An integrated care pathway improves the management of paracetamol poisoning.

BACKGROUND: Paracetamol poisoning remains a major cause of morbidity and mortality. Clinical care of paracetamol poisoning depends on a range of patient variables and typically involves both medical and nursing care. An integrated care pathway (ICP) is a multidisciplinary management plan that incorporates guidelines and best practice to enhance care and documentation for a specific patient group. Paracetamol overdose is thus amenable to an ICP. AIM: To evaluate the introduction of an ICP on process of care of the paracetamol poisoned patient. METHODS: A retrospective case note review of cons

Ezrin Orchestrates Signal Transduction in Airway Cells.

Ezrin is a critical structural protein that organizes receptor complexes and orchestrates their signal transduction. In this study, we review the ezrin-meditated regulation of critical receptor complexes, including the epidermal growth factor receptor (EGFR), CD44, vascular cell adhesion molecule (VCAM), and the deleted in colorectal cancer (DCC) receptor. We also analyze the ezrin-meditated regulation of critical pathways associated with asthma, such as the RhoA, Rho-associated protein kinase (ROCK), and protein kinase A (cAMP/PKA) pathways. Mounting evidence suggests that ezrin plays a role …

Hospital protocols and evidence-based therapies: the importance of integrating aldosterone blockade into the management of patients with post-acute myocardial infarction heart failure.

Left ventricular systolic dysfunction (LVSD) and clinical heart failure are common complications of acute myocardial infarction (AMI) and result in substantially increased mortality and morbidity. Evidence-based cardiovascular protective therapies, including angiotensin-converting enzyme inhibitors, beta blockers, antiplatelet agents, and lipid-lowering medications, improve outcomes for these patients. However, this population is significantly undertreated with these guideline-recommended agents. Critical pathways have been demonstrated to improve the quality and consistency of treatment; as s…

Critical genes in human photoaged skin identified using weighted gene co-expression network analysis.

Photoaging is unique to the skin and is accompanied by an increased risk of tumors. To explore the transcriptomic regulatory mechanism of skin photoaging, the epidermis, and dermis of 16 healthy donors (eight exposed and eight non-exposed) were surgically excised and detected using total RNA-Seq. Weighted gene co-expression network analysis (WGCNA) identified the most relevant modules with exposure. The hub genes were identified using correlation, p-value, and enrichment analysis. The critical genes were identified using Support Vector Machine-Recursive Feature Elimination (SVM-RFE) and least

Clinical impact of implementing an orthogeriatric nurse in the care pathway for elderly patients with hip fragility fractures.

INTRODUCTION: Fragility hip fractures (HF) in older adults are associated with substantial morbidity, mortality, and functional decline. In Spain, evidence regarding the contribution of orthogeriatric nursing (OGN) roles within multidisciplinary teams remains limited. This study evaluated clinical outcomes associated with the implementation of an orthogeriatric nurse within a hip fracture clinical care pathway. METHODS: A retrospective cohort study was conducted including patients aged ≥ 75 years admitted with fragility hip fracture between 2017-2019 (pre-OGN period) and 2022-2023 (post-OGN p

Benchmarking Cervical Cancer Care Pathways-Measuring Timelines and Identifying Factors Associated With Delay for Women Undergoing Definitive Radiotherapy in New South Wales, Australia.

BACKGROUND: Diagnosis and management of cervical cancer require multiple investigations and specialist reviews, with delays leading to increased mortality and morbidity. Australia's Optimal Care Pathways (OCPs) recommends that patients with suspected diagnosis of cervical cancer should have their diagnostic processes completed within certain timeframes. Despite these guidelines, there is currently no benchmarking of cervical cancer timelines. AIMS: This study aims to benchmark cervical cancer care and identify factors associated with delay. MATERIALS AND METHODS: Patients who underwent radio

A cluster randomized controlled trial of an electronic medical record-based pathway for pediatric asthma in primary care in Alberta.

BACKGROUND: Primary care physicians, who manage the care of most children with asthma, often do not optimally assess disease control, prescribe asthma controller medications, or provide family asthma education. We developed a pediatric asthma clinical pathway embedded in an electronic medical record (EMR) for use by primary care practices, and we sought to evaluate its effects on prescription and use of asthma controller medication for children with asthma. METHODS: We conducted a cluster randomized controlled trial, enrolling primary care practices in Alberta that used Wolf or Med Access EMR

Prostate cancer diagnostic pathway in men with lower urinary tract symptoms or performing opportunistic screening: The Italian Society of Urology (SIU) position paper.

BACKGROUND: Voluntary PCa screening frequently results in excessive use of unnecessary diagnostic tests and an increasing risk of detection of indolent PCa and unaffordable costs for the various national health systems. In this scenario, the Italian Society of Urology (Società Italiana di Urologia, SIU) proposes an organized flow chart guiding physicians to improve early diagnosis of significant PCa avoiding unnecessary diagnostic tests and prostate biopsy. METHODS: According to available evidence and international guidelines [i.e., European Association of Urology (EAU), American Association

A simplified diagnostic pathway for the differential diagnosis of iron deficiency anaemia and anaemia of chronic disease.

INTRODUCTION: Iron deficiency anaemia (IDA) and anaemia of chronic disease (ACD) are common causes of anaemia with similar clinical and laboratory features. IDA is caused by low iron stores while ACD is due to iron-restricted erythropoiesis occurring in inflammatory states. Differential diagnosis requires analysis of multiple biochemical and haematological parameters. IDA can occur simultaneously to ACD (mixed aetiology). It is essential that true iron deficiency is identified, as these patients will require iron therapy. This preliminary study investigated whether hepcidin, the master regulat

A discrete event simulation model to evaluate the treatment pathways of patients with cataract in the United Kingdom.

BACKGROUND: The number of people affected by cataract in the United Kingdom (UK) is growing rapidly due to ageing population. As the only way to treat cataract is through surgery, there is a high demand for this type of surgery and figures indicate that it is the most performed type of surgery in the UK. The National Health Service (NHS), which provides free of charge care in the UK, is under huge financial pressure due to budget austerity in the last decade. As the number of people affected by the disease is expected to grow significantly in coming years, the aim of this study is to evaluate
