We report the results of a prospective study of the surgical release of 45 knee flexion contractures in 28 patients with myelomeningocele. The neurosegmental level was thoracic in ten patients, L1/2 in one, L3/4 in 11, and L5/S1 in six. In walkers the indication for surgery was a fixed flexion contracture impeding walking, and in non-walking patients it was a flexion contracture impeding transfers or sitting balance, or likely to do so with increasing deformity. We present a newborn infant with paraplegia and contractures of the lower limbs, consistent with neurologic injury rather than malformation. The mother was involved in a severe motor vehicle accident during the sixth month of pregnancy. We propose that this infant's injuries are a result of that accident.

Children with CMC present with blue sclerae, megalocornea, hypoplastic and translucent irides, miosis and high myopia. The lenses may be dislocated as in familial Marfan syndrome but they are often in place and microspherophakic. The clinical history of a boy with CMC is presented. Pathological examination of the eyes showed megalophthalmos with thinned sclera, anomalies of the chamber angle and iris, ill-developed ciliary body and choroid and a small in situ lens.

The Ilizarov fixator was used to correct chronic burn deformities of the foot and ankle. These contractures can be classified as "simple" or "complex" based on associated deformities and musculoskeletal function. The simple, undirectional deformities of equinus, cavus, rockerbottom, and toe dislocations are fairly easy to correct and maintain. Correction and maintenance of complex deformities with varus or valgus angulation, bone abnormality, or muscle loss are more difficult.

Congenital Contractural Arachnodactyly is an inherited disorder of connective tissue characterised by congenital contractures, arachnodactyly, marfanoid habitus, crumpled auricles and in some kindreds, progressive kyphoscoliosis. An extensive kindred with many affected members is described. The importance of the early recognition of this syndrome is highlighted.

A newborn boy with congenital contractural arachnodactyly (CCA) was found to have an atrial septal defect, ventricular septal defect, patent ductus arteriosus, and aortic arch anomalies. These resulted in congestive failure but subsequent progressive dilatation of both great arteries and development of aortic regurgitation were associated with eventual cardiorespiratory failure and death at 11.5 months. The purpose of this study was to assess the effectiveness of serial casting of burn contractures that were resistant to traditional methods of treatment such as paraffin therapy, massage, exercise, and splinting. Serial casting was used to increase the range of motion in 35 joints in 15 patients with burns. A mean increase of 54% was achieved. Casting provided immediate results with minimal complications and was accepted well by patients. Casts were easy to apply and effective even with noncompliant patients. They also delayed or eliminated the need for surgical correction.

The characteristics of transient contractures elicited by rapid cooling of frog or mouse muscles perfused in vitro with solutions equilibrated with 0.5-2.0% halothane are reviewed. In a study of 350 patients with multiple congenital contractures (arthrogryposis), 80 (23%) patients had mental retardation or were developmentally delayed. Out of that group of 80 patients, 13 (16%) were found to have abnormal karyotypes. Based on the position of the thumb metacarpal, 102 burned thumb contractures in children were classified into four categories: adduction, opposition, extension and flexion. Rater reliability characteristics of four positioning techniques used to measure hip extension (prone hip extension, Thomas, Mundale, and pelvifemoral angle) were compared. Two raters examined 45 children (90 hips), including patients with spastic diplegia and with meningomyelocele, who are prone to developing hip flexion contractures, and healthy subjects. Digital contractures caused by Dupuytren's disease can best be treated by open fasciotomy plus excision of all overlying involved skin and replacement by full-thickness skin grafts. Such a procedure results in minimal operative morbidity, early recovery of function, and low incidence of recurrent contractures.

One-hundred and forty-six postburn elbow contractures in children were classified into four categories--simple band, complex band (crosses shoulder and/or wrist joint), diffuse scar, and limited scar--to assess the results of surgical treatment. Although dantrolene reverses the muscle contracture seen during a malignant hyperpyrexia (MH) crisis, its site of action is not known. It has been inferred from previous work that the major abnormality in MH is in the sarcoplasmic reticulum, and that dantrolene must act on this organelle. Five children with popliteal webbing and flexion contractures of the knees are described. When their associated anatomical abnormalities are considered, classification into two distinct syndromes, multiple pterygium and popliteal pterygium, is possible and of importance in genetic counseling. Soft tissue surgical corrections of the flexion contractures were undertaken in each case. The operative technique, local abnormalities, and orthopaedic management are discussed.

We report our experience in the treatment of 39 patients with Dupuytrens' contracture using the "open-palm" technique (McCASH). In this method the palmar fascia is excised through a transverse incision in the distal plamer crease. The wound is left open. The advantages of this method are: no necrosis of the skin, absence of haematomas, closure of other incisions without tension, early motion with little postoperative discomfort. Based on our experience we recommend this method for a selected group of patients.
