Generalized convulsive status epilepticus in Creutzfeldt-Jakob disease.

We report a patient who was referred to our department because of generalized status epilepticus. His condition deteriorated rapidly and he died 1 month after admission. Autopsy confirmed the clinical diagnosis of Creutzfeldt-Jakob disease (CJD). CJD should be added to the list of rare but possible causes of generalized status epilepticus.

[Pathogenic factors of convulsion in neonates with hypoxic-ischemic encephalopathy].

OBJECTIVE: To explore the pathogenic factors of convulsion in neonates with mild to severe hypoxic-ischemic encephalopathy. METHODS: Altogether 70 neonates with mild to severe hypoxic-ischemic encephalopathy (HIE) were admitted into our hospital between Jan. 1988 and Jan. 2001. Serum calcium was dynamically monitored in all the patients, with cerebral ultrasonography performed. RESULTS: Convulsions occurred within the first and second days after birth in the neonates of convulsion group, whose serum calcium levels were significantly decreased during the third to sixth days in comparison with

[Concussive convulsions in the differential diagnosis of post-traumatic epilepsy].

INTRODUCTION: Concussive convulsions are motor manifestations in acute head injury. This clinical phenomenon should be distinguished from epileptic seizures. We present two young men with motor and convulsive manifestations in acute head injury. Patient 1. A 18-year old basketball player fell on the parquet during a game. Initially he was struck on the right shoulder which caused brief and vigorous twitch of the head towards the ground and additional temporal impact. At the moment of impact he lost consciousness and developed tonic leg and arm posturing with both clenched fists. His legs were

Convulsions and retinal haemorrhage: should we look further?

BACKGROUND AND AIMS: The prevalence of retinal haemorrhages after convulsions is not well established. As these haemorrhages are considered characteristic of child abuse, we investigated their occurrence after convulsive episodes to see whether the finding of haemorrhage should prompt further investigation. METHODS: Prospective study of 153 children (aged 2 months to 2 years), seen in the emergency department after a convulsive episode. After a thorough history and physical examination, a retinal examination was performed by an ophthalmologist. If findings were positive, further investigation

Idiopathic familial temporal lobe epilepsy with febrile convulsions.

Efforts to duplicate the genetic and molecular breakthroughs of autosomal dominant frontal lobe epilepsy have lead to increased interest in familial temporal lobe epilepsy. In this report we describe three kindreds. The epilepsy syndrome described manifests after the teenage years and was generally mild and treatment responsive. The predominant seizure types were simple and complex partial seizures, typical of mesial temporal onset. Some family members had febrile convulsions only and others had epilepsy without preceding febrile convulsions. Three patients had both febrile convulsions and tem

The use of antipyretic medications in the prevention of febrile convulsions in children.

Febrile convulsions are a relatively common outcome in paediatric febrile illness, although it is not known why some children suffer these. Antipyretic medications may form the basis for some treatment regimens, although they are not recommended in published guidelines. There is little evidence that the prophylactic use of antipyretics has any effect in reducing the incidence of febrile convulsions. Consequently, educational interventions aimed at reducing parental fear and helping them to care for their children during febrile illnesses may be more efficacious.

Can convulsions alone cause retinal haemorrhages in infants?

AIM: To evaluate the likelihood that, in children under the age of 2 years, convulsions alone may cause retinal haemorrhages. METHODS: Children under the age of 2 years admitted to hospital following convulsions, were examined within 48 hours of admission. The convulsions were classified by a paediatric neurologist and detailed ocular examination, including indirect ophthalmoscopy, was performed by an ophthalmologist. Statistical analysis was undertaken using Hanley's rule of three. RESULTS: 32 consecutive children admitted with convulsions were examined; 10 of them were admitted following e

Seizure risk in offspring of individuals with a history of febrile convulsions.

UNLABELLED: One-hundred and seventy-nine offspring of 120 probands with a history of febrile convulsions (FC) were studied to determine the risk of seizures and possible factors influencing this risk. The conditions for this study were especially good since all of the probands had undergone clinical and EEG examinations as well as an assessment of family history of seizures during childhood. Hence, for the first time the seizure status of the probands' parents could be included in the calculation of risk in offspring. In sibs the risk was highest if the mother of the proband had experienced se

Neurotoxic convulsions induced by theophylline and its metabolites in mice.

To evaluate the risk of neurotoxicity induced by theophylline and its main metabolites, 1-methylxanthine (1-MX), 3-methylxanthine (3-MX), 1,3-dimethyluric acid (1,3-DMUA) and 1-methyluric acid (1-MUA), we compared their convulsive potency to central nervous system (CNS) after intracerebral administration to mice. All compounds studied induced clonic convulsion in a dose-dependent manner, and the ED50 values for convulsion were 490, 546, 1107, 360 and 620 nmol/kg for theophylline, 1-MX, 3-MX, 1,3-DMUA and 1-MUA, respectively. These compounds were also administered intravenously to mice by const

Value of brain CT scan in children with febrile convulsions.

Studies specifically dealing with the value of brain CT scans in different types of febrile convulsion are lacking when compared to the large amount of studies on other favorable convulsive disorders. This study is correlation study between children who presented with febrile convulsions (n = 38) and the results of their brain CT scans. Twenty four patients had risk factors for subsequent epilepsy (group A), 3 of them had abnormal CT scans in the form of mild diffuse brain atrophy, and all 3 patients were found to have prior neurodevelopmental deficits. The other 14 patients did not have such

Serum sodium levels and probability of recurrent febrile convulsions.

UNLABELLED: In a prospective study of 69 children with febrile convulsions, serum sodium levels were often lower than normal (52% had levels < 135 mmol/l). The mean level (134.4 +/- 0.4 mmol/l) was significantly lower as compared to a group of children without fever (140.6 +/- 0.4 mmol/l, n = 23) and as compared to a group with fever but without convulsions (137.6 +/- 0.6 mmol/l, n = 31). The probability of a repeat convulsion within the same febrile period appeared to be significantly related to the serum sodium level. CONCLUSION: Measurement of the serum sodium is a valuable investigation i

A post-thyroidectomy convulsion: an unusual presentation of chronic hypoparathyroidism.

A 59-year-old woman with previously undiagnosed hypoparathyroidism presented with a tonic-clonic seizure 38 years after thyroidectomy. This case is unusual because of the initial presentation, but also unique because it is the longest latency period between surgery and presentation in recent literature.

Convulsions after enflurane in a schizophrenic patient receiving neuroleptics.

A schizophrenic patient suffered from an episode of unexpected grand mal seizure following an enflurane unaesthetic for biopsy of an orbital lesion. The seizure was brief and subsided spontaneously. An assessment of the anaesthetic technique and a thorough neurological examination which included a CT scan and an EEG, failed to demonstrate any obvious cause for the convulsion. The patient was not an epileptic and was receiving neuroleptic drugs preoperatively for the treatment of schizophrenia. A synergistic role of enflurane and neuroleptic drugs is producing seizure activity in this patients

Clinical study of epileptic children with history of febrile convulsion.

BACKGROUND: Clinically, some epileptic syndromes have been linked to febrile convulsions (FC), but the exact relationship between FC and later epilepsy remains a mystery. METHODS: Detailed histories of 81 Chinese children among 1950 pediatric epileptics with a history of FC were obtained retrospectively. The clinical courses and risk factors were analyzed. RESULTS: According to their epileptic patterns, the children were divided into a generalized group (Group G, 45/81), a partial group (Group P, 30/81) and those with severe myoclonic epilepsy in infancy (6/81). Patients of Group P had an ea

[5th day convulsions of the newborn infant in rotavirus infections].

AIM OF THE STUDY: The pathogenesis of fifth day fits in newborns has not yet been explained. From 1989-1991 we observed an epidemic occurrence of 21 cases in 1917 hospitalized newborn children. After finding rotavirus in the faeces of several cases we suspected a coincidence. METHODS: We tested the faeces of 19 fifth day fitters as well as 30 healthy newborns (4.-6. day of life) for rotavirus infection. The results of all clinically indicated rotavirus examinations in neonates of 4 to 6 days of age from 1989-1991 were analysed. In 6 rotavirus positive fifth day fitters we performed an immunol
