews were conducted. Data were analyzed using qualitative content and thematic analysis. Thirty-two parents (30 mothers and 2 fathers) and 14 patients (10 males and 4 females) participated. Seven themes emerged across three domains: (a

Post-traumatic stress symptoms in parents of adolescents hospitalized with Anorexia nervosa.

The current study was a planned secondary analysis to examine post-traumatic stress symptoms (PTSS) in parents of youth hospitalized for medical stabilization due to anorexia nervosa (AN). Questionnaires were administered to 47 parents (34 mothers, 13 fathers; 10 parental dyads) after admission; follow-up occurred at discharge and 4 weeks, 3 months, and 6 months post-discharge. PTSS were present in the majority of mothers (55.9%) and fathers (61.5%). PTSS were not associated with illness severity, but were associated with parental report of mood symptoms, avoidance, inflexibility, and symptom

A further family of Stromme syndrome carrying CENPF mutation.

Stromme syndrome is a rare genetic disorder characterized by microcephaly, anterior ocular chamber anomalies, and "apple peel" type jejunal atresia. Here, we report a Stromme syndrome family with two affected siblings with a homozygous truncating frameshift mutation in CENPF. A 3-month-old girl was hospitalized due to prenatally diagnosed microcephaly, microphthalmia, and dysmorphological features. The history of a previous child with the same findings in addition to "apple peel" intestinal atresia had been noted. Regarding the clinical features of both affected siblings, a diagnosis of Stromm

Specificity of childhood maltreatment and emotion deficit in nonsuicidal self-injury in an inpatient sample of youth.

The present study investigated the function of two specific emotion-related skills, emotion expressivity and emotion coping, as potential mediators in the relations between childhood sexual, physical, and emotional abuse and NSSI. A robust body of work supports the role of emotion regulation in nonsuicidal self-injury, but additional research is warranted to tease apart the role of specific emotion regulation deficits as predictors of NSSI. Participants included 95 youth (Mage=14.22, SDage=1.67; 58% female) hospitalized on one of two acute care psychiatric inpatient units. Participants complet

Encephalopathy from unintentional donepezil and memantine ingestion.

Donepezil and memantine are commonly prescribed antidementia drugs. There is a paucity of literature concerning pediatric ingestions of these drugs. We describe a case of a 2-year-old child who developed encephalopathy after an unintentional ingestion of donepezil and memantine. A 2-year-old girl was found by her family members agitated and reporting visual hallucinations. In the emergency department, she became sedated and had rightward eye deviation. She was hospitalized and had extensive neurological and infectious disease testing that was unremarkable, except for an electroencephalogram, w

Small bowel intussusception by local recurrence of an inflammatory myofibroblastic tumor: report of a case and review of the literature.

Inflammatory myofibroblastic tumor (IMT) of the ileum is a rare, usually solitary lesion, that frequently presents small-intestinal intussusception and obstruction. We describe an IMT of the ileum in a 4.5-year old child who presented a small bowel intussusception. During laparotomy, an annular mass around the ileum was resected, and the IMT was histologically diagnosed. Three months after the operation, the patients were hospitalized with the symptoms of intestinal obstruction. Laparotomy showed a ileal intussusception. Along the previous suture line of anastomosis, a smooth polypoid tumor wa

Incidence of extended-spectrum beta-lactamases in clinical isolates of the family Enterobacteriaceae in a pediatric hospital.

The incidence of extended-spectrum beta-lactamases (ESBLs) was analyzed in Enterobacteriaceae population circulating in the Upper Silesian Child and Mother Health Center in Katowice (USC&MHC). Altogether 1164 clinical specimens, collected from children hospitalized in 8 different hospital units of USC&MHC were investigated. Five hundred and eighty-five clinical isolates of the family Enterobacteriaceae were identified in specimens collected from 403 patients. Two hundred and twenty-nine Enterobacteriaceae strains (39%) isolated from 162 patients were found to be putative ESBL producers as reve

Anthropometric nutritional assessment of critically ill hospitalized children.

In a prospective study, the nutritional status was evaluated in 46 consecutive admissions to a Pediatric Intensive Care Unit, using anthropometric parameters. About 65% of the patients presented malnutrition on admission, with predominance of the chronic form. The mortality rate was greater in the malnourished (20 versus 12.5%) and chronic malnutrition was associated with a higher incidence of infection (42%). There was a fall in channel of percentile for weight-for-height in 36% of the patients evaluated at the final follow up. These results suggest that a significant number of patients are a

Pimozide in autistic children.

This open pilot study explored the efficacy and safety of pimozide, over a 3-week period, in hospitalized autistic children. Eight males, ages 4.2 to 8.3 years, completed the study. Intellectual functioning ranged from moderate to profound mental retardation. Symptoms included severe withdrawal, stereotypies, hyperactivity and/or hypoactivity, aggressiveness, and temper tantrums. Therapeutic daily doses of pimozide ranged from 3.0 mg to 6.0 mg with a mean of 4.9 mg (0.12-0.32 mg/kg; mean, 0.22). Laboratory studies including electrocardiogram and liver function tests remained within normal limi

[African histoplasmosis. A case].

The case reported concerns a 12 year-old girl, native of the Cameroons, hospitalized in France for the treatment of the most severe disseminated type of African histoplasmosis (Histoplasma duboisii). In addition to a severe infectious syndrome, the child presented with 3 associated typical involvements: diffuse lymphadenopathy, skin lesions consisting of nodules of the face and trunk and suppurative osteo-articular lesions with an impressive radiological appearance: extensive bone lysis and metaphyseal fractures, without any sign of bone reconstruction, even after several months of treatment.

Enteric fevers in children. The importance of age in the varying clinical picture.

The experience with enteric fevers in 71 children hospitalized at the Jordan University Hospital over a 4-year period is reviewed. Forty-one children were male and 30 female. Salmonella typhi was the causative organism in 26 and Salmonella paratyphi A and B, in the remaining 45 patients. Twenty-five children were less than two years of age, and the remaining 46, older than two. Gastroenteritis-like symptoms were seen more often in the younger age group, who also tended to lack splenomegaly and leukopenia. Further, the younger children were sicker at the time of admission and had a longer illne

Neisseria meningitidis serogroup W-135 primary monarthritis of the hip in an immunocompetent child.

Reported here is the first known case of primary monoarthritis of the hip due to Neisseria meningitidis W-135. The isolate was obtained from an immunocompetent child suffering from acute hip pain as the only symptom upon presentation at the hospital. Meningococcal infection must be considered in the diagnosis of any child presenting with arthritis, even if afebrile and without rash.

[Rotary C1-C2 dislocation: a case report].

The atloid-axoid rotatory slipped disc is a rare pathology with still uncertain etiology. Many situations can be factors of this disease. We report a case in a child who was admitted to the hospital for a stiff neck that had been evolving for 1 month. The clinical examination found an irreducible angular deformity of the neck and multiple cervical adenopathies. The ORL examination was normal, the biological tests showed no disorders, and the X-ray examinations were also normal. Unexpected admission features were also disconcerting. The child suddenly presented a stiff neck on waking 2 days aft

Screening Adult Patients in Emergency Departments (EDs) for Risk Factors of Child Maltreatment. A Swedish Pilot Study.

AIM: Swedish healthcare staff are obliged to immediately report suspected child maltreatment (CM) to child protection services (CPS), but often fail this duty. The use of screening instruments in EDs increases detection of adults with risk factors for CM. Such instruments have previously not been available in Sweden. The aim of the present study was to pilot test a Swedish version of the Dutch 'child-check' instrument, and evaluate a retrospective case series. METHODS: Our study was performed in an ED at Skåne University Hospital. The original Dutch instrument was translated with adaptation t
