A guideline-based decision support system for headache diagnosis.

The ratio of correct diagnosis for primary headache has always been relatively low for general practitioners due to the unacquaintance with … uracy of the system, the result is: migraine 144/153 (94.1%), tension-type headache 89/100 (89.0%), cluster headache 10/11 (90.9%) and chronic daily headache 53/57(93.0%). The proposed system is in the starting phase of the implementation at the outpatient department of Neurology in Chinese PLA general hospital.

Stabbing headache in patients with autoimmune disorders.

Stabbing headache is a relatively rare type of chronic "primary" headache with distinctive features with respect to more common forms of paroxysmal headache, such as cluster headache and trigeminal neuralgia. Drug treatment is empirical because of the lack of knowledge on the pathophysiology of stabbing headache. We examined 26 patients recruited over 10 years, who met the diagnostic criteria for stabbing headache. Interestingly, more than half of these patients had autoimmune disorders, including multiple sclerosis, Sjögren's disease, Systemic Lupus Erythematosus, Behçet's disease, autoimmune…

Cerebrovascular reactivity in migraine: a breath-hold BOLD fMRI study.

BACKGROUND AND OBJECTIVES: Prior studies assessing cerebrovascular reactivity (CVR) in participants with migraine primarily utilized Doppler ultrasonography and focused on large vascular territories due to limitations of available technology. Newer functional MRI techniques detect alterations in CVR at much higher spatial resolution and, when coupled with an anatomical scan, allow correlation with specific brain regions. The goal of this study was to measure and compare CVR in participants with migraine during (ictal) versus between (interictal) spontaneous attacks using functional magnetic re

[Headaches and multiple sclerosis].

The article discusses the possible association between multiple sclerosis (MS) and headache. MS has a broad range of neurological symptoms, but headache is not included among them. Research on the link between MS and headache focuses on primary headaches such as migraine, tension-type headache and cluster headache. Studies on the possible association between MS and migraine have had conflicting results and have found a wide range of prevalence rates for migraine in MS patients. The possible mechanisms proposed linking migraine and MS can be unidirectional, bidirectional or involving a common c…

Headache management for the pain specialist.

Headache is a common symptom caused by a wide variety of diseases. Primary headaches include migraine, cluster headache, tension-type headache, and other less common diseases. It is important to differentiate these headaches from secondary headaches caused by vascular, neoplastic, infectious, metabolic, or toxic disorders. Most primary headaches have a genetic basis, with environmental factors acting as triggers. Recent advances in basic research resulted in the development of more specific and effective therapies. Medication-overuse headache is a very common cause of chronic daily headache. D…

Redefining primary headaches.

In the light of the pathophysiologic knowledge acquired in the recent years, a tentative redefinition is now possible of some types of headache until now defined as idiopathic, and indistinctly described as primary headaches. Cluster headache and trigeminal neuralgia are known examples of diseases classified as primary, which are, in contrast, well-defined diseases to be distinguished from headaches without any recognized anatomic site of lesion or pathogenesis. Another still debated condition, chronic migraine, is proposed here as the consequence of "processes" to be ascribed to mechanisms ac…

Approach to acute headache in adults.

Approximately one-half of the adult population worldwide is affected by a headache disorder. The International Headache Society classification and diagnostic criteria can help physicians differentiate primary headaches (e.g., tension, migraine, cluster) from secondary headaches (e.g., those caused by infection or vascular disease). A thorough history and physical examination, and an understanding of the typical features of primary headaches, can reduce the need for neuroimaging, lumbar puncture, or other studies. Some red flag signs and symptoms identified in the history or during a physical e

Uncommon headache syndromes in the pediatric population.

Headache is one of the most common symptoms in children and adolescents, and headache syndromes are an important reason for medical consulting. According to the second edition of the International Classification of Headache Disorders, there are 196 possible headache diagnoses, of which 113 have been described in pediatric population. Herein, we focus on unusual pediatric headache syndromes. We group them as headaches with migraine features, short-duration headaches with autonomic features, short-duration headaches without autonomic features, and potentially ominous forms of headaches. Although

[Determining the diagnosis from the pain pattern. Brief and stabbing or chronic and dull?].

For the neurological differential diagnosis of facial pain, symptomatic pain must be differentiated from the so-called primary pain syndromes. Trigeminal neuralgia is usually readily diagnosed on the basis of the typical history. The treatment of choice is carbamazepine. If this fails, invasive options are available. Atypical facial pain should be diagnosed only when all known primary and secondary pain syndromes have been excluded. Treatment is difficult and comprises the administration of tricyclic antidepressants. Cluster headache and chronic paroxysmal hemicrania each has an unmistakable t…

[Beta-endorphin levels in patients with pain syndromes in the areas of the face and head].

Blood plasma beta-endorphin concentrations were measured in 87 patients with various facial and head pain syndromes: trigeminal neuralgia or neuropathy Horner syndrome and migraine, facial autonomic pains. beta-endorphin concentrations were measured before and after treatment. In the groups under investigation, the neuropeptide showed opposite changes in plasma levels after the therapy depending on the type of the syndrome.

In 43 patients with migraine, 20 with Horton's headaches, 13 with trigeminal neuralgia and 50 healthy subjects the circulating platelet aggregates were determined by the method of Wu and Hoak. Moreover, in 23 patients with migraine, 8 with Horton's headache and 21 healthy subjects spontaneous platelet aggregation was determined in platelet-rich plasma with a transmission aggregometer. It was found that the aggregation coefficient was much lower in patients with migraine and Horton's headache than in the control group. In the group with migraine the per cent of spontaneous aggregation in 8 pati

Prevalence of primary headaches in people with multiple sclerosis.

The aim was to investigate the lifetime prevalence of headache and primary headache (diagnoses according to International Headache Society criteria) in multiple sclerosis (MS). The relationships between headache and clinical features of MS and MS therapy were also investigated. We studied 137 patients with clinically definite MS; 88 reported headache, 21 of whom developed headache after the initiation of interferon. The prevalence of all headaches in the remaining 116 patients was 57.7%. Migraine was found in 25.0%, tension-type headache in 31.9%, and cluster headache in one patient. A signifi…

[A case of pericarotid syndrome with retention cyst in ethmoid sinus].

A 42-year-old man noted pain on the left side of his forehead and left ptosis. On examination, he showed conjunctival hyperemia, ptosis and miosis in the left side, as well as hyperesthesia in the first branch of left trigeminal nerve. An MRI of his brain showed a retension cyst in the left ethmoid sinus. There was neither abnormalities in the parasellar lesion nor in the neck. We diagnosed him with pericarotid syndrome rather than cluster headache or Raeder syndrome. Five cases who had paranasal sinus lesions as a cause of cluster headache or Raeder syndrome have been reported. More cases are…

Neuromodulation for Chronic Daily Headache.

PURPOSE OF REVIEW: We reviewed the literature that explored the use of central and peripheral neuromodulation techniques for chronic daily headache (CDH) treatment. RECENT FINDINGS: Although the more invasive deep brain stimulation (DBS) is effective in chronic cluster headache (CCH), it should be reserved for extremely difficult-to-treat patients. Percutaneous occipital nerve stimulation has shown similar efficacy to DBS and is less risky in both CCH and chronic migraine (CM). Non-invasive transcutaneous vagus nerve stimulation is a promising add-on treatment for CCH but not for CM. Transcut…

Neuromodulation in treatment of refractory headaches.

The field of neuromodulation is emerging as a promising and alternative therapeutical option for many drug-resistant clinical conditions, including painful syndromes such as refractory chronic cluster headache (CCH) and trigeminal neuralgia. We here report a series of patients who have undergone Deep Brain Stimulation (DBS) of Posterior Hypothalamus for chronic cluster headache, trigeminal neuralgia and atypical facial pain, matching their corresponding clinical results and also suggesting a role for Great Occipital Nerve Stimulation (which is a much less invasive procedure) in the treatment o…

The Comorbidity Burdens of Osteoarthritis and 174 Diseases, 1990-2021: A Modeling Study Based on the Global Burden of Disease Study 2021.

AIM: Using data from the Global Burden of Disease (GBD) Study 2021, this study aims to estimate the comorbidity burdens of osteoarthritis (OA) at global, regional, and national levels from 1990 to 2021. METHODS: The age-standardized prevalence and years lived with disability (YLD) rate for OA and 174 other Level-3 diseases from 204 countries and territories were extracted. A modified formula was employed to determine the comorbidity burdens for the co-occurrence of OA and one of the other 174 diseases. Cluster analysis was performed to identify different patterns in the comorbidity burden. R
