Microfluidic devices for the isolation of circulating rare cells: a focus on affinity-based, dielectrophoresis, and hydrophoresis.

Circulating rare cells have attracted interest because they can be good indicators of various types of diseases. For example, enumeration of circulating tumor cells is used for cancer diagnosis and prognosis, while DNA analysis or enumeration of nucleated red blood cells is useful for prenatal diagnosis or hypoxic anemia, and that of circulating stem cells to diagnose cancer metastasis. Isolation of these cells and their downstream analyses can provide significant information such as the origin and characteristics of a disease. Novel approaches based on microfluidics have many advantages, incl

Liquid biopsy in genitourinary cancers: Diagnostic and prognostic implications.

Genitourinary neoplasms, including bladder, prostate, renal, and testicular cancers, represent 25% of all solid tumors worldwide. Great advances have been achieved in the last few decades in diagnostic and therapeutic modalities. Among these, liquid biopsy (LB) technology has evolved during the past few years and offers emerging and novel modalities in the field of oncology. LB is performed by withdrawing bodily fluids (i.e., blood or urine) and looking for circulating tumor DNA, circulating tumor cells, extracellular vesicles, and non-coding RNAs, among others. Over the past years, several te…

Diving into the Pleural Fluid: Liquid Biopsy for Metastatic Malignant Pleural Effusions.

Liquid biopsy is emerging as a promising non-invasive diagnostic tool for malignant pleural effusions (MPE) due to the low sensitivity of conventional pleural fluid (PF) cytological examination and the difficulty to obtain tissue biopsies, which are invasive and require procedural skills. Currently, liquid biopsy is increasingly being used for the detection of driver mutations in circulating tumor DNA (ctDNA) from plasma specimens to guide therapeutic interventions. Notably, malignant PF are richer than plasma in tumor-derived products with potential clinical usefulness, such as ctDNA, micro R…

Latest developments in urinary tumor DNA and its current clinical role in urothelial bladder cancer and upper tract urothelial carcinoma.

PURPOSE OF REVIEW: Urinary tumor DNA (utDNA) has emerged as a promising noninvasive biomarker that may complement or reduce the need for cystoscopies in the diagnosis, surveillance, and treatment monitoring of urothelial carcinoma. Unlike plasma circulating tumor DNA (ctDNA), which is heavily diluted by hematopoietic cell-derived DNA, utDNA originates directly from tumor cell shedding into urine and often appears at higher concentrations, improving molecular detection. While there is an increasing number of publications using utDNA with promising results, most of these studies were developed f…

Circulating tumor cell viability during and after radiotherapy mirrors treatment response in cancer patients.

The response to radiotherapy (RT) is influenced by the individual DNA repair capacity of both the tumor cells and the host. In this study, we assessed whether circulating tumor cell (CTC) enumeration, kinetics, and CTC viability (i.e., the apoptotic rate) could provide a more accurate tool for monitoring and stratifying the RT response. We analyzed CTCs and tumor-derived extracellular vesicles (tdEVs) during RT in 71 lung (n = 35) and breast (n = 36) cancer patients receiving RT treatment for brain (n = 54) and bone (n = 19) metastases. The DNA repair capacity of the host was assessed by ex vi

Neutrophil extracellular traps in the tumor microenvironment, metastasis, therapy, and beyond: advances, challenges, and perspectives.

Neutrophil extracellular traps (NETs), the DNA-protein structures released by neutrophils within the tumor microenvironment (TME), play a role in cancer that extends far beyond their traditional antimicrobial function. Specifically, NETs exert multifaceted and context-dependent effects on tumor progression, metastasis, immune regulation, and therapeutic response, showing their robust activity in shaping the complex landscape of tumor biology. This review systematically elucidates how NETs promote tumorigenesis by inducing DNA damage and epithelial-mesenchymal transition (EMT), and drive metast

Feasibility of IG and TCR rearrangements quantification in ctDNA for monitoring clinical response in pediatric lymphomas.

INTRODUCTION: Lymphomas are malignancies of the lymphatic system characterized by clonal rearrangements of immunoglobulin (Ig) and T-cell receptor (TCR) genes. While diagnosis relies on tissue biopsy, circulating tumor DNA (ctDNA) analysis offers a minimally invasive approach for real-time disease monitoring. This study evaluated next-generation sequencing (NGS) for the detection and quantification of Ig and TCR rearrangements in ctDNA from pediatric lymphoma patients. METHODS: Forty cases (21 Hodgkin and 19 non-Hodgkin lymphomas) were analyzed using NGS with BIOMED-2 and EuroClonality primer…

Epigenetic functions and biomarker potential of PIWI-interacting RNAs in oncogenesis.

BACKGROUND: Cancer remains a leading cause of global morbidity and mortality, underscoring the need for innovative diagnostic and prognostic biomarkers. PIWI-interacting RNAs (piRNAs) have emerged as critical regulators of gene expression and genomic stability, with increasing evidence linking their dysregulation to oncogenesis. piRNAs can modulate DNA methylation through interactions with DNA methyltransferases (DNMTs), contributing to tumor suppressor gene silencing and broader epigenetic reprogramming. However, the functional implications of piRNA-mediated methylation changes in cancer are

Circulating tumour DNA in oligometastatic oesophago-gastric cancers: applications, challenges, and future directions.

Oesophago-gastric cancer (OGC) is associated with a poor prognosis, with most patients presenting with or ultimately developing metastatic disease. Oligometastatic disease (OMD) represents an intermediate disease state characterised by a limited metastatic burden and, in selected patients, a more indolent clinical course. Increasing evidence suggests that carefully selected patients with OMD may benefit from local metastasis-directed therapies combined with systemic treatment. The OligoMetastatic Esophagogastric Cancer (OMEC) consortium has recently proposed a standardised consensus definition
