Acute acalculous cholecystitis is inflammation of the gallbladder in the absence of gallstones. It usually occurs in critically ill patients and is rare in the pediatric age group. We describe a 12-year-old boy who presented with fever, jaundice, and abdominal pain and was found to have acute acalculous cholecystitis, sacroiliitis, and pelvic osteomyelitis associated with bacteremia as a result of Staphylococcus aureus. Antibiotic therapy without surgical intervention was effective. A high index of suspicion is required to make an early diagnosis and institute appropriate treatment for childre…

Since 1997, laparoscopic cholecystectomy has been performed as one-day surgery (LC/DS) at our institution. Among the 122 patients enrolled in this program, 97 (80%) were successfully discharged within 24 hours after admission. Discharge was delayed for the other 25 patients, although 12 (48%) of them were discharged on postoperative day (POD) 2 or 3. This study not only verified the efficacy of LC/DS in shortening convalescence and allowing an early resumption of work but also confirmed the safety of LC/DS except in one patient with hemophilia A who required laparotomy for intraabdominal bleed

OBJECTIVE: To evaluate the role of laparoscopic cholecystectomy in acute cholecystitis and establish the outcomes of this treatment modality at North Oakland Medical Centers.
METHODS: This was a retrospective analysis over a three-year period (January 1, 1994 to December 31, 1996), performed at a University-affiliated urban teaching hospital, North Oakland Medical Centers, Pontiac, Michigan. Five hundred and fifty-seven patients underwent surgical treatment for gallbladder disease; 88 patients had acute cholecystitis, and 469 patients had chronic cholecystitis. Acute cholecystitis patients un…

Results are submitted of ultrasonic monitoring of gallbladder, pancreas and duodenum during the I, II, III phases of digestion in 186 patients who ranged from 30 to 60 years old. The form was studied of the gallbladder as was its size and location together with the volume, thickness of walls, presence of deformities; echogenicity of pancreas, outline, state of the main duct. Interdigestive duodenal motility and intracavitary pressure were recorded. Relationship has been established of the gallbladder and pancreas contraction phases to the condition of the intracavitary pressure in the duodenum

Emphysematous cholecystitis is a variant of acute cholecystitis characterized by the presence of gas in the gall bladder lumen, wall or pericholecystic tissues in the absence of an abnormal communication between the biliary system and the gastrointestinal tract. In the past, the diagnosis has relied on the plain abdominal radiograph (AXR), since there are no clinical features to separate this condition from simple acute cholecystitis. The apparently high mortality and morbidity associated with emphysematous cholecystitis has previously emphasized the importance of emergency cholecystectomy. We…

Hydrocephalus is a prevalent pediatric problem, and ventricular peritoneal shunting is the preferred procedure for surgical treatment. A system may become dysfunctional if the distal end of the catheter fails to drain because of intraabdominal adhesions, cerebral spinal fluid cysts, or peritonitis. Then additional revisions become necessary. Pediatric surgeons can come to the aid of their neurosurgical colleagues by placing the distal end of the catheter into the pleural space, cardiac atrium, ureter, stomach, or fallopian tube. These have all met with limited success. The ventricular cholecys

Acute acalculous cholecystitis (AAC) is a severe inflammatory disorder of the gallbladder. It occurs primarily in patients acutely ill from other disorders and is related to sepsis and shock. We previously found that platelet-activating factor (PAF), a phospholipid autacoid purported to be a mediator of the shock response, produced AAC. This study was performed to determine the effect of intravenous lipopolysaccharide (LPS) on feline gallbladders. Anesthetized cats underwent LPS administration with and without administration of a cyclooxygenase inhibitor and PAF antagonist. Gallbladder inflamm…

The author suggested chromofibroduodenoscopy for the recognition of cystic duct patency in patients with acute cholecystitis. The method has certain advantages over the accelerated chromatic duodenal catheterization. Approbation of the method in the clinic showed its high informativeness on the condition of the cystic duct, which is very important in choosing the surgical tactics in acute cholecystitis, in determining the indications for the operation in particular. Complications associated with the use of the method were not encountered.

Cholecystostomy and cholecystectomy remain appropriate and effective therapy for acute and chronic cholecystitis. Cholecystectomy is the gold standard against which all alternative methods of treatment of inflammatory biliary stone disease should be judged. The pathogenesis, diagnosis, and surgical treatment of acute and chronic cholecystitis have been described. Techniques of cholecystostomy, cholecystectomy, and intraoperative cholangiography used by the author have been given. Our results and those generally described in the literature indicate that the overall mortality rate for cholecyste…

Acute cholecystitis is a frequent consideration in patients presenting to the emergency department with the challenging complaint of upper abdominal pain. It is estimated that 20% of American adults have gallstones, and of these a large percentage (about one-third) will at some point develop acute cholecystitis. The epidemiology and associated risk factors of acute cholecystitis are briefly reviewed along with the pathogenesis and clinical presentation of the disease. Finally, an approach to the diagnosis in the emergency department and suggested management is discussed including a comparison …

In cases of acute cholecystitis, especially with cholecystolithiasis, the diagnosis is usually quickly made. In contrast, the symptoms in the presence of such complications as penetration, perforation and local abscess are often unspecific, and diagnostic procedures may be time-consuming, extending into weeks. One reason for this is the rareness of the entity. The same is true for emphysematous cholecystitis, although this presents with more acute symptoms. Computed tomography seems to be the most effective imaging method. Therefore, it should be used in cases of nonspecific problems in the up…

Ultrasound densitometry was used to determine echo density of the wall and contents of the gallbladder in normal persons and in patients suffering from cholelithiasis and chronic calculus-free cholecystitis. In patients with cholelithiasis, the gallbladder was later removed, followed by histological examination of its wall. The ultrasound and histological data were studied and compared. The highly significant differences in the echo density of the wall and contents of the gallbladder were obtained in normal persons and patients afflicted with chronic cholecystitis.

A retrospective analysis of 487 cases of deaths after operations for acute cholecystitis has revealed the dependence of lethal outcomes on two factors: degree of destructive alterations in the gallbladder and concomitant complications, 60% of them being pyo-septic processes. Conservative therapy during 2-3 days before operation failed to promote favourable outcomes. An alternative is urgent operation within 24 h after diagnosing acute cholecystitis.

Results of the ultrasonic examination (USE) in 511 patients with cholecystitis and biliurolithiasis are described. The authors point to possible use of USE on urgent indications for differential diagnosis of acute cholecystitis. The diagnostic signs of biliary and urinary stones are divided into the absolute and relative ones.

We describe an immunosuppressed patient with enteric cryptosporidiosis who developed combined cryptosporidial and cytomegaloviral hepatitis and cholecystitis as well as Enterobacter cloacae cholecystitis. To our knowledge, the presence of Cryptosporidium in a liver biopsy specimen has not previously been reported.
