Meaningful assessment of a treatment modality for osteonecrosis (ON) must take into account a number of factors: (1) an accurate diagnosis, (2) consistent staging of the disease process, (3) understanding of the variability of the disease, (4) consistent application of the treatment modality (or the surgical technique), and (5) a clear understanding of the goal of the treatment used. This article reviews the important steps of a diagnostic algorithm that has been used to accurately diagnose and stage the disease process of ON. A consistent surgical technique with clearly defined goals is also

Orbital decompression for dysthyroid eye disease is traditionally performed either through an external approach or transantrally. The introduction of endoscopic sinus surgery over the last decade has led to a better understanding of the intranasal surgical landmarks of orbital walls and the development of endoscopic orbital decompression. We have adopted the endoscopic technique as the treatment of choice for orbital decompression in dysthyroid eye disease for the last two years. The surgical technique is described and illustrated, and the results of our first series of 15 patients (30 orbits)

Objectives: Transforaminal endoscopic lumbar foraminotomy (TELF) is an emerging minimally invasive surgical technique for lumbar foraminal stenosis. However, its effectiveness is debated because of concerns regarding adequate decompression and its long-term consistency. This study introduced the extended form of TELF, an advanced technique, to provide more extensive decompression using the same approach. Thus, this study aimed to describe the surgical technique and clinical outcomes of this technique. Methods: This retrospective cohort study included patients who underwent conventional (n = 67

OBJECT Decompression without fusion for degenerative lumbar stenosis is an effective treatment for both the pain and disability of neurogenic claudication. Iatrogenic instability following decompression may require further intervention to stabilize the spine. The authors review the incidence of postsurgical instability following lumbar decompression, and assess the impact of surgical technique as well as study design on the incidence of instability. METHODS A comprehensive literature search was performed to identify surgical cohorts of patients with degenerative lumbar stenosis, with and witho

Lumbar canal stenosis (LCS) and lumbar foraminal stenosis (LFS) are frequently observed in elder patients. For patients with radiculopathy caused by both LCS and LFS, surgical manipulations require nerve root decompression from its exit zone to the lateral part of the vertebral foramen, and often need spinal instrumentation. In the present report, we performed a new strategy of surgical decompression without spinal fixation. An 81-year-old woman suffered from bilateral buttock pain, left leg pain and right leg numbness in the L4 and L5 nerve root distribution. MRI and CT scans showed LCS at L3

The spine is the most common site of bone metastases from malignant tumors, with metastatic epidural spinal cord compression occurring in about 10% of patients with spinal metastases. Palliative radiotherapy and simple laminectomy and decompression have been the main treatments for metastatic spinal cord compression. The former is ineffective and delayed for radiation-insensitive tumors, and the latter often impairs spinal stability. With the continuous improvement of surgical techniques and instrumentation in recent years, the treatment model of spinal metastases has changed a lot. Decompress

Orbital decompression is a surgical procedure aimed at increasing the orbital volume and/or decreasing the volume of the orbital fat. The indications for orbital decompression are determined in the course of thorough eye examination. An important objective of examination of a patient with thyroid eye disease (TED) is determination of inflammation activity and severity. Orbital decompression is a surgical procedure that can be performed in both the active and nonactive stages of the disease. However, the indications for the surgery in these cases are different. Optic neuropathy and severe corne

Lumbar spinal decompression surgery is commonly performed to relieve radicular symptoms to good effect. The formation of post-operative spinal subdural hygroma, or spinal subdural extra-arachnoid hygromas (SSEH) following incidental intra-operative durotomies, have been described in previous literatures. We report a case of a 63 years old lady who had initially underwent an uneventful posterior lumbar decompression for lower limb radiculopathy, without intraoperative durotomy, with development of concurrent SSEH and spinal subarachnoid hygroma (SSSH) during the early post-operative period with

Endoscopic lumbar decompression has become an important minimally invasive technique for the treatment of lumbar spinal stenosis (LSS). Decompression of adjacent levels often requires multiple skin incisions or portals. Strategies to reduce surgical footprint while maintaining effective neural decompression remain an important goal in minimally invasive spine surgery. To our knowledge, single-incision uniportal bilateral decompression at two adjacent lumbar levels has not been formally described. We describe a single-incision uniportal endoscopic technique that allows bilateral decompression o

OBJECTIVE: A randomized controlled trial was reported recently, in which simple decompression and anterior subcutaneous transposition were compared for treatment of ulnaropathy at the elbow. Clinically, both surgical options seem to be equally effective. The objective of this study is to compare the costs, from a societal standpoint, of simple decompression versus anterior subcutaneous transposition in Euros.
METHODS: Published and unpublished data from a randomized controlled trial that compared simple decompression and anterior subcutaneous transposition were used. The investigators tabulat

Lumbar spinal stenosis (LSS) is a common disease in the elderly, mostly due to degenerative changes in the lumbar spinal complex. Decompression surgery is the standard surgical treatment for LSS. Classically, total laminectomy-which involves resection of the spinous process, entire laminae and medial facet-has been the standard decompression technique; however, it can cause post-surgical instability. To overcome this disadvantage, various minimally invasive techniques that preserve the stabilization structures of the spine have been developed, and surgeons have begun to re-evaluate decompressi
