Fibrosis is a common pathological feature observed in muscle from patients with Duchenne muscular dystrophy and in mdx diaphragm. The purpose of this study was to determine whether pentoxifylline (PTX) treatment for 4 weeks (16 mg/kg/day) could significantly attenuate the process of fibrosis in diaphragm muscle from mdx mice. Duchenne muscular dystrophy is characterized by myofiber necrosis, muscle replacement by connective tissue, and crippling weakness. OBJECTIVES: To examine whether diaphragmatic plication is an effective and lasting treatment option for non-malignant diaphragmatic paralysis.
METHODS: Nineteen patients who had undergone diaphragm plication (1983-1990) were recalled for interview, pulmonary function testing and chest X-ray.

METHODS: The effects of dantrolene (10(-8) to 10(-4) M) were investigated in vitro on diaphragm muscle strips in adult rats and in postnatal rats aged 3, 10, and 17 days, and compared with those of ryanodine (10(-8) to 10(-6) M). We describe two cases of Budd-Chiari syndrome detected by magnetic resonance imaging that resulted from compression of the inferior vena cava by an elevated right hemidiaphragm. Magnetic resonance images demonstrated elevation of the right hemidiaphragm and medial deviation of the inferior vena cava with short segmental narrowing. The hepatic veins and inferior vena cava were patent but discontinuous. Hepatic venous drainage was assisted by multiple large intrahepatic collaterals.

Over a 2-year period (1987-1988), 31 children (3% of 960 operations) were found to have a paralysed diaphragm following cardiac surgery. The median age was 5 months with 65% less than 1 year of age. The histories of 67 patients with a diaphragmatic rupture due to blunt trauma were reviewed in four hospitals. In 45 patients the diagnosis was made within 24 hours after the accident, in the other 22 patients the rupture was diagnosed in a later stage. HYPOTHESIS: The interaction of large, membrane-bound vesicles leads to the development of extended diaphragms that act as intermediates in the fusion process. Such hemifusion diaphragms confined inside a stiffer container may disassemble through energetically distinct rim-pore expansion modes (radial or lateral), and the preferred mode depends on diaphragm size and lipid reabsorption costs. We propose a theoretical model based on constrained-wetting physics to explain why large hemifusion diaphragms favor lateral rim-pore propagation while small hemifusion diaphragms prefer radial growth.

Diaphragm dysfunction that leads to respiratory failure is a significant clinical consequence of sepsis-induced critical illness.
