[Opisthorchiasis-induced suppurative cholangitis (clinical course and diagnosis)].

Purulent cholangitis was found in 128 (44.6 per cent) of the 288 patients operated on for complicated forms of opisthorchiasis. Mechanical cholestasis due to sclerotic changes in the bile ducts, major duodenal papilla and pancreas is the most responsible factor in disease prognosis. Esophagogastroduodenoscopy, ultrasound examination, intra-operative cholangiography and fibrocholedochoscopy are used for diagnosis as well as study of bile microflora in the common bile duct. The acute form of purulent cholangitis was attended by acute liver insufficiency in 42.4 per cent of the cases; fatal outco…

AIDS-related cholangitis: radiographic findings in nine patients.

Acalculous inflammation of the biliary tract is a recently reported complication of the acquired immunodeficiency syndrome (AIDS). In a 33-month period, nine men with AIDS were evaluated because of right upper quadrant and/or epigastric pain, jaundice, or abnormal liver function test results. Each patient underwent ultrasonography and endoscopic retrograde cholangiopancreatography; seven also underwent computed tomography. In eight of nine patients the imaging studies disclosed intrahepatic and extrahepatic bile duct changes identical to those seen in sclerosing cholangitis (strictures, focal …

Endoscopic treatment of ascariasis causing acute obstructive cholangitis.

Ascariasis lumbricoides is one of the most common helminthic infections in developing countries. Biliary ascariasis presents with recurrent cholangitis and sometimes with liver abscesses. We report the case of a young Philippino worker in Saudi Arabia who presented with obstructive jaundice, cholangitis and liver abscesses secondary to biliary tract ascariasis. The diagnosis was made at endoscopic retrograde cholangio-pancreatography (ERCP) and the obstructing worms were removed with a Dormia basket after endoscopic sphincterotomy at this procedure. Full resolution of the biliary and hepatic s…

Risk of colorectal cancer in patients with primary sclerosing cholangitis and concomitant inflammatory bowel disease compared with primary sclerosing cholangitis only.

AIM: Primary sclerosing cholangitis (PSC) increases the risk of colorectal cancer (CRC) in inflammatory bowel disease (IBD) patients; however, there is a paucity of literature to suggest PSC alone as an independent risk factor for CRC. We aimed to determine if PSC is an independent risk factor for CRC in a large tertiary care medical center. Optimizing screening intervals is of great importance, given the burden and risks associated with a lifetime of colonoscopy screening. METHODS: This retrospective cohort study consists of patients diagnosed with PSC preceding IBD (PSC-IBD) and PSC-only be…

High-fat diet modulates bile acid composition and gut microbiota, affecting severe cholangitis and cirrhotic change in murine primary biliary cholangitis.

Increasing evidence suggests that, in addition to a loss of tolerance, bile acid (BA) modulates the natural history of primary biliary cholangitis (PBC). We focused on the impacts of dietary changes on the immunopathology of PBC, along with alterations in BA composition and gut microbiota. In this study, we have taken advantage of our unique PBC model, a Cyp2c70/Cyp2a12 double knockout (DKO), which includes a human-like BA composition, and develops progressive cholangitis following immunization with the PDC-E2 mimic, 2-octynoic acid (2OA). We compared the effects of a ten-week high-fat diet (H…

Inflammation patterns in early post-operative cholangitis predict long-term outcomes in biliary atresia: a potential role of non-suppurative cholangitis.

PURPOSE: Frequent post-operative cholangitis in biliary atresia (BA) affects the long-term native liver survival. This study assessed the characteristics of early cholangitis and their influence on the prognosis. METHODS: Forty-three patients with BA who underwent surgery between 2000 and 2020 were analyzed for routine inflammatory markers. Early cholangitis characteristics were compared between native liver survivor (NLS) and living donor liver transplant (LDLT) patients. RESULTS: Among the 43 patients, 30 (69.8%) experienced 130 episodes of cholangitis. In the area under the receiver opera…

Evaluation of Inhibitory Antibodies against the Muscarinic Acetylcholine Receptor Type 3 in Patients with Primary Biliary Cholangitis and Primary Sclerosing Cholangitis.

BACKGROUND: Primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC) constitute rare chronic inflammatory biliary diseases which likely comprise genetic, environmental and autoimmune factors. Specific inhibitory (auto-) antibodies against the muscarinic acetylcholine receptor type 3 (mAChR3 auto-ab) may contribute to the pathogenesis of chronic biliary inflammation by modulating mAChR3- mediated signaling. AIMS: The aim of this study was to analyze the prevalence and relevance of inhibitory mAChR3 auto-ab (mAChR3inh+ auto-ab) in a large cohort of PBC patients from two indepe…

Clinical diagnostic criteria for IgG4-related sclerosing cholangitis 2020: (Revision of the clinical diagnostic criteria for IgG4-related sclerosing cholangitis 2012).

Several years have passed since the clinical diagnostic criteria for IgG4-related sclerosing cholangitis 2012 were published. New findings and knowledge have accumulated since then. The Research Committees for IgG4-related Diseases and for Intractable Diseases of the Liver and Biliary Tract, in association with the Ministry of Health, Labor, and Welfare of Japan and the Japan Biliary Association, have established a working group consisting of researchers specializing in IgG4-SC and have drawn up new clinical diagnostic criteria for IgG4-SC 2020. The diagnosis of IgG4-SC is based on a combinati…

External validation of the United Kingdom-primary biliary cholangitis risk scores of patients with primary biliary cholangitis treated with ursodeoxycholic acid.

The United Kingdom-Primary Biliary Cholangitis (UK-PBC) risk scores are a set of prognostic models that estimate the risk of end-stage liver disease in patients with PBC at 5-, 10- and 15-year intervals. They have not been externally validated outside the United Kingdom. In this retrospective, external validation study, data were abstracted from outpatient charts and discrimination and calibration of the UK-PBC risk scores were assessed. A total of 464 patients with PBC treated with ursodeoxycholic acid were included. The median diagnosis age was 52.4 years, and 88% were female patients. The c…

Clinical application of the GLOBE and United Kingdom-primary biliary cholangitis risk scores in a trial cohort of patients with primary biliary cholangitis.

The GLOBAL Primary Biliary Cholangitis (PBC) Study Group and United Kingdom-PBC (UK-PBC) Consortium have demonstrated that dichotomous response criteria are not as accurate as continuous equations at predicting mortality or liver transplantation in PBC. The aim of this analysis was to assess the clinical utility of the GLOBE and UK-PBC risk scores using data from POISE, a phase 3 trial investigating obeticholic acid (OCA) in patients with PBC. Data (N = 216) at baseline and month 12 were used to calculate the GLOBE and UK-PBC risk scores to assess the projected change in risk with OCA versus p…

Extensive Metastatic Cholangiocarcinoma Associated With IgG4-Related Sclerosing Cholangitis Misdiagnosed as Isolated IgG4-Related Sclerosing Cholangitis: A Case Report and Literature Review.

As cholangiographic features of IgG4-related sclerosing cholangitis (IgG4-SC) resemble those of cholangiocarcinoma, it is highly confusing between the 2 conditions on the basis of cholangiographic findings. This study presents a case of extensive metastatic cholangiocarcinoma with IgG4-SC misdiagnosed as isolated IgG4-SC, and reviews recent studies of the 2 diseases.A 56-year-old man with no family history of malignant tumors or liver diseases presented with recurrent mild abdominal pain and distention for 3 months. Magnetic resonance cholangiopancreatography showed a 3.7 cm nodular lesion wit…

Increased cholestatic enzymes in two patients with long-term history of ulcerative colitis: consider primary biliary cholangitis not always primary sclerosing cholangitis.

Several hepatobiliary disorders have been reported in ulcerative colitis (UC) patients with primary sclerosing cholangitis (PSC) being the most specific. Primary biliary cholangitis (PBC), previously known as primary biliary cirrhosis, rarely occurs in UC. We present two PBC cases of 67 and 71 years who suffered from long-standing UC. Both patients were asymptomatic but they had increased cholestatic enzymes and high titres of antimitochondrial antibodies (AMA)-the laboratory hallmark of PBC. After careful exclusion of other causes of cholestasis by MRI/magnetic resonance cholangiopancreatogra…

Liver transplantation for cholestatic liver diseases: Trends in primary biliary cholangitis and primary sclerosing cholangitis in the US and Europe over two decades.

Cholestatic liver diseases [primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC)] are important indications for liver transplantation (LT). Adult LT recipients with PBC or PSC were identified from the European Liver Transplant Registry (ELTR) and U.S. Scientific Registry of Transplant Recipients (SRTR) (2000-2022). We assessed temporal trends in PBC/PSC prevalence among the LT population and used Cox models to identify variables associated with post-transplant mortality in PBC/PSC. A total of 22,034 LT recipients were included (PBC n=8774; PSC n=13,260). HCC was present i…

Antimitochondrial antibodies in rheumatic diseases: beyond primary biliary cholangitis.

Antimitochondrial antibodies (AMAs) targeting the E2 subunit of the pyruvate dehydrogenase complex are the serological hallmark of primary biliary cholangitis (PBC), but may also occur across systemic autoimmune rheumatic diseases. This narrative review integrates clinical and experimental evidence on AMAs in rheumatic diseases, delineating disease-specific patterns and clinical implications beyond the hepatobiliary context. AMAs and other mitochondrial autoantibodies have been reported in systemic sclerosis, systemic lupus erythematosus, rheumatoid arthritis, Sjögren disease, and inflammatory…

Characterization of a Northern German Primary Biliary Cholangitis Cohort.

BACKGROUND: Real-world data on risk stratification and clinical management of patients with primary biliary cholangitis (PBC) are limited. Identification of patients with increased risk for disease progression is, however, important, since they may benefit from more stringent treatment goals and second line therapy. METHODS: In this study, we retrospectively characterized a cohort of 535 PBC patients from a university hospital in northern Germany. We evaluated risk factors of disease progression, such as inadequate response to ursodeoxycholic acid (UDCA), younger age, vibration-controlled tra…

Endoscopic management of biliary stricture in primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) is a progressive fibroinflammatory disease characterized by multifocal biliary strictures, recurrent cholangitis, and a markedly increased lifetime risk of cholangiocarcinoma (CCA). Endoscopic retrograde cholangiopancreatography (ERCP) remains central to the diagnosis of CCA and management of PSC-related complications. This review synthesizes current evidence guiding the use of ERCP in patients with PSC, highlighting the importance of careful patient selection to mitigate adverse events. We review traditional ERCP techniques for the evaluation of dominant s…

Treatment Goals for Primary Biliary Cholangitis, an Australian Perspective.

Primary biliary cholangitis (PBC) is an autoimmune liver disease that targets the small intrahepatic bile ducts with diagnosis based on elevated serum alkaline phosphatase (ALP) levels and the presence of anti-mitochondrial antibodies. Untreated, it may run a progressive course to cirrhosis and hepatic decompensation, at which point liver transplantation is the only effective treatment. This review provides discussion of PBC treatment goals in the Australian context. People with PBC often experience a quality of life burden characterized by fatigue and itch. Extra-hepatic associations may incl…
