Successful closed reduction of a trans-scaphoid perilunate dislocation in a 11-year-old boy: a case report.

CASE: Perilunate dislocation is an uncommon injury in children. We report the case of an 11-year-old boy who presented a trans-scapho-perilunate fracture-dislocation of the carpus after falling from his bike. Treatment consisted of a closed reduction under general anesthesia followed by cast immobilization for 12 weeks. The injury healed with good wrist function on follow-up. CONCLUSION: Closed reduction should be attempted because it can be successful and allows for closed treatment with the cast. It provides good radiological healing and satisfying functional results.

[Surgical treatment strategies and their radiological potential for correction of proximal radius fractures in children and adolescents].

BACKGROUND AND OBJECTIVE: The isolated proximal radius fracture in children is a quite rare injury. In difference to adults a conservative treatment is often possible. But in case of increasing dislocation the indication for surgery is given. Options for an operative treatment are "closed reduction", "percutaneous reduction", "intramedullary nailing", "open reduction" with and without fixation. Aim of this study is to compare these procedures with each other. STUDY DESIGN AND METHODS: This was a retrospective investigation and 82 patients who underwent surgery after an isolated proximal radiu

Does Preoperative Manual Reduction (Omoto Technique) Affect Surgical Outcomes for Calcaneal Fractures? A Multicenter (TRON Group) Retrospective Study.

The Omoto technique is a well-known method that is commonly used for noninvasive manual repair of calcaneal fractures. However, there have been no detailed studies on its clinical outcomes in preoperative closed reduction for surgical cases. This multicenter retrospective study aimed to compare the clinical and radiographic outcomes of calcaneal fractures treated with and without the preoperative Omoto technique, assessing its effectiveness. We extracted 335 patients with calcaneal fracture who underwent surgery between 2015 and 2020 from our multicenter database, named TRON. We evaluated the

Reducing resource utilization during non-operative treatment of pediatric proximal humerus fractures.

INTRODUCTION: The majority of proximal humeral fractures in the skeletally immature are treated non-operatively. Operative indications vary but are largely based on degree of displacement. Non-union is rare. Non-operatively treated fractures are typically monitored radiographically to assess healing. HYPOTHESIS: We hypothesize that the decision to treat fractures operatively is made at the time of first imaging and that follow-up X-rays do not lead to a change in management. MATERIAL AND METHODS: We retrospectively reviewed the records of 239 patients treated for proximal humerus fractures o

Comparison of Open and Closed Nailing for Femoral Shaft Fractures: A Retrospective Analysis.

Introduction Open and closed nailing are the two reduction methods used for the fixation of femoral shaft fractures. The study aims to assess the clinical and functional outcomes of open and closed nailing for closed femoral shaft fractures. Methodology A total of 398 patients who underwent intramedullary nailing fixation of nonpathological femoral shaft fracture between January 2016 to December 2019 were reviewed retrospectively. Two hundred seventy-four underwent closed nailing, and 124 were considered for open nailing. Results The primary outcome reviewed was the union rate of fracture. Oth

Operative Treatment Versus Spica Casting for Treatment of Pediatric Femoral Shaft Fractures in Children Aged 4 to 6 Years: A Large Multi-Institutional Age-Stratified Comparative Analysis.

BACKGROUND: Femoral shaft fractures in children aged 4 to 6 years represent a clinical gray zone between closed reduction with casting and operative treatment. Traditional guidance favors closed treatment under age 6, yet recent reports suggest increasing use of operative treatment despite limited evidence. This study characterized national treatment rates from 2013 to 2024 and compared health care utilization outcomes. METHODS: This retrospective cohort study used the TriNetX Research Network, a federated platform containing nationwide deidentified health records. Patients from 2013 to 2024

Immediate microcirculatory derangements in skeletal muscle and periosteum after closed tibial fracture.

BACKGROUND: Severe musculoskeletal soft tissue injury sustained after a closed fracture to the extremities significantly influences bone healing and determines the patient's prognosis. The present study was aimed at quantitatively assessing immediate microcirculatory changes in skeletal muscle and periosteum after standardized closed fracture. METHODS: Standardized closed fracture of the left tibia in isoflurane-anesthetized Sprague-Dawley rats (n = 14) was induced using a modified weight-drop technique. The left extensor digitorum longus (EDL) muscle (n = 7) and tibial periosteum (n = 7) wer

Irreducible tarsometatarsal fracture-dislocation.

Three mechanisms may prevent closed manipulative reduction of the tarsometatarsal fracture-dislocation: anterior tibialis tendon interposition, incongruity of the medial cuneiform-first metatarsal articulation, and interposition of a fracture fragment in the second metatarsal-intermediate cuneiform joint. Awareness of these mechanisms will assist in the surgical approach to athe irreducible tarsometatarsal fracture dislocation. Such an irreducible fracture-dislocation in a 25-year-old male was successfully reduced openly: anatomic reduction and normal passive range of ankle motion resulted. Th

Epiphyseal fractures of the proximal tibia.

Fractures of the proximal tibial epiphysis and apophysis are rare. Data of ten patients seen in the last 17 yr with an epiphyseal (6) or apophyseal (4) fracture of the proximal tibia were reviewed. Three patients with an epiphyseal fracture were treated by open reduction and fixation, the other three by closed reduction. All apophyseal fractures were treated by open reduction and internal fixation. No major complications occurred. The final results were good.

Subtrochanteric fracture: the effect of cerclage wire on fracture reduction and outcome.

INTRODUCTION: Subtrochanteric neck of femur fractures are a challenge to treat due to anatomical and biomechanical factors. Poor reduction, varus deformity, nonunion and return to theatre risks are high. A cerclage wire can augment an intramedullary nail to help fracture reduction and construct stability. Concerns exist regarding the use of cerclage wire on fracture zone vascularity. The aim of this study was to assess the benefits and adverse outcomes associated with the use of cerclage wiring. PATIENTS AND METHODS: A 7-year retrospective review of all subtrochanteric fractures at a Level 1

Internal fixation of malleolar fractures using the A.O. tension band wiring techniques.

The principle of tension band wiring in open reduction and internal fixation of malleolar fractures is presented in three case reports. A brief history of the classification of ankle fractures is given so that the mechanism of injury can be understood. This must be known when attempts at closed or open reduction and immobilization are done. If closed reduction fails to bring about good anatomical reduction and alignment, the fracture must be openly reduced and internally fixated. The chief indications for tension band wiring are given, and their application in treating ankle fractures.

Iatrogenic Metacarpal Shaft Fracture Following Distal Radius External Fixator Removal in Clinic.

BACKGROUND: External fixation remains an accepted option for certain unstable distal radius fractures, particularly when ligamentotaxis and percutaneous augmentation can restore and maintain alignment. Known complications include pin-tract infection, loss of reduction, nerve irritation, stiffness, and complex regional pain syndrome. Fracture through a metacarpal Schanz pin site during in-clinic removal has not, to our knowledge, been specifically described. CASE PRESENTATION: A 70-year-old right-hand-dominant female sustained a shortened, dorsally angulated, extra-articular fracture of the le

Bennett's fracture: Comparative study between open and closed surgical techniques.

The aim of this study was to assess long-term outcomes of surgically treated Bennett fractures, while comparing open reduction and internal fixation (ORIF) with closed reduction and percutaneous fixation (CRPF). Patients treated between 1994 and 2010 were assessed retrospectively during an outpatient visit using a validated questionnaire (i.e. DASH, pain assessed through VAS), sensory testing, grip- and pinch-strength and radiographic analysis for post-traumatic arthritis. Fifty patients were included. Mean follow-up was 10 years. Mean age at trauma was 34 years. ORIF was used in 35 patients.

Brachial artery injury in closed posterior elbow dislocation case report.

The authors describe a case with a closed posterior elbow dislocation associated with a distal radial fracture and complete transsection of the brachial artery. The patient had a pulseless distal upper extremity and immediate gross swelling of the elbow and forearm. As closed reduction was not possible, open reduction had to be performed through an anteromedial approach to the elbow. End-to-end suture of the brachial artery was successful. After fasciotomy and internal fixation of the distal radial fracture, the elbow was stabilized with an external fixator spanning the elbow joint. After two

The adult Tillaux fracture: one not to miss.

This case is a rare adult type Tillaux fracture. This fracture should be diagnosed in the emergency department. The fracture requires careful evaluation with awareness of associated injury. Standard radiological views (antero-posterior and lateral) of the ankle may not clearly show the fracture displacement hence, an oblique view is required. The fracture should be managed by the emergency physician, if the displacement is less than 2 mm, by immobilising with a non-weight bearing cast or ankle braces for 6 weeks. If the fracture fragment is displaced more than 2 mm, it should be treated by clo

Reverse obliquity femoral neck fractures: two case reports with a literature review.

INTRODUCTION: Vertical femoral neck fractures-those with a Pauwels angle >70°-are an especially demanding subset, notorious for their recalcitrance to fixation and high risk of non-union. We propose a previously unrecognized variant of femoral neck fracture in which the fracture plane is vertical (Pauwels ≥ 90°) and lacks any Gotfried-positive cortical contact, a configuration we designate "reverse-obliquity femoral neck fracture" (ROFNF). This report describes two cases of ROFNF and their respective therapeutic strategies. CASE REPORT: The index patient was a 56-year-old woman who sustained

A quantitative computed tomography-based finite element analysis of iatrogenic fracture during shoulder dislocation reduction: biomechanical mechanism and efficacy of prophylactic K-wire fixation.

BACKGROUND: Iatrogenic fracture during closed reduction of anterior shoulder dislocation with greater tuberosity fracture is a serious complication. This study aimed to quantify its biomechanical risk and evaluate prophylactic Kirschner wire (K-wire) fixation. METHODS: We retrospectively analyzed 126 patients (age ≥ 50 years). A patient-specific finite element model of the proximal humerus was created from quantitative computed tomography (QCT) data. Parametric models with different greater tuberosity defect sizes (0-40%) were loaded under pure axial traction (300N) or combined (axial 300 N +

Incarceration of the intermeniscal ligament in tibial eminence injury: a block to closed reduction identified using MRI.

Tibial eminence fractures are a relatively uncommon injury, but most frequently occur in children and adolescents with open physes. Entrapment of the intermeniscal ligament or the meniscus itself can occur in the setting of a tibial eminence fracture and when present, poses a significant obstacle to successful closed or surgical reduction. Identification of these entrapped structures on preoperative imaging may be helpful to the physicians caring for these patients and assist with preoperative planning. We present a case of a 13-year-old male who sustained a tibial eminence fracture while play
