Infection related to closed diaphyseal femoral fractures treated with external fixation and subsequent internal fixation at a reference hospital: A cross-sectional study.

INTRODUCTION: The definitive management of fractures is not completely unhazardous, with fracture-related infection (FRI) being one of the complications. The main objective of this study was to determine the prevalence of FRI in patients with closed femur fractures treated with initial external fixation followed by definitive fixation at a reference hospital. MATERIALS AND METHODS: A cross-sectional observational study was conducted over a seven-year period. Patients aged 18 to 65 years with closed diaphyseal femoral fractures were included. FRI was defined according to the international cons

Outcomes of modular diaphyseal vs. nonmodular metaphyseal reverse total shoulder arthroplasty stem for 3- and 4-part proximal humerus fractures.

BACKGROUND: Proximal humerus fractures (PHFs) are increasingly prevalent in the aging population, with reverse shoulder arthroplasty (rTSA) emerging as a preferred surgical treatment for complex fracture patterns. Recent advancements have introduced shorter, metaphyseal-fixation humeral stems as alternatives to traditional modular diaphyseal stems. However, limited data exist comparing the clinical outcomes of these designs following rTSA for PHF. METHODS: A retrospective analysis of 99 patients undergoing rTSA for acute three- or four-part PHFs between 2014 and 2024 was conducted. Patients r

Distal Humerus Metaphyseal-Diaphyseal Junctional Fracture of a Four-Year-Old Child Treated With Intramedullary Steinmann Wire Fixation: A Case Report.

Distal humeral metaphyseal-diaphyseal fractures are rare and inherently unstable injuries. Non-operative treatments can make it hard to maintain reduction. Open or closed reduction with percutaneous K-wire fixation may be the preferred treatment option for these fracture types. This case report describes successfully managing a rare distal humerus metaphyseal-diaphyseal junctional (MDJ) fracture in a four-year-old child using intramedullary Steinmann wire fixation. A four-year-old male child applied to the emergency service with a swollen elbow. He had a history of trauma 10 days ago. There wa

Incomplete Diaphyseal Atypical Femoral Fracture due to Increased Anterolateral Bowing: Treatment with Corrective Osteotomy and Intramedullary Nailing with Augmented Plate Fixation.

BACKGROUND: Increased anterolateral bowing of the femur is associated with atypical femoral fractures among older patients (including both those receiving and those not receiving treatment with bisphosphonates) as well as a greater likelihood of occurring in the diaphyseal versus the subtrochanteric region. Prophylactic intramedullary (IM) nailing is recommended for the treatment of incomplete atypical femoral fractures, which are radiographically evident. However, this bowing deformity is considered to be a major obstacle to IM nailing as it may be associated with postoperative complications.

Masquelet's induced membrane technique associated with Reamer Irrigation Aspiration grafting and intramedullary Nailing (MaRIAN) for chronic diaphyseal osteomyelitis of the lower limb.

INTRODUCTION: Masquelet's induced membrane technique offers a definitive contribution to the treatment of diaphyseal osteomyelitis. To overcome its drawbacks while maintaining its principles, technical modifications have been proposed: antibiotic cement, femoral intramedullary autograft harvested by RIA (Reamer Irrigation Aspiration) and interlocking nails. MATERIAL AND METHOD: This retrospective study gathered patients with chronic osteomyelitis of the femur or tibia. The first surgical stage consisted of bone resection in the healthy zone and use of a gentamicin cement spacer to fill the bo

Long-term Evaluation Using Finite Element Analysis of Bone Atrophy Changes after Locking Plate Fixation of Forearm Diaphyseal Fracture.

PURPOSE: To determine the optimal timing of plate removal in patients with forearm diaphyseal fractures fixed with a locking plate via the analysis of bone atrophy over time. METHODS: The study subject was a 56-year-old man. Computed tomography was performed at 0.5, 1, 1.5, 2, 3, 4, and 5 years after plate fixation. Finite element analysis was performed to measure the fracture load of the radius and ulna. The fracture loads of the affected and healthy sides were compared, and their ratio was calculated by dividing the value of the affected side by that of the healthy side at each time point.

Uncoated vs. Antibiotic-Coated Tibia Nail in Open Diaphyseal Tibial Fracture (42 according to AO Classification): A Single Center Experience.

Implant-associated infections remain one of the main problems in the treatment of open tibia fractures. The role of systemic antibiotic prophylaxis is now agreed and accepted; nevertheless, recent literature also seems to emphasize the importance of local antibiotic therapy at the fracture site. Several therapeutic strategies have been proposed to overcome this new need. Antibiotic-coated nails play crucial role in this, allowing both infection prevention and favoring the fracture stabilization. We describe the outcome of patients with open diaphyseal tibia fracture treated either with a stand

Diaphyseal femoral fracture due to severe vitamin D3 deficiency and low parathyroid hormone levels on long-term hemodialysis: a case report.

INTRODUCTION: Currently, there are no reports of diaphyseal femoral fracture equivalent to atypical femoral fractures (AFFs) in patients receiving long-term hemodialysis (HD). CASE REPORT: A 56-year-old Japanese man receiving long-term HD for 34 years was admitted to our hospital due to a delay in postoperative healing. The patient began maintenance hemodialysis at 22 years of age. The patient then underwent surgical parathyroidectomy (PTX) for secondary hyperparathyroidism at 43 years of age, which resulted in decreased levels of parathyroid hormone (PTH). Thereafter, this patient's serum 1,

A new case of Greenberg dysplasia and literature review suggest that Greenberg dysplasia, dappled diaphyseal dysplasia, and Astley-Kendall dysplasia are allelic disorders.

BACKGROUND: Greenberg dysplasia is a rare, autosomal recessive, prenatal lethal bone dysplasia caused by biallelic pathogenic variants in the lamin B receptor (LBR) gene. Pathogenic variants in LBR are also associated with Pelger-Huët anomaly, an autosomal dominant benign abnormality of the nuclear shape and chromatin organization of blood granulocytes, and Pelger-Huët anomaly with variable skeletal anomalies, a mild, regressing to moderate-severe autosomal recessive condition. Conditions with abnormal sterol metabolism and different genetic basis have clinical and radiographic features simila

Risk factors of non-union in intramedullary stabilized diaphyseal long bone fractures: identifying the role of fracture stabilization strategies and concomitant injuries.

PURPOSE: Concomitant chest injury is known to negatively affect bone metabolism and fracture healing, whereas traumatic brain injury (TBI) appears to have positive effects on bone metabolism. Osteogenesis can also be influenced by the timing of fracture stabilization. We aimed to identify how chest injuries, TBI and fracture stabilization strategy influences the incidence of non-union. METHODS: Patients with long bone fractures of the lower extremities who had been treated between 2004 and 2014 were retrospectively analysed. Non-union was defined as fracture healing not occurring in the expec

Treatment of diaphyseal pathological fractures in children with monostotic fibrous dysplasia using cortical strut allografts and internal plating: A retrospective clinical study.

Children with fibrous dysplasia (FD) chronically suffer from pain, pathological fractures, and limb deformities. The most effective methods for managing the associated pathological fractures remain controversial. The purpose of this study was to evaluate the clinical results of the treatment of diaphyseal pathological fractures in children with monostotic fibrous dysplasia (MFD) using cortical strut allografts and internal plating. We retrospectively analyzed outcomes in nine children (5 boys, 4 girls) with diaphyseal pathological fractures due to MFD, who were treated with cortical strut allog

Femoral malrotation from diaphyseal fractures results in changes in patellofemoral alignment and higher patellofemoral stress from a finite element model study.

BACKGROUND: Malrotation of the femur is a frequent complication in the management of a diaphyseal fracture. It is often responsible for pain and adverse functional results. Among these complications, contact stress effects on the patellofemoral joint are recognized as predictive factors of impaired results. The purpose of this study was to analyze the effect of malrotation on stress distribution on the patellofemoral joint, using radiological measurement and three-dimensional finite element models. METHODS: Functional analysis of the patellofemoral joint was evaluated in eight knee pairs from

Does morbid obesity negatively affect the hospital course of patients undergoing treatment of closed, lower-extremity diaphyseal long-bone fractures?

Obesity is prevalent in the developed world and is associated with significant costs to the health care system. The effect of morbid obesity in patients operatively treated for long-bone fractures of the lower extremity is largely unknown. The National Trauma Data Bank was accessed to determine if morbidly obese patients (body mass index >40) with lower extremity fractures have longer length of hospital stay, higher cost, greater rehabilitation admission rates, and more complications than nonobese patients. We identified patients with operatively treated diaphyseal femur (6920) and tibia (5190

Influence of cephalomedullary nail length and caput-collum-diaphyseal angle on tip-apex distance and early mechanical cut-out in trochanteric femur fractures.

BACKGROUND: Early mechanical cut-out remains one of the most severe complications after cephalomedullary fixation of trochanteric femur fractures. While tip–apex distance (TAD) and lag screw position are well-established predictors, the independent influence of nail length and the caput–collum–diaphyseal (CCD) angle remains unclear. This question is clinically relevant because implant standardisation may constrain screw trajectory in anatomically variable proximal femora and thereby hinder optimal TAD attainment and potentially increase cut-out risk. METHODS: We retrospectively analysed 373 co
