Risky dieting amongst adolescent girls: Associations with family relationship problems and depressed mood.

OBJECTIVE: This study examined the association of risky dieting amongst adolescent girls with depressed mood, family conflict, and parent-child emotional closeness. METHOD: Grade 6 and 8 females (aged 11-14years, N=4031) were recruited from 231 schools in 30 communities, across three Australian States (Queensland, Victoria, and Western Australia). Key measures were based on the Adolescent Dieting Scale, Short Mood and Feelings Questionnaire, and widely used short measures of family relationship quality. Controls included age, early pubertal onset, and socioeconomic status. RESULTS: Risky die

Therapy of nonalcoholic fatty liver disease: current status.

Nonalcoholic fatty liver disease (NAFLD), the hepatic manifestation of the metabolic syndrome, has become a common entity in clinical practice. In most of the patients it presents as simple steatosis with nonprogressive clinical course. However, some patients have progressive form of NAFLD, nonalcoholic steatohepatitis (NASH), and are at increased risk of developing cirrhosis and hepatocellular carcinoma. NAFLD treatment includes lifestyle modifications and pharmacotherapy aiming at increasing insulin sensitivity, and attenuating inflammation and hepatic fibrosis. Weight reduction has consiste

A satisfaction enhancement intervention for long-term weight loss.

OBJECTIVE: To compare the effects of two satisfaction enhancement interventions for weight control on behavioral and weight changes among individuals in a 15-month weight loss program. The primary hypothesis was that long-term weight loss would be improved by an intervention that enhances perceived satisfaction with achieved outcomes. RESEARCH METHODS AND PROCEDURES: Men and women (331) were randomized to weight control programs comprised of diet and exercise counseling and a cognitive intervention that emphasized either comparing experienced outcomes with expectation of ideal outcomes or com

Breastfeeding, food choices, restrictive diets, and nutritional fads.

In the past year, a number of studies were published that illustrated the importance of dietary choices at all ages and singled out pediatricians as useful and vociferous advocates for healthy nutrition in children. Obesity continues to top the list as a major threat to the health of children and adults worldwide and is increasingly being recognized as a direct result of inappropriate nutritional intake. Parental misperceptions and misinformation about their children's diets beginning in infancy and stretching into the college-age years may represent prime targets for pediatricians ready and a

The skinny on high-protein, low-carbohydrate diets.

Short-term studies of high-protein, low-carbohydrate diets have shown weight loss and improvements in plasma lipid profiles. Studies of greater than 6 months' duration, however, have failed to show continued benefit of high-protein, low-carbohydrate diets on weight loss and cardiovascular risk factors compared with conventional diets. Without concurrent weight loss and caloric restriction, these diets offer no additional benefit to lipids or body weight over other weight-loss regimens. In fact, high-protein, low-carbohydrate diets may add additional risk to individuals with cardiovascular dise

Systematic Review and Meta-analysis of Randomized Clinical Trials Comparing Time-Restricted Eating With and Without Caloric Restriction for Weight Loss.

CONTEXT: Although it is well established that caloric restriction (CR) is the primary driver of weight loss, circadian-driven metabolic benefits have been recognized as possibly enhancing the effects of CR. Time-restricted eating (TRE) has emerged as a promising approach in this context. OBJECTIVE: We conducted a systematic review and meta-analysis to compare the effects of TRE with isocaloric diet controls (analysis 1) and non-isocaloric controls (analysis 2) on anthropometric and body-composition parameters in adults with overweight or obesity. DATA SOURCES: A search was carried out in the

Effects of a Short-Term Meal Replacement Hypocaloric Diet in Subjects with Obesity and High Fatty Liver Index.

INTRODUCTION: Dietary changes play a role in metabolic response of patients with metabolic-associated fatty liver disease, and there is little evidence on the use of partial meal replacement (pMR) diets in this pathology. AIM: We decided to evaluate the modifications in transaminases levels after a pMR hypocaloric diet in subjects with obesity and elevated fatty liver index (FLI). MATERIAL AND METHODS: A sample of 606 patients with obesity and FLI ≥ 60 were enrolled and treated during 3 months with a pMR diet. Patients were divided as group I (Alanine amino transferase (ALT) normal) or group

[Combined therapy with weight loss and amiodarone improved cardiac function in a patient with idiopathic dilated cardiomyopathy complicated with severe obesity: a case report].

Obesity is associated with an increased risk of development of chronic heart failure, but recent epidemiological studies indicate that a higher body mass index (BMI) is associated with a better survival rate. This is described as the 'obesity paradox' or 'reverse epidemiology'. A 42-year-old male was admitted because of recurrent episodes of decompensated heart failure, and the diagnosis was idiopathic dilated cardiomyopathy complicated with severe obesity (BMI 46.0), nonsustained ventricular tachycardia, and central type sleep apnea syndrome. Combined therapy with weight loss (BMI 46.0 to 30.

Sibutramine as an adjuvant therapy in adolescents suffering from morbid obesity.

BACKGROUND: The prevalence of morbid obesity is increasing rapidly. Weight reduction is very difficult using diet restriction and physical activity alone. Sibutramine has been shown to be effective and safe as an adjuvant therapy to diet restrictions. OBJECTIVES: To describe our experience using sibutramine in weight reduction treatment of adolescents suffering from morbid obesity. METHODS: The study group comprised 20 young persons (13 females, mean age 15 years 4 months, range 13-18 years) with morbid obesity (body mass index above the 95th percentile for age and/or > or =30 kg/m2) who wer

Intensive therapies for pediatric obesity.

The intensive therapies that have been used to treat pediatric and adolescent obesity include very-low-calorie diets, pharmacotherapy, and bariatric surgery. None of these approaches have been reported in sufficient numbers of subjects who have taken part in well-designed experiments with long-term follow-up to demonstrate convincingly their true value for the treatment of pediatric obesity. As the potency of the therapy increases, so does its possible adverse consequences. Of the intensive approaches, only bariatric surgery can be said to have even small studies supporting its ability to indu

Stress and food choice: a laboratory study.

OBJECTIVE: This study investigated experimentally whether acute stress alters food choice during a meal. The study was designed to test claims of selective effects of stress on appetite for specific sensory and nutritional categories of food and interactions with eating attitudes. METHODS: Sixty-eight healthy men and women volunteered for a study on "the effects of hunger on physiology, performance, and mood." Eating attitudes and food preferences were measured on entry to the study. The stressed group prepared a 4-minute speech, expecting it to be filmed and assessed after a midday meal, alt

Clinical implications of psychosocial research on bulimia nervosa and binge-eating disorder.

Bulimia nervosa and binge-eating disorder are characterized by a persistent course, are associated with comorbid psychopathology, and can result in serious medical complications. Although current treatments for these disorders show promise, they are not effective for approximately 40% of clients. Significant advances have been made in psychosocial research on the etiology and maintenance of bulimia nervosa and binge-eating disorder, as well as on the predictors of treatment response. This article reviews these advances and discusses the clinical implications of these findings. Research on etio

Inhibition of eating behavior: negative cognitive effects of dieting.

This study tested the hypothesis that dieters would score higher than nondieters in terms of food rumination. Two hundred and thirty one college undergraduates completed the Eating Obsessive-Compulsiveness Scale (EOCS) and responded to a questionnaire that inquired about dieting status. Subjects also completed measures that tapped neuroticism and social desirability. Results showed that current dieters were significantly more obsessed with thoughts of eating and food than were nondieters. Neither dieting status nor EOCS scale scores were related to neuroticism or social desirability. These res

Eating disorders revisited. II: Bulimia nervosa and related syndromes.

Bulimia nervosa and related syndromes are common, and occur in up to 5% of women who attend general practitioners. Young women in First World countries, particularly those who "diet", are at increased risk. Behaviours, such as binge eating and induced vomiting, are typically kept well hidden. Only a minority of those with these disorders present for treatment. General practitioners play a key role in primary and secondary prevention. Effective treatments include psychotherapies that focus on the patient's attitudes and relationships, not just the binge eating behaviour. About 50% of patients m

Depression, dietary restraint, and binge eating in female runners.

Female runners (n = 174) were assessed on their levels of dietary restraint, depression, and binge eating, as well as a number of exercise variables. Thirty-eight (19%) of the women in the sample were found to meet diagnostic criteria for DSM-IIIR Bulimia Nervosa. The level of exercise was unassociated with any of the affective and eating variables as was the risk for meeting diagnostic criteria for bulimia nervosa. Severity scores for depression in bulimic runners were notably lower than in earlier nonexercising samples. The relationships between the variables were similar to those found in p

Lipodystrophy and its treatment.

Lipodystrophy, almost exclusively seen in female patients, causes psychological problems at an early age. In later life, additional complaints are heavy painful legs, edema, and varicose veins. Nearly all patients suffer from alimentary adiposity. Dermolipectomies in general are not advisable because of the resulting large scars and the risk of damaging the lymphatic system. Subcutaneous lipectomy according to the Illouz method can provide good results. Weight loss is a good alternative to liposuction of the upper legs. After sufficient weight loss, only additional liposuction of the trochante

Long-term efficacy of medical treatments of obesity.

Obesity is a chronic, usually life-long condition. Therefore, the success of any treatment should be measured by the long-term weight loss. More patients lose weight than maintain the weight loss after the active phase of therapy has ended. Conservative approaches like dietary restrictions or behavior modification techniques have been only modestly successful in weight maintenance. Both are, therefore, suitable for patients with milder degrees of obesity. Anorexicants seem to have specific but limited use in the treatment of obesity. Fasting and protein sparing modified fast are indicated for
